The percutaneous fate of the rodenticide flocoumafen in the rat: role of non-biliary intestinal excretion.

1. Appreciable penetration of radioacticity occurred through rat skin following percutaneous administration of 14C-flocoumafen. At 7 days after dosing 12% of the administered radioactivity remained at the site of application, while 25% was located in the liver as unchanged flocoumafen. 2. Excretion of flocoumafen metabolites via the urine accounted for 10% dose over the 7 day experiment, this is some 30-fold greater than that seen after a single oral dose. 3. Unchanged flocoumafen comprised the major product detected in faeces. Biliary elimination was a very minor route of excretion and did no

Exploring the mechanism of Huanglian ointment in alleviating wound healing after anal fistula surgery through metabolomics and proteomics.

Anal fistula is a common anal and intestinal disease. The wound of anal fistula surgery is open and polluting, which is the most difficult to heal among all surgical incisions. To investigate the mechanism of Huanglian ointment (HLO) on wound healing after anal fistula incision. The S. aureus infected wound in SD rats were used to imitate poor healing wound after anal fistula surgery. SD rats with wound sites (n = 24) were randomly divided into four groups (Control group, Model group, Potassium permanganate (PP) treatment group, and HLO treatment group). The wound healing rate was evaluated, H

[Abdominal wall defects caused by postoperative infections].

Infectious defects of the abdominal wall often result from deeper subfascial infections of the abdominal wall caused by a persisting intraabdominal inflammation or the result of laparotomy in peritonitis. Therapies applied are first of all the removal of the sources of infection and a careful debridement of the abdominal wall. There are several ways of temporarily covering the abdominal wall, but there is no ingenious solution. In our own experience the application of zip fastening and PGS nets have proved successful. In case of deeper infections of the abdominal wall the most awkward situatio

[Value of computed tomography in the diagnosis of fistulas].

PURPOSE: The purpose of the study was to determine the value of computed tomography (CT) in demonstrating fistulae in comparison with conventional radiographic methods. METHODS: In a prospective study 25 patients were evaluated by conventional radiographic methods and CT. RESULTS: The identification of the fistulous tract was possible with CT in 27 of 29 cases, whereas 2 fistulae could only be detected by indirect signs. Furthermore, CT showed a larger extent of the fistulous tract in 5 patients and revealed complications such as inflammatory mass, abscess or osteomyelitis in 11 cases. CONC

Sigmoid colonic tuberculosis presenting as a colovesical fistula mimicking colorectal malignancy: a case report.

BACKGROUND: Intestinal tuberculosis (ITB) most commonly involves the ileocecal region. Isolated sigmoid colonic tuberculosis complicated by a colovesical fistula is extremely rare and may closely mimic colorectal malignancy or Crohn's disease (CD). CASE PRESENTATION: A 73-year-old man presented with subacute diarrhea, fever, and lower urinary tract symptoms. Laboratory tests showed markedly elevated inflammatory markers and anemia. Cross-sectional imaging demonstrated segmental thickening of the sigmoid colon, pericolic lymphadenopathy, multiple serous effusions, and findings consistent with

Constructing Cationic Polyamide Membranes by Surface-Initiated Grafting for Precise Lithium Extraction from Salt Lake Brines.

BACKGROUND AND AIMS: Length of diagnostic delay is associated with bowel strictures and intestinal surgery in adult patients with Crohn's disease [CD]. Here we assessed whether diagnostic delay similarly impacts on the natural history of paediatric CD patients. METHODS: Data from the Swiss IBD Cohort Study were analysed. Frequency of CD-related complications [bowel stenosis, perianal fistula, internal fistula, any fistula, resection surgery, fistula/abscess surgery, any complication] at diagnosis and in the long term [up to 30 years after CD diagnosis] was compared between paediatric patients

Decellularized small intestine scaffolds: a potential xenograft for restoration of intestinal perforation.

