Providing Contraception to Adolescents.

Adolescents have high rates of unintended pregnancy and face unique reproductive health challenges. Providing confidential contraceptive services to adolescents is important in reducing the rate of unintended pregnancy. Long-acting contraception such as the intrauterine device and contraceptive implant are recommended as first-line contraceptives for adolescents because they are highly effective with few side effects. The use of barrier methods to prevent sexually transmitted infections should be encouraged. Adolescents have limited knowledge of reproductive health and contraceptive options, a

Social impairments in chromosome 22q11.2 deletion syndrome (22q11.2DS): autism spectrum disorder or a different endophenotype?

High prevalence of autism spectrum disorders (ASD) has been reported in 22q11.2DS, although this has been based solely on parent report measures. This study describes the presence of ASD using a procedure more similar to that used in clinical practice by incorporating history (Social Communication Questionnaire) AND a standardized observation measure (Autism Diagnostic Observation Schedule) and suggests that ASD is not as common as previously reported in 22q11.2DS. Differences in methodology, along with comorbid conditions such as anxiety, likely contribute to false elevations in ASD prevalenc

Classifications for adult spinal deformity and use of the Scoliosis Research Society-Schwab Adult Spinal Deformity Classification.

Adult spinal deformity (ASD) is a complex disease state that pathologically alters standing upright posture and is associated with substantial pain and disability. This article provides an overview of classification systems for spinal deformity, clarifies the need to differentiate between pediatric and adult classifications, and provides an explanation on the use of the Scoliosis Research Society-Schwab Adult Spinal Deformity Classification (SRS-Schwab ASD Classification). This information allows surgeons, researchers, and health care providers to (1) identify sources of pain and disability in

Discrimination between biothreat agents and 'near neighbor' species using a resequencing array.

Timely identification of biothreat organisms from large numbers of clinical or environmental samples in potential outbreak or attack scenario is critical for effective diagnosis and treatment. This study aims to evaluate the potential of resequencing arrays for this purpose. Albeit suboptimal, this report demonstrated that respiratory pathogen microarray version 1 can identify Bacillus anthracis, Francisella tularensis, Yersinia pestis and distinguish them from benign 'near neighbor' species in a single assay. Additionally, the sequence information can discriminate strains and possibly the sou

Nurse on nurse.

TOPIC: Horizontal violence among nurses. PURPOSE: To see if horizontal violence in the workplace exists in nursing. If it does, what is its relationship to patient care? SOURCES OF INFORMATION: This literature review was conducted using research articles published in 2003 and 2004. The articles were searched for in the electronic database, CINAHL. Both Ovid and EBSCO vendors were used. CONCLUSIONS: It is clear that horizontal violence exists in nursing today. It affects nursing in all areas. When tension is elevated in patient care areas, nursing staff are not likely to perform at their bes

Smoking cessation and cancer.

OBJECTIVES: To provide an overview of scientifically based smoking cessation interventions for delivery to all tobacco-dependent users. DATA SOURCES: Research studies, published articles, and government reports. CONCLUSION: Smoking cessation interventions for patients with cancer remain poorly described with only a few studies investigating the efficacy of smoking cessation among this population. However, data suggest that quitting smoking after the diagnosis of cancer improves survival and quality of life. IMPLICATIONS FOR NURSING PRACTICE: Nurses serve a critical role in tobacco-dependenc

A methodology for preparing 'ideal' treatment outlines in psychiatry. The Quality Assurance Project.

The methodology of preparing treatment process criteria for agoraphobia and the depressive disorders is described. To avoid bias three sources of information were used: the results of a meta-analysis of the treatment outcome literature, the views of a sample of practising psychiatrists, and the views of a small representative expert committee. When criteria for schizophrenia and the neuroses have been completed the quality assurance project should serve two main purposes: first as an aid to continuing and postgraduate education and second as a basis for clinical review criteria required by pee

Pharmacologic contraception methods for people with cystic fibrosis: A practical review for clinicians.

