Consequences of intrauterine acidosis for early morbidity of term newborn infants.

OBJECTIVES: The study was designed to evaluate early morbidity of term newborn infants according to the umbilical artery pH and umbilical artery base excess. STUDY DESIGN: The study was based on the 1 173 infants who were born at term in the study period in 1997 with a UA-pH below 7.20. A retrospective review was carried out of maternal and infant records during the study period. All data were evaluated using SPSS for windows. RESULTS: Apgar-values and UA-pH correlated closely, the rate of 1-minute Apgar scores < 7 increased with decreasing UA-pH. The rate of 5-minute Apgar score < 7 increas

[A descriptive study of perinatal asphyxia and its sequelae].

INTRODUCTION: Perinatal asphyxia is a potential cause of brain injury that can produce some alterations on the neurologic development of the newborn. On the last years most part of the investigation have been focused on the physiopathology of the perinatal asphyxia, but correlation between asphyxia and brain damage is not well defined. PATIENTS AND METHODS: A retrospective study was made of the patients with the diagnosis of perinatal asphyxia born at the General Hospital of Segovia during a period of ten years (1992-2001). We took data about gestation, birth, neonatal period and follow-up pe

Continued reduction in the incidence of birth trauma and birth asphyxia related to instrumental deliveries after the study period: was this the Hawthorne effect?

BACKGROUND: The incidence of birth trauma and birth asphyxia related to instrumental deliveries in our obstetric unit was high (2.8%) in 1998-1999. A study was performed in 2000 to identify the risk factors. Unexpectedly, the incidence (0.6%) was reduced significantly during the study period. We attributed this phenomenon to the famous Hawthorne effect (tendency to improve performance because of awareness of being studied). OBJECTIVES: The objectives were to study whether there is a continued reduction in the incidence of birth trauma and birth asphyxia related to instrumental deliveries in t

Cerebral palsy.

Cerebral palsy (CP) is a group of disorders of movement and posture resulting from nonprogressive disturbances of the fetal or neonatal brain. More than 80% of cases of CP in term infants originate in the prenatal period; in premature infants, both prenatal or postnatal causes contribute. The most prevalent pathological lesion seen in CP is periventricular white matter injury (PWMI) resulting from vulnerability of the immature oligodendrocytes (pre-OLs) before 32 wk of gestation. PWMI is responsible for the spastic diplegia form of CP and a spectrum of cognitive and behavioral disorders. Oxida

Differential changes in peripheral and central components of the brain stem auditory evoked potentials during the neonatal period in term infants after perinatal hypoxia-ischemia.

To identify any differences in dynamic changes between peripheral and central hearing after perinatal hypoxia-ischemia, we studied 80 term infants during the neonatal period by serially recording brain stem auditory evoked potentials (BAEPs) at 60 dB normal hearing level. All BAEP wave latencies and the I-V interval increased significantly on day I (analysis of variance, all p < .001). Thereafter, the wave I latency decreased gradually with some variation. The wave V latency and the I-V interval increased further on day 3 and then decreased progressively. On day 30, neither the latencies nor t

Selective head cooling with hypothermia suppresses the generation of platelet-activating factor in cerebrospinal fluid of newborn infants with perinatal asphyxia.

Hypoxic-ischemic encephalopathy (HIE) remains one of the most important neurologic complications in the newborn. Several experimental and clinical studies have shown that hypothermia is the most effective means known for protecting the brain against hypoxic-ischemic brain damage. Furthermore, recent data have suggested that platelet-activating factor (PAF) could play a pathophysiologically important role in the progression of hypoxic-ischemic brain injury. The aim of the present study was to investigate the role of head cooling combined with minimal hypothermia in short-term outcome of infants

Why babies die--a perinatal care survey of South Africa, 2000-2002.

OBJECTIVE: To identify the major causes of perinatal mortality in South Africa. SETTING: Seventy-three state hospitals throughout South Africa representing metropolitan areas, cities and towns and rural areas. METHOD: Users of the Perinatal Problem Identification Programme (PPIP) amalgamated their data to provide descriptive information on the causes of perinatal death and the avoidable factors, missed opportunities and substandard care in South Africa. RESULTS: A total of 8,085 perinatal deaths among babies weighing 1,000 g or more were reported from 232,718 births at the PPIP user sites.

[Results of recent research on perinatal risk factors: resuscitation using oxygen increases the risk of childhood leukemia].

The five studies presented in this thesis were all conducted in Sweden as population based case-control studies. Children with Down's syndrome were excluded. A total of 652 cases were encompassed in the studies. Exposure data were blindly extracted from standardized medical records. There was no association between prenatal exposure to ultrasound or diagnostic x-rays and childhood leukemia. A history of maternal lower genital tract infection significantly increased the risk of childhood leukemia. This association was especially evident in children diagnosed at four years or older or in infancy

[Risk factors of 3rd stage retinopathy of prematurity progression].

Retinopathy of prematurity is a vasoproliferative, multifactorial disease of retina affecting the preterm neonates. Purpose of the study was to assess the association between a range of clinical features and the development of the stage 3 ROP. The studies comprised 79 preterm infants with diagnosed 3rd stage of ROP and 1117 preterm neonates without ROP as a control group. The analysis included the clinical data of patients--gestational age, birth weight, presence of hypotrophy, Apgar score, duration of oxygen therapy, ventilation and phototherapy. Among the potential risk factors, hypotrophy a

Group B streptococcus infection, not birth asphyxia.