Small intestine perforation is a serious medical condition that requires immediate medical attention. The traditional course of treatment entails resection followed by anastomosis; however, it has complications such as small bowel syndrome (SBS), anastomotic leakage, and fistula formation. Here, a novel strategy is demonstrated, that utilizes the xenogeneic, decellularized goat small intestine as a patch for small intestine regeneration in cases of intestinal perforation. The goat small intestine scaffold underwent sodium dodecyl sulfate decellularization, which revealed consistent, quick, and

Early Enteral Nutrition Preserves Intestinal Barrier Function through Reducing the Formation of Neutrophil Extracellular Traps (NETs) in Critically Ill Surgical Patients.

BACKGROUND: The gut was suggested as the driver of critical illness and organ injury. Recently, excessive formation of neutrophil extracellular traps (NETs) was associated with mucosal inflammation. Direct investigation of intestinal mucosa is essential to illuminate the potential mechanism of gut barrier in critically ill patients. We hypothesized that early enteral nutrition (EN) could decrease intestinal NETs and maintain the gut barrier. METHODS: Intestinal biopsies were obtained using biopsy forceps from critically ill surgical patients complicated with enterocutaneous fistula. Expressio

Not all intestinal traumatic injuries are the same: a comparison of surgically treated blunt vs. penetrating injuries.

PURPOSE: Traumatic intestinal injuries are less common with blunt compared to penetrating mechanisms of trauma and blunt injuries are often associated with diagnostic delays. The purpose of this study is to evaluate differences in the characteristics and outcomes between blunt and penetrating intestinal injuries to facilitate insight into optimal recognition and management. METHODS: A retrospective analysis of trauma admissions from January 2009 to June 2011 was performed. Mesenchymal Stem Cells to Treat Crohn's Disease with Fistula.

Crohn's disease, which mainly affects the gastrointestinal tract, is a refractory inflammatory disease that has clinical manifestations of abdominal pain, fever, bowel obstruction, and diarrhea with blood or mucus. Together, these symptoms can severely impair a patient's quality of life. Besides the common complication of intestinal obstruction, fistulas, particularly anorectal fistulas, are common in Crohn's disease patients. Since radical surgical cures can be difficult to achieve and relapse is common, Crohn's disease patients often seek other effective treatments in addition to surgery. St

Late complications of incisional hernias following prosthetic mesh repair.

BACKGROUND: Incisional hernias still continue to be a serious problem for surgeons. In this study, we aimed to investigate the effects of polypropylene mesh and mersilene mesh, which are frequently employed in incisional hernias, as well as the effects of their application techniques on late complications. METHODS: Two-hundred-sixty-four open abdominal hernia repairs were performed between 1986--2000, using prosthetic materials: polypropylene mesh and mersilene mesh were used in hernia repair. Mesh was placed as onlay, underlay and sandwich techniques. Follow-up data were obtained from medica

Participation of the adrenergic receptors in the regulation of large bowel motor activity in sheep.

In sheep with long-standing fistulae of the caecum, proximal loop of the colon and rumen the motor activity of these parts of the intestine was studied using the balloon method. Slow isoprenaline infusion caused first inhibition of caecal contractions, followed by gradually decreasing intensity of contractions of the colon and rumen. This inhibitory effect of isoprenaline was eliminated by propranolol. Intravenous infusion of phenylephrine failed to inhibit the colonic motor activity. A small dose of atropine, similarly as isoprenaline, caused an irregular inhibition of the contractions of the

A modified technique for the surgical correction of urethral diverticula using a porcine xenograft.

We report a case of urethral diverticulectomy re-enforced with a porcine xenograft to prevent the risk of recurrence or fistula in the presence of a large urethral communication. The use of porcine small intestinal submucosal xenograft (SIS, Surgisis, Cook, Ireland) material has a low graft rejection rate and erosion is rare as the material is degraded after 3 to 6 months. In this case, xenograft achieved tension-free closure of the urethral defect without any postoperative complications.

[Post-operative superior mesenteric arterio-venous fistulas. A report on one case (author's transl)].