Over the last several decades, substantial treatment advances have improved the quality of life and median predicted survival in people with cystic fibrosis (PwCF). It is critical for CF clinicians to begin to discuss health considerations related to an aging and overall healthier CF population. Such considerations include family planning, reproductive health, and contraception. CF care teams are trusted sources of medical information and therefore often have initial discussions related to contraception for PwCF. The purpose of this article is to review the available pharmacologic contraceptiv

Pain Recognition and Assessment in Birds.

The recognition and assessment of pain in avian species are crucial tools in providing adequate supportive care in clinical, laboratory, zoologic, rehabilitation, and companion animal settings. With birds being a highly diverse class of species, there is still much to be determined regarding how to create specific criteria to recognize and assess pain in these animals. This article provides a clinical review on the physiology of pain in birds, observed behavioral and physiologic alterations with pain, how different sources and degrees of pain can alter behaviors observed, and how this informat

[Early identification of a clustered epidemic of typhoid fever through pathogen molecular subtyping monitoring].

Objective: To identify a suspected clustered Typhoid fever by whole genome sequencing(WGS) and pulsed field gel electrophoresis (PFGE) subtyping. Methods: The nature of the epidemic was determined by combination of subtyping results of isolates and epidemiological information. Results: Five S. typhimurium isolates showed identical PFGE patterns and almost the same whole genome sequence. Epidemiological survey showed that five cases had dined in the same restaurant on the same day. Conclusion: Combined with the longest incubation period of typhoid fever, molecular subtyping of pathogenic bacter

Assessment of the paraphilias.

Paraphilias are recurrent, persistent, and intense sexual interests in atypical objects or activities. The most commonly encountered paraphilias in sexological or forensic settings are pedophilia, sexual sadism, exhibitionism, and voyeurism. Paraphilias are often comorbid with other sexual, mood, and personality disorders. Assessment and diagnosis require an integration of multiple sources of clinical information, given the limits and biases of self-report (through clinical interview or questionnaires). Clinicians ideally have access to more objective assessment methods, such as phallometric t

Topical photodynamic therapy in the treatment of hidradenitis suppurativa.

Topical photodynamic therapy (PDT) is well established for the treatment of malignant and pre-malignant skin lesions and now it is increasingly used to treat benign dermatological conditions. The studies reporting the use of PDT in hidradenitis suppurativa are reviewed. Though small in number, there is considerable variation in the application of topical photosensitisers, light sources used and treatment regimes. In addition there is often limited information about patient selection in terms of disease severity and measuring precise patient outcome. These issues need to be addressed in future

The search of 'novel' mtDNA mutations in hypertrophic cardiomyopathy: MITOMAPping as a risk factor.

MITOMAP is by far the most frequently cited Web resource that is referred to in substantiating novelty of an mtDNA mutation. This database, as is now known, has quite an incomplete coverage of the mtDNA mutations from the literature. This circumstance has seduced many scholars of medical genetics in the past to claim novelty of rather 'worn-out' mtDNA mutations. What is, however, really novel in the field is that researchers take advantage of this situation and deliberately suppress information from other sources, as it appears to have occurred in two recently published cases of hypertrophic c

Watershed issues associated with Clostridium botulinum: a literature review.

Botulism the disease, the related organism (Clostridium botulinum) and toxin have gained renewed attention in these times of heightened homeland security and bioterrorism preparedness. Since C. botulinum is ubiquitous in nature, botulism outbreaks resulting from environmental exposure can be of concern to watershed managers and drinking water utilities. This paper reviews aspects of naturally occurring C. botulinum in light of concerns for source water watersheds. Information regarding sources and occurrence of botulism, C. botulinum and botulism toxins are discussed. Ecology and physiology of

Critical care of neurotrauma.

Traumatic brain injury often affects people in their most productive years, inflicting a significant burden on families and society. The advances in modern critical care have improved survival of patients; thus more patients live after traumatic brain injury, which raises an important issue about their neurologic outcome. At the present time, there are limited data regarding methods to optimize neurologic recovery. In this review, we try to bring information from different sources to show new approaches to achieve that goal. Some of the techniques employed are investigational and some are wait
