One-bone forearm reconstruction using vascularized fibular graft for massive forearm soft-tissue and bone defect: case report.

The authors report a case of recurrent malignant fibrous histiocytoma with extensive bone- and soft-tissue involvement, successfully treated with one-bone forearm reconstruction using a vascularized fibular graft and multiple tendon transfer. Twenty-four months after surgery, elbow and hand functions were maintained, and the patient had no disturbance of hand function in daily activity, although rotation of the forearm was sacrificed. No local recurrence or metastasis was noted. The procedure is a useful reconstructive option for complex tissue defects following wide excision of sarcoma of the…

Nerve grafting for traction injuries of the common peroneal nerve. A report of 17 cases.

We report 17 patients who had grafting of the common peroneal nerve after traction injuries. Sixteen were reviewed at a median follow-up of 36 months. The nerve gap ranged from 7 to 20 cm. A functionally satisfactory result was achieved in six patients (37.5%), a fair result in six and a poor result in four. Seven patients had, in addition, a posterior tibialis tendon transfer; this improved the result in five. Grafting produced some satisfactory results even when the nerve gap was as long as 20 cm.

Use of free serratus anterior muscle slips for the reconstruction of dorsal-side defects of the hand resulting from hot press injury.

Mutilation of the hand as a result of hot press injury, the common characteristics of which are extensive soft tissue and extensor tendon loss, metacarpal and phalangeal necrosis, exposition of multiple joints, and infection, presents a serious challenge to the hand surgeon. Free transfer of the inferior three slips of the serratus anterior muscle is a useful surgical option for the reconstruction of dorsal-side defects in the hand. The versatility of the three separate slips, which are easily divisible for contouring, enables individual reconstruction of the different digits. Long vascular pe

Mechanical strength of the side-to-side versus Pulvertaft weave tendon repair.

PURPOSE: The side-to-side (SS) tendon suture technique was designed to function as a repair that permits immediate postoperative activation and mobilization of a transferred muscle. This study was designed to test the strength and stiffness of the SS technique against a variation of the Pulvertaft (PT) repair technique. METHODS: Flexor digitorum superficialis (FDS) and flexor digitorum profundus (FDP) tendons were harvested from 4 fresh cadavers and used as a model system. Seven SS and 6 PT repairs were performed, using the FDS as the donor and the FDP as the recipient tendon. For SS repairs,

Operative Management of Noninsertional Tendinosis: Indications and Current Treatment.

Noninsertional Achilles tendinopathy is a degenerative condition caused by chronic mechanical overload, commonly affecting athletes and active individuals. While nonoperative treatment is preferred, approximately one-third of cases require surgery. Various surgical options exist, including debridement, tendon scraping, paratenon release, and flexor hallucis longus transfer, with open and minimally invasive techniques yielding comparable outcomes. The condition's pathophysiology remains unclear, complicating treatment standardization. Given the lack of high-quality comparative studies, surgical

Treatment options for locked posterior shoulder dislocations and clinical outcomes.

Posterior dislocations are rare and diagnostically difficult injuries. Diagnosis is often delayed and this leads to a locked posteriorly dislocated humeral head.Treatment options include conservative methods and surgical anatomic reconstruction options as well as non-anatomic surgical procedures such as subscapularis tendon transfer, hemiarthroplasty and total shoulder arthroplasty.Decision-making for treatment as well as prognosis depend on the extent of the articular defect size of the humeral head, duration of the dislocation and patient-specific conditions such as age and activity levels. …

Ankle instability and arthroscopic lateral ligament repair.

Over the last 50 years, the surgical management of chronic lateral ankle ligament insufficiency has focused on 2 main categories: local soft-tissue reconstruction and tendon grafts/transfer procedures. There is an increasing interest in the arthroscopic solutions for chronic instability of the ankle. Recent biomechanical studies suggest the at least one of the arthroscopic techniques can provide equivalent results to current open local soft-tissue reconstruction (such as the modified Brostrom technique). Arthroscopic lateral ankle ligament reconstruction is becoming an increasingly acceptable

Pull-out technique for plantar plate repair of the metatarsophalangeal joint.

Plantar plate rupture is a common cause of forefoot pain, multiplanar malalignment, subluxation, or dislocation of the metatarsophalangeal joint (MTPJ). The treatments that have been described for MTPJ instability of lesser toes include amputation, lengthening and/or tendon transfer, periarticular soft-tissue release (capsule, collateral ligaments, and plantar plate), colateral ligament reconstruction, metatarsal shortening osteotomy, and suture of plantar plate lesion. This article outlines the anatomy, pathogenesis, and diagnosis of plantar plate rupture, and describes a new technique that c…

Advanced Hirayama disease with successful improvement of activities of daily living by operative reconstruction.

