PURPOSE: Magnetic resonance imaging (MRI) has been commonly used for the preoperative evaluation of recurrent lateral patellar dislocation (RLPD). The purpose of this study was to determine the usefulness of high-resolution MRI (HR-MRI) with a microscopy coil for diagnosing RLPD.
MATERIALS AND METHODS: The study group consisted of 15 patients with clinically diagnosed RLPD and 10 normal volunteers. All studies were performed on a 1.5-T MR system. First, conventional MRIs of the whole knee joint were obtained using the knee coil. Then HR-MRI scans using a microscopy coil in the medial aspect o

Unicompartmental knee arthroplasty is an alternative to total knee arthroplasty for patients with unicompartmental tibiofemoral noninflammatory disease. Careful patient selection and newer instrumentation has reduced the progression of arthrosis in the other compartment and tibial loosening, leaving polyethylene wear as the predominant failure mechanism in more contemporary designs. Increased wear increases the debris volume at the bone-implant interface, and wear particles will generate osteolysis leading to component loosening and unreplaced compartment degeneration. The design-related facto

Patellofemoral instability is a common knee pathology affecting children and adolescents, with recurrence rates exceeding 50% in skeletally immature patients. Identification and stratification of anatomic risk factors for recurrence are essential. Trochlear dysplasia, patella alta, skeletal immaturity, and elevated tibial tubercle groove distance have been identified as anatomic predictors of recurrence. Emerging evidence is supporting more aggressive treatment and earlier surgical intervention in high-risk patients, even after a first-time dislocation. Medial patellofemoral ligament reconstru

Lateral patellar instability is one of the most common knee disorders among adolescents. Numerous anthropometric features, including trochlear dysplasia, patella alta, high tibial tubercle-to-trochlear groove distance, and coronal and rotational plane malalignment, are considered primary risk factors for patellar dislocation, and the understanding of their complex interplay is continuously evolving. Because of the multifactorial nature of patellar instability, there is a lack of consensus regarding many aspects of surgical intervention. Medial patellofemoral ligament reconstruction is consider

BACKGROUND: Trochlear dysplasia is a considerable risk factor for patellar instability. Surgical treatment options include medial patellofemoral ligament reconstruction, medial quadriceps tendon-femoral ligament reconstruction, femoral rotational osteotomy, tibial tubercle osteotomy, and trochleoplasty.
CASE REPORT: A 26-year-old female presented with bilateral knee pain and recurrent patellar dislocations since childhood that were unresponsive to physical therapy and bracing. Initial evaluation by an orthopedic trauma service revealed excessive femoral anteversion and Dejour type D trochlear

PURPOSE: This study analyzed the interaction of the anteromedial and posterolateral portions of the anterior cruciate ligament (ACL) in resisting medial and lateral tibiofemoral compartment subluxations under multiple loading conditions.
METHODS: By use of a 6-df robotic simulator, 10 human cadaveric knees were tested in 3 states: intact ACL, partial ACL (loss of either the anteromedial bundle [AMB] or posterolateral bundle [PLB]), and deficient ACL. The testing profile involved anterior-posterior translation and internal-external rotation, as well as 3 pivot-shift loading conditions with var

Ultrasound scanning of the shoulders, knee joints, Achilles' tendon and muscles is presented as a noninvasive technique for the diagnosis of joint and soft tissue disorders. In experimental ultrasound studies on shoulders, knee joints, tendons and muscles of cadavers the diagnostic value of the method is assessed. The experimental observations were compared with clinical results collected in three years from 610 patients. Particularly characteristic findings in sports-induced soft tissue lesions are reviewed as examples.

An operative procedure has been developed to prevent anterior subluxation of the proximal end of the tibia in patients with anterolateral rotatory instability of the knee. The results of this procedure in eighteen consecutive patients are reported. Ten (55.6 per cent) had had prior surgery on the involved knee. All patients had an additional pathological lesion and all were incapacitated for their sports prior to reconstruction. The shortest follow-up in this series was thirty-one months and the longest, forty-four months. Forty-four per cent had an excellent result, participating in sports wi

Ruptures of the knee extensor apparatus in elderly individuals or following total knee replacement present a significant functional limitation and surgical challenge. The Leeds-Keio ligament, a synthetic graft, has been historically used for reconstruction but has seen declining popularity with advancements in biological grafts. This study revisits the application of the Leeds-Keio ligament in managing chronic and complex extensor mechanism ruptures. It is a retrospective review of five patients (mean age 73.6 years) who underwent surgical reconstruction of the ruptured patella or quadriceps t

The purpose of this study was to define the diagnostic value of magnetic resonance imaging (MRI) and plain radiographs (X-ray [XR]) in identifying an osteochondral defect or loose body in patients undergoing operative treatment for patellar instability. A total of 87 patients treated operatively for patellar instability with medial patellofemoral ligament (MPFL) reconstruction between 2015 and 2019 were identified. Inclusion criteria were evidence of clinical patellar instability, preoperative XR and MRI studies, and concomitant diagnostic knee arthroscopy and MPFL reconstruction performed to

PURPOSE: Maltracking of the patella after total knee arthroplasty (TKA) remains a well-recognized problem. The medial patellofemoral ligament (MPFL) has shown to be important for patellar stabilization and reconstructions of the MPFL have already shown excellent functional outcomes for patellar instability of the native knee. Nevertheless, there is only limited literature on using an MPFL reconstruction for correction of patellar maltracking after TKA. In this retrospective study, a consecutive case series was evaluated.
METHODS: Between 2007 and 2010, nine patients (nine knees) with anterior

The medial patellofemoral ligament (MPFL) is of critical importance for patellar stability and is universally incompetent in patients with patella dislocations. However, radiological (magnetic resonance imaging [MRI]) assessment of the MPFLs' integrity following dislocation is variable, adding confusion to patients and providers during treatment decision-making. We aimed to assess the reliability of MRI to evaluate MPFL integrity by measuring inter- and intraobserver agreement between a musculoskeletal radiologist and an orthopaedic surgeon, specializing in sports medicine using a novel standa

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction is the primary surgical treatment for recurrent patellofemoral instability. Hamstring tendon (HT) autografts have traditionally been used, while quadriceps tendon (QT) autografts have gained popularity as an alternative. The purpose of this study was to compare clinical, functional, stability, and objective strength outcomes following MPFL reconstruction using QT versus HT autografts.
METHODS: PubMed, Scopus, and Ovid MEDLINE were searched. Comparative studies directly evaluating QT and HT autografts for isolated MPFL reconstruc

Spondyloepiphyseal dysplasia (SED), Omani type (OMIM 608637) is a recessively inherited skeletal dysplasia previously described in two distantly related families from the Republic of Oman. The phenotype consists of short stature, severe kyphoscoliosis, arthritic joints (elbows, wrists, knees), secondary large joint dislocations, rhizomelia, fusion of carpal bones and mild brachydactyly. Affected individuals were homozygous for a missense mutation, R304Q in CHST3 that encodes the enzyme chondroitin 6-O-sulfotransferase-1 (C6ST-1). This enzyme mediates the sulfation of proteoglycans, particularl
