PURPOSE: To avoid anterior chamber lens implantation in aphakic eyes without capsular or zonular support, the laborious scleral fixation of a standard posterior chamber intraocular lens (IOL) is widely used, despite a large range of possible complications (macular edema, IOL tilt/decentration, suture erosion, vitreous hemorrhage). A sutureless iris-fixed ciliary sulcus implantation of a posterior chamber IOL (Binder-IOL, Fa. Iolution, Itzehoe) designed especially for such cases represents an alternative.

Main retinal complications of cataract extraction are retinal detachment and cystoid macular edema. Mouse mutants affecting lens development are excellent models for corresponding human disorders. The mutant aphakia has been characterised by bilaterally aphakic eyes (Varnum and Stevens, J Hered 1968;59:147-50); the corresponding gene was mapped to chromosome 19 (Varnum and Stevens, Mouse News Lett 1975;53:35). Postsurgical or spontaneous iris cyst is a rare finding. Unfortunately, all treatments are complicated and the results may be disappointing. We report a case of postsurgical iris cyst treated by sector iridectomy with satisfactory recovery of visual acuity.

Cefazolin (2.25 mg) was injected into the vitreous cavity of phakic, aphakic, and aphakic/vitrectomized rabbits; inflamed eyes were compared to controls. Vitreous levels of cefazolin were determined at selected times from 2 to 48 hr, and the half-life was calculated. The effect of inflammation was to increase the half-life or to reduce the rate of elimination of cefazolin from the vitreous cavity. One hundred and seventy five cerclage operations had been performed in the last 5 years; this was 85 times connected with an extrascleral invagination, Paufique's pocket was applied in 3 cases of macular holes and additionally in 3 cases an injection of SF6 gas was used. The axial length of 149 eyes (84 pseudophakic and 65 aphakic) was calculated by refractive means and compared with the axial length determined by ultrasound. The mean error (+/- SD) between the measured and the calculated axial length (measured minus calculated) was 0.02 mm (+/- 0.51) and 0.13 mm (+/- 0.54) for the aphakic and the pseudophakic eyes, respectively. The mean error was not significantly different from zero (P greater than .05). Calculation of the axial length may represent a valuable alternative to biometry for the aphakic and the pseudophakic eye.