Presentation of a particularly evoking case of a post-opérative superior mesenteric arterio-venous fistula revealed by recurrent intestinal hemorrhages, diagnosed by selective arteriography and treated by resection of the anevrysmal pouch and the adjacent bowel. Review of the literature concerning 25 other cases from which it can be concluded that the digestive angiography was the only complementary examination that allowed the diagnosis. The curative surgical treatment has a 15% mortality rate in relation with hemorragic recurrences.

Harvest of a pedicled omental flap preserving the right gastroepiploic artery for gastric conduit reconstruction in multistage surgery for aortoesophageal fistula.

Aortoesophageal fistula is a rare but highly fatal condition. Thoracic endovascular aortic repair has led to improved early outcomes. However, subsequent aortic replacement, esophagectomy, and gastrointestinal reconstruction are required to achieve long-term survival. Conventional pedicled flaps using both gastroepiploic arteries can prevent gastric conduit reconstruction, requiring small or large intestinal substitutes associated with greater surgical invasiveness. We developed the pedicled omental flap technique preserving the right gastroepiploic artery, which enabled simultaneous gastric c

Bouveret's syndrome treated with endoscopic electrohydraulic lithotripsy.

Bouveret's syndrome is a rare form of small bowel obstruction caused by a large biliary stone entering the lumen of the duodenum or the stomach through a bilioenteric fistula. Treatment options include various surgical techniques. However, recent advances in endoscopy also allow non-surgical endoscopic treatment options. We report a 68-year-old man, with a disseminated small intestinal neuroendocrine tumour, presenting with Bouveret's syndrome without any previously reported biliary disease. He experienced a number of symptoms 1 month prior to his admittance, which were difficult to differenti

[Hypertrophic osteoarthropathy and aortic graft infection: Two cases].

INTRODUCTION: Hypertrophic osteoarthropathy (HOA) located to lower extremities may be the initial manifestation of an infected aortic graft. CASE REPORTS: We report two patients with HOA secondary to aortobifemoral vascular prosthesis infection and aortoenteric fistula. As reported in the literature, initial manifestations included fever, painful swelling of limbs, joint pain, clubbing and in one case intestinal bleeding. These symptoms preceded frank episodes of septicaemia due to a wide variety of bacteria (microbial enteric flora). Bone scan was an appropriate tool for confirming the diagn

Colobronchial fistula: an unusual complication after peritonectomy and hyperthermic intra-peritoneal chemotherapy (HIPEC).

BACKGROUND: Cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) is an innovative approach to peritoneal carcinomatosis. Due to the complexity of the combined procedure, high rates of potentially life-threatening complications have been reported. This is the first report of colobronchial fistula following CRS and HIPEC. CASE REPORT: A 70-year-old woman underwent CRS and HIPEC for papillary well-differentiated peritoneal mesothelioma. During the postoperative course, recurrent pneumonia occurred and bacteria of intestinal origin were isolated from expectorated sput

[Hemobilia (author's transl)].

Hemobilia is a syndrome caused by hemorrhage into the biliary ducts. It is characterized by colicky pains, jaundice, and intestinal bleeding; a correct diagnosis of this syndrome is made rather rarely. The most common cause is trauma; inflammatory or tumorous disease of the liver and bile duct system, bile stones and aneurysms of the hepatic artery are rarer causes. The diagnosis can be established and the arterio-biliary fistula localized by selective arteriography. A case is described, which shows, that, in addition, duodenoscopy with retrograde cholangiography is of diagnostic value. Other

[Treatment of vestibular fistulas in older girls].

The vestibular fistula is the anorectal malformation more frequent in females. In this congenital anomaly the anus is located in the vaginal vestibule, having the rectum a common wall with the vagina. With the posterior sagittal approach described by Alberto Peña it changes the treatment of this anorectal malformation, contributing to the possibility of an aesthetic and functional improvement. From 1996 we have operated on 6 girls with ages included between 6 and 22 years (mean of 11 years) that presented a vestibular fistula. Five cases had been operated on in the neonatal period of vestibula
