[On the development of the infectology service in Novi Sad].

INTRODUCTION: Infectious diseases are a part of the history of this region. Devastating epidemics of plague, smallpox, and cholera were frequent during the 18th and the 19th centuries. Other infectious diseases were a serious problem as well: alimentary tract infections, scarlet fever, diphtheria, whooping cough. Geographic position, climate, migrations, as well as the tradition and lack of medical staff and medications, affected the frequency and outcome of infections. THE HISTORY OF THE TREATMENT OF INFECTIOUS DISEASES: Patients with infectious diseases were first treated at home. Later, a …

Mucosal vaccination with outer membrane vesicles derived from Bordetella pertussis reduces nasal bacterial colonization after experimental infection.

INTRODUCTION: We previously identified Bordetella pertussis-derived outer membrane vesicles (OMVs) as a promising immunogen for improving pertussis vaccines. In this study, we evaluated the efficacy of our vaccine prototype in immunization strategies aimed at reducing disease transmission by targeting colonization in the upper airways while maintaining protection against severe disease by reducing colonization in the lower respiratory tract. METHODS: We assessed different mucosal administration strategies in a murine model, including homologous mucosal 2-dose prime-boost schedules and heterol

Rapid Diagnosis of a Family Cluster of Pertussis Starting from a Child: a Case Report.

BACKGROUND: Pertussis, caused by Bordetella pertussis (B. pertussis), is a highly transmissible, acute respiratory disease that occurs in many countries. Diagnosis of pertussis continues to be a challenge using traditional tests due to their turn-around time and sensitivity. Herein, we rapidly and accurately screened a family cluster of pertussis from a child and her mother. METHODS: We used an automated nested multiplex PCR system which included B. pertussis, influenza A virus, and 19 other respiratory pathogens. RESULTS: We detected B. pertussis, influenza A virus H1-2009 (FluA-2009), aden

Early Protection against Pertussis Induced by Live Attenuated Bordetella pertussis BPZE1 Depends on TLR4.

Pertussis is a severe respiratory disease mainly caused by Bordetella pertussis Despite wide global vaccination coverage with efficacious pertussis vaccines, it remains one of the least well-controlled vaccine-preventable diseases, illustrating the shortcomings of the current vaccines. We have developed the live attenuated nasal pertussis vaccine BPZE1, currently undergoing clinical evaluation in human phase 2 trials. We have previously shown that in mice, BPZE1 provides strong and long-lasting protection against B. pertussis challenge by inducing potent Ab and T cell responses as well as secr

Serosurveillance for vaccine-preventable diseases: A look inside the pertussis experience.

INTRODUCTION: Serological surveillance (serosurveillance) provides the most direct measure of herd immunity of vaccine-preventable diseases. Little is known about the opportunities and challenges of serosurveillance experiences, particularly pertussis. OBJECTIVE: To describe the process of serosurveillance for vaccine-preventable diseases with an emphasis on the experience of pertussis in the metropolitan area of Antioquia (Valle de Aburrá) in 2015 and 2016 and analyze the contributions and challenges for its sustainability. MATERIALS AND METHODS: We described the planning and conduction of

The influence of neonatal Bacille Calmette-Guérin (BCG) immunisation on heterologous vaccine responses in infants.

INTRODUCTION: Bacillus Calmette-Guérin vaccine (BCG), one of the most widely used vaccines, does not only provide protection against tuberculosis and other mycobacterial infections, but also has non-specific (heterologous) immunomodulatory effects. In participants in a randomised trial, we investigated the effect of neonatal BCG immunisation on antibody responses to routine infant vaccines given in the first year of life. METHODS: Antibodies against antigens in the diphtheria, tetanus, pertussis, polio, Haemophilus influenzae type b (Hib), and the 13-valent pneumococcal conjugate vaccines wer

Development and validation of an Australian in-house anti-pertussis toxin IgG and IgA enzyme immunoassay.

AIMS: Although anti-pertussis toxin (PT) immunoglobulin G (IgG) is considered one of the most specific serological markers for Bordetella pertussis infection, there are few commercial kits available in Australia. We aimed to present the process of development, quality control and on-going clinical validation of an anti-PT IgG and IgA enzyme immunoassay (EIA) in use since 1999, and discuss the application of such tests in the diagnosis of B. pertussis infections. METHODS: A total of 1311 serum samples were used during multiple clinical validations from 1998 to 2010. The samples were drawn from

Importance of timing of maternal combined tetanus, diphtheria, and acellular pertussis (Tdap) immunization and protection of young infants.