Juvenile muscular atrophy of the distal upper extremity (Hirayama disease) is a benign and non-progressive motor neuron disease. Application of a cervical collar is believed to prevent progression of symptoms in the early stages, but there is no effective therapy for the advanced disease. We found that tendon transfer improved the activities of daily living (ADL) of a patient with advanced Hirayama disease. An operative reconstruction can be valuable, even in patients with Hirayama disease who have developed impaired ADL due to extensive intrinsic hand muscle atrophy.

Small finger pollicization in the radial club hand.

This article describes pollicization of the small finger. A procedure that should only be used in the rare instance of an inoperable radial club hand. Children with bilateral radial club hands and severe, fixed radial deviation of the hand at the end of the forearm, and stiff elbows that cannot be mobilized either with a tendon transfer, splinting, or triceps release can be considered candidates for this operation. The technique, as described by Dieter Buck-Gramcko for pollicization of the index finger, was modified to be used in this procedure.

Supination deformity of the forefoot.

Supination deformity of the forefoot is rare. Observations of eleven cases and the production of the deformity in rabbits suggest that loss of function of the peroneus longus muscle in combination with the effect of a strong tibialis anterior muscle is decisive for the appearance of the deformity. Primary conditions causing deformity were: spina bifida, talipes equinovarus, poliomyelitis, trauma and muscular dystrophy. Treatment by wedge resection of the first cuneiform and the first metatarsal bones and transfer of the tibialis anterior tendon was successful. Operations for supination deformi

Dorsal bunions in children.

Iatrogenic dorsal bunions in children, observed mostly after operations for post-poliomyelitic paralysis or club foot, have been successfully treated by an operation in which the tendons of the flexor brevis and the abductor and adductor muscles of the hallux are transferred to the neck of the first metatarsal. This is supplemented by removal of the sesamoids, capsulotomy of the metatarsophalangeal joint, and when necessary by arthrodesis or tenodesis of the metatarsophalangeal joint. In seventeen feet treated in this way only one failure, in a patient with arthrogryposis, was recorded.

De novo generation in an in vivo rat model and biomechanical characterization of autologous transplants for ligament and tendon reconstruction.

BACKGROUND: Surgical reconstruction of ligaments and tendons is frequently required in clinical practice. The commonly used autografts, allografts, or synthetic transplants present limitations in terms of availability, biocompatibility, cost, and mechanical properties that tissue bioengineering aims to overcome. It classically combines an exogenous extracellular matrix with cells, but this approach remains complex and expensive. Using a rat model, we tested a new bioengineering strategy for the in vivo and de novo generation of autologous grafts without the addition of extracellular matrix or

Longus colli tendonitis, clinical consequences of a misdiagnosis.

Numerous pathologies diagnosed in the emergency department (ED) are treated with invasive procedures involving anesthetic and surgical risks. Retropharyngeal abscess is a serious condition requiring emergent treatment, often in need of trans-oral incision and drainage under general anesthesia. A misdiagnosis, especially after surgical treatment, might generate undesirable consequences, more so if the final diagnosis is a non-surgical pathology such as longus colli (LC) tendonitis. To discuss the etiology, differential diagnosis and treatment of LC tendonitis, a clinical condition still misdiag

Newer concepts in the surgical management of lesions of the foot in the patient with diabetes.

Ulcerative lesions of the foot are commonly due to ischemia, neuropathy or a combination of these factors. When these lesions fail to respond to conservative measures, they are usually treated by ablative or destructive procedures which often result in the loss of a portion of the foot or deformities, or both. These procedures or deformities may, themselves, lead to further problems as the weight is transferred to other areas not designed for this purpose. Reconstructive procedures may prevent these problems, as well as conserve the foot. Lesions of the heel, middle of the foot and forefoot we

Dynadesis for Treatment of Dynamic Scaphoid Instability with 20-Year Results.

Background  Even though the scapholunate interosseous ligament is the most common wrist ligament injury, its treatment remains a challenge for hand surgeons. We report on a surgical treatment (Dynadesis) for dynamic scaphoid instability (DSI) with a 20-year follow-up period. Description of Technique  Dynadesis utilizes antagonist forearm muscles in order to synergistically provide dynamic stabilization to the scaphoid when the wrist is loaded. It is a tendon-to-tendon transfer with the following two components: 1) Dorsal-The extensor carpi radialis longus (ECRL) is passed through a hole in the…

The prevalence of the extensor indicis tendon and its variants: a systematic review and meta-analysis.

The tendon of the extensor indicis (EI) is frequently used to restore the loss of function in other digits. However, it shows many variations which include splitting of the extensor indicis proprius (EIP) into two or three distal slips, attachment to fingers other than the index such as the extensor medii proprius (EMP), attachment onto the index and the third finger such as the extensor indicis et medii communis, or attachment to both the index and the thumb such as the extensor pollicis et indicis (EPI). This systematic review gathers the available data on the prevalence of EI tendon and its