This case illustrates 2 main points. Firstly, fetal infection can mimic exactly both the immediate and delayed signs of perinatal asphyxia. Secondly, the placenta may hold the key to the diagnosis of sepsis which may be made difficult in the neonate by labour ward practices such as the use of intrapartum and immediate newborn antibiotics. We strongly support the recommendation that newborn blood and fetal membrane cultures should always be obtained in babies with a diagnosis of 'intrapartum asphyxia and fetal distress' (1). To this we would add the recommendation that placental histology be pe

Intrapartum fetal asphyxial brain injury with absent multiorgan system dysfunction.

Current understanding of the physiologic mechanisms of intrapartum fetal asphyxial brain injury has suggested a strong association with multiorgan system injury. Thus the purpose here is to describe 14 cases of severe fetal brain injury with absent multiorgan system dysfunction (MSD). The study population was drawn from a national registry for brain injured infants. MSD was defined by clinical criteria demonstrated to reflect asphyxial injury to the pulmonary, renal, cardiac, hematologic, hepatic, and gastrointestinal systems. Involvement of one other organ in addition to the brain was defined

Hypercoagulability and DIC in high-risk infants.

Asphyxiated newborns have evidence of disseminated intravascular coagulation (DIC). Several hemostatic parameters were assayed in venous umbilical cord blood of 93 infants, 58 of which suffered from asphyxia due to abruptio placentae, breech delivery, preeclampsia, dead twin, and amniotic fluid embolism. Levels of factor XIII were markedly lower in the high-risk infants; the lowest values coincided with lowest Apgar scores. Plasma levels of thrombin-antithrombin (TAT) complexes, D-dimer, fibrin(ogen) degradation products (FDP), and soluble fibrin monomer complexes (SFMC) were markedly higher i

Plasma somatostatin and cholecystokinin levels in sick preterm infants during their first six weeks of life.

The present study reports the levels of plasma somatostatin and cholecystokinin in 19 preterm infants with asphyxia [n = 10, GA (median; range) 26; 23-30 weeks] and respiratory distress syndrome (n = 14, GA 27; 23-29 weeks) compared with preterm infants without any of these conditions (reference group, n = 59, GA 33; 25-36 weeks). In the reference group 37 infants received phototherapy and their peptide levels were compared with those not receiving phototherapy (n = 22). Plasma somatostatin and cholecystokinin were analysed by specific radioimmunoassays on day 1, day 3-4 and at 6 weeks of life

[The macrosomic neonate: incidence, early morbidity and legal aspects. An analysis of the Hessia perinatal study].

In the past 13 years in Hessian Hospitals there is an 20% increase of newborn with a birthweight range between 4000-4499 g and a 17% increase of newborns weigh more than 4499 g. The incidence in the first weight group increases from 7.8% to 9.3% and in the second weight group from 1.3% to 1.53%. Since 1990 the decrease in the incidence of obstetrics traumas in single macrosomic newborns in vertex presentation will be missed, although there was an decrease from 1982 to 1990. There is a statistically significant higher incidence of injuries in the weight group above 4499 g than in the both weigh

[Perinatally-induced hypoxic-ischemic encephalopathy? Possibilities of retrospective evaluation from the neuropediatric viewpoint].

This paper analyzes the feasibility of determining retrospectively, from a neuropediatric standpoint, whether an existing neurological disorder can be traced back to a perinatal hypoxic-ischemic encephalopathy. In principal, we can assume that there is a correlation between this time and pathogenesis of the brain injury on the one hand and the neurological symptoms on the other if these conditions are fulfilled 1. In the first days of live term infants revealed cerebral symptoms (like abnormality of muscle tone and consciousness and seizures) which manifest themselves in a typical sequence and

[Causes of perinatal brain damage].

The incidence of cerebral palsy has remained constant at 2 per 1000 life births in western countries through cesarean section and electronic fetal monitoring have been applied increasingly often since the last decades. There has been a shift towards prenatal causes and consequences of early prematurity. High risk deliveries (3% of all) should be performed only in specialized centers for perinatal medicine. For low risk deliveries local hospitals are more convenient and are traditionally well accepted in Germany. Perinatal hypoxia as a cause of perinatal brain damage is rare. Documentation of c

[Clinical results of pulse oximetry sub partu for measuring fetal arterial oxygen saturation].

By means of reflexion pulse oximetry the arterial oxygen saturation (SpO2) of 100 fetuses intrapartum was measured in a high-risk group and compared with clinical parameters. There is a connection between the duration of SpO2 measurements below 30%, the delivery mode and reduced pH-values in the umbilical artery measured post partum. SpO2 values below 30% also occur temporarily with uneventful deliveries but they occur with increased frequency within the group of operatively delivered children with a duration of more than 20 minutes. On the other hand the group with SpO2 values ranging between

Therapeutic efficacy of ACTH in symptomatic infantile spasms with hypsarrhythmia.

The records of twenty-six infants with both symptomatic infantile spasms and classic hypsarrhythmia were reviewed to determine the efficacy of various ACTH dosages and time of initiation of therapy. Mean age of infantile spasm onset was 6.4 months. Most patients (13) had sustained perinatal hypoxic-ischemic insults. Seventeen patients (65%) had complete cessation of spasms. Between these responders and the 9 nonresponders there was no difference in duration of spasms prior to treatment (2.6 and 2.0 months) or mean ACTH dose (87.4 and 84.5 U/m2, respectively). Infants treated with high-dose ACT