BACKGROUND: Pertussis booster vaccine (Tdap) recommendations assume that pertussis-specific antibodies in women immunized preconception, during, or after previous pregnancies persist at sufficient levels to protect newborn infants. METHODS: Pertussis-specific immunoglobulin G (IgG) was measured by IgG-specific enzyme-linked immunosorbent assay (ELISA) in maternal-umbilical cord serum pairs where mothers received Tdap during the prior 2 years. Geometric mean concentrations (GMCs) of pertussis antibodies and cord-maternal GMC ratios were calculated. RESULTS: One hundred five mothers (mean age,

Reduced immunogenicity of diphtheria and tetanus toxoids when combined with pertussis toxoid.

The effect of pertussis toxoid on the immunogenicity of diphtheria and tetanus toxoids (DT) was studied during a double blind efficacy trial of an acellular pertussis vaccine. Infants received DT with or without pertussis toxoid at 3, 5 and 12 months of age. Geometric mean concentrations were higher in the DT than in the DT-pertussis toxoid group 1 month (diphtheria toxoid 4.76 versus 3.58 IU/mL, P = 0.009; tetanus toxoid 4.42 versus 2.66 IU/mL, P < 0.0001) and 2 years after the third injection (diphtheria toxoid 0.15 versus 0.10 IU/mL, P < 0.0001; tetanus toxoid 0.38 versus 0.18 IU/mL, P < 0.

Benefits and costs of vaccination.

Vaccination programmes supported by the WHO and other organizations have dramatically reduced disease in developing countries, but there remains a need to extend coverage rates and expand the number of vaccines included. In industrialized countries, where paediatric vaccination is almost universal, there will be an increasing opportunity to protect against diseases which occur in later years.

Evaluation of antibodies elicited by immunization with pertussis toxin.

The monoclonal antibody termed 1B7 neutralizes pertussis toxin in vivo in cell culture systems and can also passively protect mice from a challenge with live Bordetella pertussis (9). It has been suggested that most other independently derived neutralizing monoclonal antibodies recognizing the S1 subunit apparently recognize the same epitope as 1B7, and that the S1 subunit contains only one immunodominant protective epitope (1). These antibodies have been termed Class A antibodies (8) and inhibit the ADP-ribosyltransferase but not the NAD glycohydrolase activity of the toxin (7). We are testin

Common bacterial pneumonitis in infants. Determining the etiology and tailoring the treatment.

In infantile pneumonia, we recommend close attention to the history and physical examination. Baseline studies, including CBC, ESR, blood cultures, and chest film, should be performed at onset and repeated as warranted. Nasopharyngeal secretions or washings should be drawn by means of gentle suction and specimens sent for Gram stain, fluorescent antibody stain for respiratory syncytial virus, and culture for bacteria and for viruses if possible. Acute and convalescent serum specimens should be obtained in serious cases to search for antibodies to RSV, adenovirus, influenza, parainfluenza, cyto

A NEW APPROACH TO BACTERIAL VACCINES.

Immunizing antigens against only 10 bacterial diseases-cholera, diphtheria, paratyphoid, pertussis, plague, scarlet fever, staphylococcal disease, tetanus, tuberculosis and typhoid-have been licensed for sale in Canada and the United States. Convincing evidence of efficacy is available for only four of these-diphtheria and tetanus toxoids, and pertussis and typhoid vaccines. The principles which determine the efficacy of different immunizing antigens are not always the same. Toxoids, for example, stimulate the formation of antitoxin-producing mechanisms which can neutralize toxins produced by i

Antibody waning and Bordetella pertussis resurgence after the COVID-19 pandemic in the Netherlands.

After relative absence of Bordetella pertussis during the COVID-19 pandemic, notifications in the Netherlands were increased from May 2023 till September 2024. We monitored the largest pertussis outbreak in decades and investigated underlying immunological dynamics based on IgG and IgA antibodies against pertussis antigens (Ptx, FHA & Prn). We analyzed serum from 418 participants (2-87 years) at five timepoints (November 2022-October 2024) from a nationwide prospective serosurveillance study (PIENTER-Corona). Weighted incidence over two years was 6.3% (95% CI: 4.4-8.2) in the Dutch population,

Bordetella pertussis infection and antibody dynamics in household cohorts in two South African communities, 2016 - 2018: findings from the PHIRST study.

INTRODUCTION AND METHODS: In a cohort study, enrolling new households annually during 2016-2018, we estimated the B. pertussis attack rate using serology and PCR, identified factors associated with seroconversion, and described antibody changes over time. Nasopharyngeal swabs were collected biweekly for 6-10 months annually, with cases defined as individuals testing PCR-positive at least once. Seroconversion was defined as a ≥4-fold increase in anti-pertussis toxin IgG concentration between consecutive blood draws. Logistic regression was used to identify factors associated with seroconversion
