Surgical management of major congenital malformations of the ear.

Surgical management of major congenital malformations of the ear is a difficult and complex procedure. The risk of damaging middle and inner ear structures because of their frequently aberrant location, the failure to keep a patent ear canal over the long term, and the difficulty in achieving a good hearing result all challenge the otologic surgeon. The purpose of this study was to analyze the results obtained with a surgical technique developed to prevent postoperative stenosis or total closure of the newly formed auditory meatus and to achieve at least serviceable hearing in the majority of …

Aetiological evaluation of foreign bodies in the ear and nose.

A clinical study has revealed that irritation caused by pre-existing diseases of the ear (e.g. chronic otitis externa, wax with chronic otitis externa, C.S.O.M. with otitis externa or the presence of wax in the meatus) and by those of the nose (e.g. acute or chronic rhinitis, chronic vestibulitis or the presence of dried, thick sticky mucus in the vestibule of the nose in the resolving stage of a common cold) are the major aetiological factor in foreign bodies in the ear and nose. Boredom, curiosity, whims to explore the natural cavities of the body, habit, acts of imitation, fun-making and me…

A needle otoscope. An instrument of endotoscopy of the middle ear.

A side-viewing needle otoscope, which facilitates visual inspection of the inside of the middle ear, has been devised. The instrument consists of a Selfoc rod lens for observation and a glass fiber bundle for illumination. Prisms, attached to the tip of the lens and the glass fiber bundle, facilitate the examination of a 90 degrees lateral region within the middle ear cavity. Through an existing eardrum perforation, the tip of our otoscope is safely introduced into the middle ear by using a self-illuminating otoscope. The ossicles and walls of the tympanic cavity are clearly visualized. The in…

On the overload effect of sound impulses to the inner ear.

Test series with both continuous and intermittent sound exposure to guinea pig ears yielded an area within the coordinates load frequency and load level in which a dose principle is valid more or less exactly. Exceeding the upper (level) borderline of this area provokes a damage of the organ of Corti nearly independent of the load dose. Sound levels of such order of magnitude mainly occur at sound impulses. However, to estimate the hair-cell damaging effect of a given sound impulse one needs its time or frequency function in addition to the peak level. A first order approximation seems to be p…

The underdeveloped palate in ear, nose and throat practice.

The purpose of this study is to assess the incidence of palatal underdevelopment in children undergoing tonsillectomy and adenoidectomy. Signs of underdevelopment were found in 24 out of 1,500 consecutive children admitted for tonsil and adenoid surgery (1.6%). Bifid uvula was found in 15 children (1%). Five palates showed deficiency in length or thickness (0.33%) and submucous cleft palate was found in 4 children (0.27%). The palatopharyngeal sphincter was often compensated and adequate. Middle ear diseases attributable to Eustachian tube dysfunction, with or without infection, was found in t…

[Diseases of the inner ear due to excessive noise].

Noise causes damage to the auditory system either to excessive kinetic energy, which results in acoustic trauma once the mechanical resistance capacity of internal ear tissues has been exceeded; or else, a long term exposure to noise of non-traumatic intensity can exhaust the sensory cell metabolism to such an extend that numeric atrophy of the auditory neuroepithelium will develop as a result of hyperactivity. Progressive necrosis will develop if Corti's organ barrier against endolymph is broken. The pathomorphological types of auditory organ damage are in agreement with the characteristic fu…

Regeneration of lymphatic vessels in the ear of the guinea pig.

The regenerative capacity of injured lymphatic trunks was compared to that of lymphatic capillaries following physical and chemical injuries using the techniques of micro-injection. Some transected lymphatic trunks regenerate whereas lymphatic trunks having 2 mm segments excised did not regenerate. Flow of lymph was diverted to collateral vessels. In contrast to lymphatic trunks, lymphatic capillaries showed remarkable regenerative capacity following both physical and chemical injuries.

The effects of impulse noise on the eardrum and middle ear.

Noise rarely damages the tympanic membrane and middle ear. High intensity impulse noise (explosions) can rupture the eardrum and disrupt the ossicular chain. Middle ear muscles play no part in the defense of the inner ear against high intensity impulse noise. Other mechanisms that protect the inner ear are slippage of the incudomalleal joint, a maximal limit of stapes displacement, and unlinking of the ossicular chain.

Hypothyroidism and the ear: electrophysiological, morphological, and chemical considerations.

There is both clinical and laboratory evidence that hearing loss can result from congenital and acquired hypothyroidism. The reversibility of this process, however, and its incidence and pathophysiology are not universally agreed upon. Laboratory animals rendered hypothyroid with radioactive iodine 131 or propylthiouracil demonstrated normal perilymph sodium and potassium levels but increased auditory thresholds for N1N2 response and brain stem evoked audiometry as well as a crystallized consistency of the bone of the bullae and cochleae, ossicular abnormalities, obliteration of the oval and r…

[Synthetics and metals in the middle ear (author's transl)].

A short review of synthetics and metals, which are used in middle ear surgery, is given. The own proceeding in otosclerosis and tympanoplasty is described. Special attention is drawn to the use of ventilating tubes for the treatment of sero-mucous otitis.

Patterns of ear disease in the southwestern American Indian.

Native Americans are predisposed to one of the highest incidences of otitis media in mankind. The origin of otitis media in Indians remains multifaceted. However, an unusually high prevalence of congenital anomalies of the ear and oral cavity, ie, oral clefts, facial paralysis in diabetics, and the absence of otosclerosis, suggest racial inheritance as a predominant factor for their pattern or ear disease. An analysis of outpatient and operative records at the Phoenix (Ariz) Indian Medical Center substantiates this hypothesis and shows contrasts in ear disease between American Indians and whit…

The effects of indomethacin on inner ear fluids and morphology.

Metabolic disturbances may affect inner ear fluid homeostasis in various ways. The membrane sodium-potassium pump must play a role in this homeostasis, and as this pump derives energy from A.T.P. it may also depend somewhat on the presence of prostaglandins, through their stimulatory effect on the adenylate cyclase system. Salicylates suppress prostaglandins; and Indomethacin is even more potent in this respect. Could the temporary ototoxic effects of salicylates and Indomethacin be due to prostaglandin suppression? Further, could energy system problems involving prostaglandins be related to t…

The fine structure of lipofuscin in the human inner ear.

Lipofuscin inclusions in the human membranous labyrinth were studied by electron microscopy. Lipofuscin is morphologically an irregularly shaped, membrane-bound inclusion consisting of an electron-dense structure. The most common component was a fine, granular, osmiophilic substance which was always associated with a homogenous, spherical structure resembling a lipid droplet. The combination of these two components was frequently observed in the human inner ear. Distended inclusions containing lipofuscin components were also observed within the supporting cells, saccular, utricular and ampulla…
