[Mesoatrial shunt in Budd-Chiari syndrome].

BACKGROUND: Budd-Chiari syndrome (BCS) represents partial or total occlusion of the hepatic veins with or without simultaneous obstruction of vena cava inferior (VCI). The symptoms of BCS are abdominal pain, hepatomegaly, ascites, varices of the abdominal wall, sometimes bleeding from the upper part of gastointestinal tract (GIT), lower limbs swelling and jaundice. Primary BSC is a relatively rare condition occuring in one per 100,000 of the population worldwide. CASE REPORT: A male patient, 25-year-old, facing tooth postextraction complications, was presented with acute BCS. On admission, ph

Comparison of 2 commercially available chlorhexidine mouthrinses.

BACKGROUND: It has been shown that lowering the concentration of chlorhexidine (CHX) in mouthrinses from 0.2% to 0.12% does not adversely affect inhibition of plaque accumulation. The aim of this study was to evaluate the inhibition of plaque growth by 2 commercially available mouthrinses used according to the manufacturers' instructions, one with a 30-second and one with a 60-second rinsing time. METHODS: The clinical investigation was a single-blind, randomized study involving 80 volunteers (40 female, 40 male; mean age 25.7 years). At the start of the trial, all participants received a den

Finite element analysis of stress in mandibular molars repaired after fractured instrument removal.

BACKGROUND: This study assessed stress distributions in simulated mandibular molars filled with various materials after the removal of fractured instruments from the apical thirds of the root canals. METHODS: Finite element models of the mesial and distal root canals were created, where fractured instruments were assumed to be removed using a staging platform established with a modified Gates-Glidden bur (Woodpecker, Guangxi, P.R. China). Each canal was treated with different materials: Biodentine (Septodont, Saint Maur-des-Fosses, France), mineral trioxide aggregate (MTA), and gutta-percha.

Single-session use of mineral trioxide aggregate as an apical barrier in a case of external root resorption.

External root resorption may occur as a consequence of trauma, orthodontic treatment, bacterial infection or incomplete sealing of the root canal system (bacterial re-infection), and lead to crater formation on the resorbed apex. This would deform the root apex surface, and cause loss of apical constriction. Depending on the extent of the resorptive process, different treatment regimens have been proposed. A 34-year-old male patient presented with an intra-radicular retainer and an inadequate filling on tooth #21, as well as a radiographic image suggesting periapical bone rarefaction. After ro

Effects of matrix metalloproteinase inhibitors on bone resorption and orthodontic tooth movement.

Matrix metalloproteinases are involved in the regulation of bone remodeling. The hypothesis that matrix metalloproteinase inhibitors may be useful for experimentally limiting orthodontic tooth movement, a process involving perturbations of normal bone remodeling, was tested. General matrix metalloproteinase inhibitors limited the resorption of bone slices by mouse marrow cultures stimulated by calcitriol, parathyroid hormone, and basic-fibroblast growth factor. Pre-coating dentin slices with short arginine-glycine aspartic acid (RGD) peptides, but not arginine-glycine-glutamic acid (RGE) contr

Analysis of pulpal reactions to restorative procedures, materials, pulp capping, and future therapies.

Every year, despite the effectiveness of preventive dentistry and dental health care, 290 million fillings are placed each year in the United States; two-thirds of these involve the replacement of failed restorations. Improvements in the success of restorative treatments may be possible if caries management strategies, selection of restorative materials, and their proper use to avoid post-operative complications were investigated from a biological perspective. Consequently, this review will examine pulp injury and healing reactions to different restorative variables. The application of tissue

Proximo-cervical adaptation of Class II-composite restorations after thermocycling: a quantitative and qualitative study.

Sixty caries-free extracted human molars were used to determine the proximo-cervical adaptation and sealing of Class II-composite restorations related to the location of the cavity margins: 1.0 mm or 0.5 mm coronal to the cementum-enamel-junction (CEJ), at the CEJ, and 0.5 mm apical to the CEJ. All cavities were filled with a hybride type composite resin, which was combined at the CEJ and apical to the CEJ with a dentine adhesive in half of the specimens. The proximo-cervical adaptation and sealing was determined before and after thermocycling (TC, 2000 cycles, 5-55 degrees C) by SEM and dye p

Caries management strategies for primary molars: 1-yr randomized control trial results.

Minimal invasive approaches to managing caries, such as partial caries removal techniques, are showing increasing evidence of improved outcomes over the conventional complete caries removal. There is also increasing interest in techniques where no caries is removed. We present the 1-yr results of clinical efficacy for 3 caries management options for occlusoproximal cavitated lesions in primary molars: conventional restorations (CR; complete caries removal and compomer restoration), Hall technique (HT; no caries removal, sealing in with stainless steel crowns), and nonrestorative caries treatme

Partial caries removal in primary teeth: association of clinical parameters with microbiological status.

The relationship between clinical characteristics of carious dentin and bacterial colonization after partial caries removal is not completely understood. The aim of this study was to compare microbial counts between categories of carious dentin color, consistency and humidity, and to evaluate the correlation between these characteristics and the presence of cariogenic microorganisms in deep cavities (2/3 or more of the dentin thickness) submitted to partial caries removal. Sixteen primary teeth were submitted to the removal of all carious tissue from the lateral walls of the cavity, whereas ca

Nitric oxide in pulp cell growth, differentiation, and mineralization.

Dental preparation sometimes causes transient congestion, edema, and necrosis of the pulp. We hypothesized that nitric oxide (NO) is involved in the pathophysiological changes in pulp after preparation. The mRNA and protein expression of the inducible isoform of NO synthase (iNOS) was examined in murine pulp after dental preparation. The effects of NO on the proliferation, mineralization, and apoptosis of pulp cells were also studied in vitro. We found that not only iNOS, but also mRNAs for alkaline phosphatase and plasma membrane glycoprotein-1, were expressed in the pulp after preparation. N

Clinical considerations in case selection for all-ceramic fixed partial dentures.

All-ceramic systems have been utilized for fixed partial dentures (FPDs) to replace a missing tooth since the early 1990s. Clinical studies that assess the long-term outcome of both anterior and posterior three-unit all-ceramic FPDs are required to determine whether they can serve as a viable treatment option for functional and aesthetic tooth replacement. This article discusses the evolution and development of all-ceramic systems for FPDs as well as the clinical and laboratory considerations for treating patients using these restorations.
