We describe a 58-year old patient with chronic lymphocytic leukemia (CLL) who developed systemic lupus erythematosus (SLE) with severe joint involvement. Dilated myocardiopathy precluded the use of high corticoid doses and a 15 days of prednisone (15mg/d) had no effect on the polyarthritis. Therefore, fludarabine (25mg/m2) was administered for 5 d. One month after the first cycle, fever, muscle stiffness and polyarthritis resolved. A total of 6 cycles were administered. We report the first successful reuse of a previously transplanted heart after perioperative brain death of the first recipient. The second recipient was a 66-year-old man suffering from end-stage ischemic cardiomyopathy. The intra- and postoperative course of the retransplantation was completely uneventful. Implantable programmable pump systems for insulin delivery to the peritoneal cavity or for intravenous insulin delivery have been recently developed. Thirty-one pumps were implanted in 25 patients between 1987 and 1991. At this writing, 76% of patients had functioning pumps. Ninety-two percent of pumps were functioning at 1 year; 89% at 2 years; and 50% at 3 years. No life-threatening complications, either surgical or metabolic, developed. Healthcare professionals, including practitioners of medical genetics and genetic counseling, have much to learn about the experiences of parents who are raising and caring for a child with a rare disease or developmental disability. Knowledge and understanding of the challenges in the care of a child with conditions such as Down syndrome and Wolf-Hirschhorn syndrome are at the core of the practice of genetic medicine. Insights into this experience can come from active listening to stories and from deep reading of memoirs and narratives authored by parents of children having these challenges.

BACKGROUND: The use of a phoropter for measuring phorias and vergences in children is common in the optometric profession. For young children, the use of the phoropter can be confusing, making it difficult to obtain accurate measurements. Free space testing allows for direct observation of the eyes in a natural environment and is easier for children to understand the directions. The normal values for phorias and vergences used with children are derived from testing with a phoropter or free space measurements with mostly adult patients.
