Restoration of pinch in intrinsic muscles of the hand.

The primary intrinsic muscles responsible for key and tip pinch are the adductor pollicis, first dorsal interosseous and flexor pollicis brevis muscles. Numerous conditions can lead to their dysfunction. Non-operative treatment consists of exercises of the compensating extensor pollicis longus and flexor pollicis longus muscles and use of adaptive devices, such as larger grips. Operative treatments include tendon transfers and joint fusions. The most common tendon transfer procedures include transfering of the extensor carpi radialis brevis to the adductor pollicis muscle or transfering of the

[The arthropathy of systemic sclerosis].

Joint involvement occurs in thirds of SSc patients during the course of the disease, but may be the onset manifestation. Arthralgias, stiffness and tendon sheath involvement constitute the most common clinical findings affecting all joints, but predominantly the fingers, wrists and ankles. The most common radiographic abnormalities in SSc patients are subcutaneous calcinosis and digital tuft resorptions, which are frequently observed at the hands. Juxtaarticular demineralisation, joint space narrowing and erosions also occur and are diagnostic challenges with rheumatoid arthritis. Flexion defo

[Atloido-axoid erosive osteoarthritis. Apropos of 3 cases].

Three cases of unilateral atloido-axis arthropathy are described in women aged 39 to 69 years. The initial diagnosis of arthritis made in view of the severity of the clinical (severe pain and stiffness) and radiological (marked chondrolysis, subchondral erosions), manifestations was contested in the absence of any laboratory abnormality of an inflammatory nature, as well as any bacterial, inflammatory rheumatic or metabolic cause. The final diagnosis made at the time of hospitalisation after 8 to 9 month progression of the disease was osteoarthritis, no subject to any doubt on the basis of the

[Paralysis of the intrinsic muscles of the hand].

Palliative treatment of intrinsic muscle paralysis aims at correcting the claw deformity and improving prehension. This treatment will vary according to whether the claw can be actively corrected or not, if M. P. hyperextension is blocked. In the first category it suffices to maintain M. P. joint flexion either by capsulodesis or tenodesis so that the long extensors can extend the interphalangeal joints. Tendon transfers are useful only to reinforce the flexion force of the fingers if there is an associated long flexor tendon paralysis. These transfers should be fixed to the proximal part of t

Flexor Tendon Ruptures After Distal Scaphoid Excision for Scaphotrapeziotrapezoid Osteoarthritis.

BACKGROUND: Distal scaphoid excision is one treatment option for osteoarthritis of the scaphotrapeziotrapezoid (STT) joint following failure of conservative measures. Potential complications of this procedure include injury to the carpal ligaments, cartilage, and radial artery. METHODS: A single case was identified by the senior author, and the medical record was reviewed for surgical notes, progress notes, and radiographs. RESULTS: A 68-year-old male sustained ruptures of the flexor digitorum superficialis (FDS) and flexor digitorum profundus to the index finger 3 years following a distal s

Index rotation flap for palmar thumb release in arthrogryposis.

The soft tissue envelope in congenital contractural conditions such as clasped or arthrogrypotic thumbs is often deficient in 2 planes, the thumb-index web and the flexor aspect of the thumb. Small local transposition flaps borrow tissue from 1 plane to augment the other and are likely to worsen 1 contracture whereas improving the other. Tissue must be added to the thumb envelope from outside the contracted areas. A flap based on the index finger can be designed to both open the thumb-index web and augment skin on flexor aspect of the thumb metacarpophalangeal joint. This flap is particularly

Orthopaedic management of the upper extremity of stroke patients.

Cerebrovascular accidents often produce significant pathology, including upper extremity muscle contractures and deformities that may be painful and aesthetically unappealing and that interfere with activities of daily living and hygiene. Orthopaedic intervention may be required to manage these disabilities. Nonsurgical management includes brachial plexus and phenol nerve blocks, which provide temporary relief of painful contractures and allow for a period of spontaneous neurologic recovery of up to 6 months. Definitive surgical procedures should be avoided during this time. After this period,

Whitish bullae on the fingers: what is the diagnosis? Tophaceous gout.

A 56-year-old woman presented with a 6 month history of bullae and whitish plaques of the fingers. She had chronic renal failure, but no joint pains or evidence of arthritis. An aspirate of fluid from a bulla revealed uric acid crystals and serum uric acid was elevated at 16.2 mg/dL. A diagnosis of tophaceous gout was made.

[Pachydermodactyly: a report of two cases].

Pachydermodactyly is a superficial benign, symmetrical and painless fibromatosis, characterized by a swelling of the proximal interphalangeal joints of the fingers. Histological examination shows epidermal hyperplasia increase of dermal collagen bundles and an increased number of fibroblasts. We report two cases of pachydermodactyly in male teenagers. The aetiology remains unknown. Pachydermodactyly has to be distinguished from knuckle pads.

[Oligoarthritic joint symptoms of the hands in metastasizing bronchial carcinoma].

Bronchial carcinomata very frequently metastasize to bony tissue. The most common sites are generally known, but metastases can also occur in atypical locations where they then simulate other clinical syndromes. The present paper describes the clinical picture of a patient with small-cell bronchial carcinoma metastasizing to the finger joints. The frequency of this manifestation is investigated on the basis of the literature.

Flexor Tendon Injuries in Zone 3: A Comprehensive Review and Case Report on Flexor Digitorum Profundus Rupture of the Middle Finger.

