The aim of this paper was evaluation of vertigo frequency in otosclerotic patients (group I N = 64) in comparison with control group (group II N = 20) and in people after stapedectomy (group III N = 64). Furthermore electronystagmographic recording spontaneous nystagmus, positional nystagmus were made. Computer analyses of ENG-recording and automatic calculation of directional preponderance and canal paresis were used. In recent years, different forms of physical therapy have been proposed for the treatment of benign paroxysmal positional vertigo (BPPV). These mainly consist of maneuvers aiming to reposition and disperse free-floating endolymph particles in the posterior semicircular canal. We report our experience with one of these procedures, the Epley maneuver (EM), in treating 30 cases of BPPV. Twenty-six patients (87%) were cured, while four (13%) did not respond to treatment. These results are similar to those reported in the literature for the EM and the Semont maneuver.

In order to detect endolymphatic hydrops, we applied three neuro-otologic examination methods, including the furosemide VOR test, the glycerol dehydration test, and electrocochleography in 61 patients with unilateral Menière's disease, 21 patients with delayed endolymphatic hydrops, and 10 patients with syphilitic labyrinthitis. Jumbling consists of loss of vestibular eye movement reflexes and resultant oscillopsia during movement of the head. This results in a failure of clear vision during head movement. In the course of initial examination the authors frequently observed a coincidence of labyrinthine symptoms with radiologic signs of chronic sinusitis. In 10 healthy individuals the influence of a rotating sound source (loudspeaker, narrow band noise 500 Hz, 70 dB SPL, 120 degrees/sec.) on the postrotatory nystagmus was investigated. If the loudspeaker was rotating in the opposite direction of the fast postrotatory nystagmus component, the duration and intensity of the postrotatory nystagmus was amplified by about 30%. If the loudspeaker was rotating in the direction of the fast postrotatory nystagmus component, no changes were seen.

Vestibular findings in a group of 35 patients with spasmodic torticollis without other otological or neurological symptoms were reviewed. The most consistent abnormality, present in more than 70% of cases, was a directional preponderance of vestibular nystagmus in the dark in a direction opposite to the head (chin) deviation. The results obtained by vestibular examination are reported for 20 patients with cerebellopontine angle tumours. Eye pursuits, spontaneous, positional and caloric induced nystagmus (lateral and directional preponderance for duration, frequency and speed of slow phase) were evaluated in order to identify the most sensitive parameter. The rupture of the round window membrane has been the origin of sudden deafness in two cases that are described in this presentation. We are able to verify the rupture of the round window membrane before surgery by using differential diagnostic criteria, though there existed only relative slight hearing losses of about 40 dB. After surgery we found normal hearing in one case and a decreased hearing loss in the other. Vestibular symptoms have been leading to the diagnosis in both cases and the middle ear effusion in one case.

To explain the origin of the narrow nystagmus writing (petite écriture), selected patients with hypoxemia were examined using the pendular rotating test and ENG. Bloodgas analysis was used to determine pO2 reduction. The decreased oxygen saturation was corresponding to the decreased amplitude of nystagmus. In a second experiment with alert rabbits the inspired gas mixture was altered to obtain hypoxia (8% O2 + 92% N2, controlled with bloodgas analysis). A technique to quantify the tandem Romberg test (posturography) was used to study ten patients with otosclerosis before (series 1), 48 hours after (series 2), and between two to four months after (series 3) a stapedectomy.
