Acute epiglottitis in adults: experience with fifteen cases.

Fifteen adults with acute epiglottitis are discussed. Three required tracheostomy because of delayed diagnosis. There were no deaths. Epiglottitis occurs more often in adults than is generally recognized. The early symptoms of epiglottitis in adults are sore throat and dysphagia. Any patient with acute, painful dysphagia should have indirect laryngoscopy to rule out epiglottitis. Throat and blood cultures were obtained from 14 of our cases. Cultures from only two patients were positive for Hemophilus influenzae, type B; cultures from the other 12 patients did not grow any bacterial pathogens.

Trachea-innominate artery fistula: successful management of 3 consecutive patients.

Trachea-innominate artery fistula is an uncommon but frequently fatal complication of tracheostomy. Three successive patients who developed this complication while receiving ventilatory assistance through a tracheostomy tube were successfully managed, with long-term survival. Bleeding was controlled by direct digital pressure on the innominate artery or by hyperinflation of the balloon cuff of the tracheostomy tube. In 2 patients, replacement of the tracheostomy tube with an orotracheal tube improved direct access to the innominate artery for digital compression. Late follow-up examination of

Laryngeal suspension and epiglottic flap in laryngopharyngeal trauma.

Physicans treating laryngeal trauma are still looking for ways to improve results. Severe disruption of the supporting structures between hyoid and trachea may allow stretching and narrowing of the laryngeal lumen during the healing period. This can easily be prevented by placing wires across the disrupted zone. In addition, loss of the epithelial lining of the larynx is a serious cause of stenosis. By dissecting out the epiglottic cartilage, one can create a sizeable, useful flap of mucous membrane based in the vallecula to use in relining of the anterior larynx.

Circulating calprotectin levels are associated with lung damage and disease outcome in patients with COVID-19.

OBJECTIVE: The present study aimed to investigate whether circulating calprotectin levels were associated with the extent of lung parenchyma … lizumab-treated patients revealed that rising levels during follow-up were associated with death or tracheotomy, independent of baseline values. CONCLUSION: Our findings support the clinical utility of circulating calprotectin as a complementary biomarker in COVID-19 management.

Transoral videolaryngoscopic surgery for hypopharyngeal anaplastic metastasis from papillary thyroid carcinoma.

We report an extremely rare case of hypopharyngeal metastasis with anaplastic transformation (anaplastic thyroid carcinoma) originating from … -mm mass on the posterior hypopharyngeal wall causing airway compromise, necessitating an emergency tracheotomy. Biopsy confirmed hypopharyngeal metastasis with anaplastic thyroid carcinoma. Given his comorbidities (chronic leukemia, severe pulmonary arterial hypertension), he underwent transoral videolaryngoscopic surgery to achieve local tumor control while …

Educational value of a live porcine model in head and neck surgery: A face and content assessment.

PURPOSE: Surgical training has evolved with the integration of simulation models to enhance skills and reduce complications during the learning process. This study aimed to evaluate the face and content validity of a live porcine model for head and neck surgery training, focusing on tracheotomy, neck dissection, and total laryngectomy. METHODS: A questionnaire was distributed to experienced otolaryngologists familiar with the porcine model and to residents who had trained with it. Using Likert-type scales, the survey assessed realism, anatomical accuracy, and perceived utility corresponding t…

Prevention of immunoglobulin on ventilator-associated pneumonia in critically ill children:a retrospective study.

BACKGROUND: Ventilator-associated pneumonia (VAP), a leading ICU-acquired infection, prolongs ICU stays, increases costs, and elevates morta … ith a substantially reduced risk of VAP (aOR = 0.448, 95 % CI: 0.288-0.697, p < 0.001). Conversely, tracheotomy, underlying conditions, and malnutrition were among the factors associated with an increased risk. CONCLUSIONS: IVIG administration within one month prior to ventilation may be associated with a reduced incidence of VAP.

Prediction model of ICU readmission in Chinese patients with acute type A aortic dissection: a retrospective study.

BACKGROUND: Readmission to the intensive care unit (ICU) remains a severe challenge, leading to higher rates of death and a greater financia … m the ICU. ICU readmission was predicted with age ≥ 65 years old, body mass index (BMI) ≥ 28 kg/m2, tracheotomy, continuous renal replacement therapy (CRRT), and the length of initial ICU stay were predictors of ICU readmission. The AUC was 0.837 (95%CI: 0.789-0.884) and the model fit the data well (H-L test, P = 0.519). DCA also demonstrated good clinical pr …

Multidisciplinary therapeutic strategy with appropriate timing and modalities for treating cervicofacial lymphatic malformations in children.

PURPOSE: The study reviewed a multidisciplinary approach to treating cervicofacial lymphatic malformations (CFLMs) in children. METHODS: Between 2007 and 2023, 53 children with CFLMs were treated with the median on-set age of 5 months (0-165) at our institute. For infants, airway management, including possible tracheotomy was prioritized, and a "wait-and-see" policy was adopted to expect spontaneous regression. Once children reached one year of age or diagnosed after infancy, OK-432 sclerotherapy and surgical treatment with/without sirolimus were considered for residual lesions. RESULTS: The…

Clinical Features of Severe Deep Neck Space Infection: Five Clinical Cases and Our Experience in Their Management.

BACKGROUND: Deep neck space infection (DNSI) is an acute and severe condition, with severe cases being relatively rare but considerably more … esented with diabetic ketoacidosis, and three had mediastinal involvement. Three patients underwent tracheotomy, while two required tracheal intubation. All patients were treated with neck incision, drainage, regular dressing changes, and targeted antibiotic therapy. A Fast-Track Respiratory Protocol for High Cervical Spine Injury: A Case Report.

BACKGROUND: Cervical spinal cord injury can greatly affect pulmonary function, resulting in complications, including respiratory failure wit … y transition to spontaneous mode, early tracheostomy and humane care, and high-flow oxygenation via tracheotomy after weaning off the ventilator. As a result, the patient experienced significant improvement in pulmonary function and was successfully liberated from the ventilator within a 2-week period. CONCLUSION: This case highlights the potential effective …