INTRODUCTION: Isolated rupture of the flexor digitorum profundus tendon within flexor zone 3 is exceptionally rare and easily mistaken for the far more common zone 1 "Jersey finger" lesion. To the best of our knowledge, only two such middle-finger cases have been documented over the past six decades. Reporting this case, together with a literature synthesis, highlights the diagnostic pitfalls and supports timely, tendon-preserving intervention. CASE REPORT: A 65-year-old right-hand-dominant White male felt a sudden snap in his right palm while restraining a dog leash, followed by an inability

Clinical Characteristics of Hand and Finger Contractures in Postburn Patients: A Single-Center Retrospective Analysis at a Top Referral Hospital in West Java, Indonesia.

PURPOSE: Contracture is a common complication following severe burns, with the hands being particularly susceptible. Although not life-threatening, hand and finger contractures can significantly reduce a patient's quality of life by impairing joint function and mobility. This study aims to describe the characteristics of hand and finger contractures in postburn patients who underwent surgery at Top Referral Hospital in West Java, Indonesia between 2019 and 2023. METHODS: This research is an observational descriptive study with a cross-sectional design, using medical record data of postburn ha

An assessment of 2 objective measurements of web space position.

PURPOSE: To describe 2 simple objective clinical methods of measuring the web position between fingers and to determine their intra-observer and inter-observer reliabilities. METHODS: Two observers examined the second, third, and fourth web spaces on both hands of 30 adult healthy volunteers. The web index measured the web height as a relative ratio to constant anatomical landmarks on both fingers subtending the web. The dorsal web index took reference from the distance between the metacarpophalangeal and proximal interphalangeal joints, whereas the palmar web index was measured in relation t

[Thumb/big-toe localizing test: examination for deficit of proprioception through the posterior column-medial lemniscal system].

"Thumb localizing test" (TLT) and "big-toe localizing test" (BLT) are bedside examinations to detect abnormalities in proprioceptive afferent pathways from the limbs. In TLT, the patient is asked to close the eyes, one upper limb of the patient is placed in a fixed position by the examiner, and the patient is asked to localize the thumb of the fixed upper limb with the thumb and index finger of the other upper limb. In BLT, the patient is asked to close the eyes, a lower limb is passively immobilized by the examiner, and the patient is asked to locate the big toe with either index finger. Norm

Early mobilization of fractures of the metacarpals and phalanges.

Early mobilization of fractures of the metacarpals and phalanges is advocated. It should begin as soon as the stability of the fracture and the condition of the soft tissues permits. It has to be adapted to each case, and often consists of mobilization of the distal phalanges with the wrist and metacarpophalangeal articulations immobilized in a protective position. It is effective in preventing stiffness. Because it is a specific technique, it must be entrusted to specially trained physiotherapists, and requires regular and repeated clinical and radiologic follow up by the surgeon.

Unusual presentation of an usual condition-cystic ganglionosis.

Cystic ganglionosis is an unusual benign condition, which presents as multiple ganglion cysts involving multiple joints. The case we report here is probably the first case of an adult reported in the literature. The only other case of multiple ganglion cysts reported in the literature is that of an 11-year-old child with ganglion cysts in multiple joints. A 57-year-old female presented with an approximately 4-year history of pain and swelling below the left knee and in the right hand. An MRI of the left knee showed multiloculated cystic-intensity lesions around the knee joint with intra-/extra

Pseudo-knuckle pads: an unusual cutaneous sign of obsessive-compulsive disorder in an adolescent patient.

Knuckle pads are discrete benign cutaneous lesions overlying the extensor surfaces of the fingers and hand joints and are unrelated to trauma, whereas pseudo-knuckle pads may be considered as a form of callosity that appears after repeated trauma. This type of knuckle pad has been described in children with obsessive behavior as "chewing pads" and in adults as occupational disorder. Cases of pachydermodactyly, benign fibromatosis of the fingers, have been described as the unusual forms of knuckle pads that usually affect young adult males. We believe that pseudo-knuckle pads, chewing pads or p

Arthropathy associated with anti-Jo-1 antibody.

Anti-Jo-1 antibody is associated with an overlap syndrome usually described as the association of idiopathic inflammatory myopathy, pulmonary fibrosis and polyarthritis. We report three observations illustrating different aspects of arthropathy associated with anti-Jo-1 antibody. Two patients presented with a deforming and erosive arthritis affecting the hands, periarticular calcifications and dislocation of the interphalangeal (IP) joint of the thumb. The third patient, who had a short disease course, presented only with a mild non-erosive polyarthritis of both hands, metacarpophalangeal join

[Spondyloepiphyseal dysplasia tarda with progressive arthropathy: 2 siblings affected].

We described a Mexican family whose parents were consanguineous. Therefore two children were affected by a progressive arthropathy with deformity in all finger joints, restricted joint mobility and broad major joints. This condition was diagnosed like atypical juvenile rheumatoid arthritis (JRA) because the test for serum rheumatoid factors and antibodies were negative and failed to respond to anti-rheumatoid treatment. However their radiographic studies showed the spine with universal platyspondyly, enlargement epiphyses of the hands, the absence of destructive and the presence of the dysplas…

Acupuncture-related Digital Joint Infection Managed With Staged Surgery And Oral Antibiotic Therapy.

Acupuncture is generally considered safe; however, inadequate aseptic technique may result in infections involving the skin, soft tissue, bone, or joints. Gram-negative bacilli causing hand infections are uncommon and may present diagnostic and therapeutic challenges, particularly when multiple digits are involved. A 73-year-old woman developed progressive pain, swelling, and functional limitation of the right third and left fifth fingers approximately 1 month after acupuncture. Imaging suggested osteomyelitis with involvement of the interphalangeal joint. Separate sterile specimens were obtai
