What can be done for a 17-year-old girl with localized juvenile periodontitis, a Class II malocclusion, and flared teeth? Is it possible to regain epithelial attachment and rebuild the bone architecture? Is it possible to achieve good esthetics and occlusion with conventional orthodontic treatment, or is an interdisciplinary approach needed? What sequence should be followed? What is the prognosis for the affected teeth in the long term? Our aim in this article was to review the literature and present a clinical case to shed more light on the subject.

Thirty persons with Class II Division 1 subdivision malocclusions, ranging in age from 12 years 8 months to 42 years, underwent computed tomography of the temporomandibular joints. The images obtained from sagittal slices were used to assess the depth of the mandibular fossa, the angulation of the posterior wall of the articular tubercle, the condyle-fossa relationship, and the concentric position of the condyles associated with this malocclusion. Paired Student t tests were applied, and Pearson product moment correlations (r) were determined after measurements on both Class I and Class II sid

The purpose of the study was to investigate root lengthening during orthodontic treatment in relation to the age of the patient, the developmental stage of the root, and the anticipated growth. Specifically, the potential benefit of treating young teeth was addressed. The sample consisted of 80 patients with Angle Class II division 1 malocclusions, treated with extraction of at least two maxillary first premolars, and edgewise technique with 0.018-inch slot brackets. Additionally, a cross-sectional control group of 66 untreated individuals matched to gender, and pre- and post-treatment age of

Marked lower anterior crowding (late crowding or post-adolescent crowding) often develops at about the time of third molar eruption. Many studies have implicated the presence of third molars as a cause of this crowding. Despite the fact that there is no clear evidence that this is the case, many orthodontists and their patients' referring dentists routinely remove third molars at the retention stage of treatment to prevent crowding later; Ricketts et al. even advocate enucleation of the third molar while the patient is very young to prevent future crowding. In the following report on two ortho

Little is known about asymmetry of children's dental arches. The purposes of this study were to quantify and describe dental arch asymmetry of 151 children with large overjets, and to determine if a spatial relationship exists between dental landmarks in opposing arches. The median palatal plane (MPP) was the reference for transverse measurements. A computer-constructed transverse palatal plane (TPP) was the reference for anteroposterior measurements. More than 30% of the children had transverse asymmetries > or = 2 mm at the maxillary first permanent molar (paired t-test; p = 0.0001). The hig

The following two nonextraction therapy case reports demonstrate four important points: (1) Static repelling magnets, with certain characteristic parameters, distalize molars rapidly without adverse effects that are clinically discernable. Beneficial properties such as considerably reduced patient compliance requirements for force application, reduced mobility and discomfort, and mostly bodily movement are demonstrated clinically. (2) Anchorage can be controlled by using conventional techniques of anchorage augmentation or force reduction. (3) Treatment can be satisfactorily completed and docu

Farrar reported that in chronic anterior disk displacement without reduction (ADD w/o R) of the temporomandibular joint (TMJ), the range of mandibular movement gradually increases and the condylar movement normal-disk is still displaced anteriorty. The relationship between condylar rotation and translation was studied in opening/closing jaw movements before and after joint stabilization splint therapy in a patient with ADD w/o R. Movements were recorded by means of an optoelectronic jaw tracking system (Metropoly, Jaws-3D) consisting of three cameras that register the position of six light-emi

Adult patients increasingly seek invisible, nonsurgical solutions for complex skeletal Class II malocclusion with bimaxillary protrusion, hyperdivergent pattern, and gummy smile. Managing these cases with full aesthetic demands while maintaining vertical control remains challenging. This case report describes the nonextraction treatment of a 32-year-old woman with skeletal Class II due to mandibular retrusion, bimaxillary incisor proclination, increased lower facial height, unilateral Class II subdivision, midline deviation, and excessive gingival display. She was treated with a premolar-prese

OBJECTIVES: While current evidence suggests that dentofacial orthopaedic treatment neither increases nor decreases TMD signs and symptoms, corresponding data on treatment performed in adults are lacking. Therefore, the objective of this study was to assess the long-term (≥15 years) prevalence of signs and symptoms of TMD in adult Class II Herbst-multibracket appliance (MBA) patients including oral health related quality of life.
SUBJECTS AND METHODS: All patients (Department of Orthodontics, University of Giessen, Germany) in which Herbst-MBA treatment had been performed during adulthood and

INTRODUCTION: This 3-arm parallel randomized clinical trial evaluated the 3-dimensional positional changes (anteroposterior, vertical, and mediolateral) in the condyle and glenoid fossa using different Class II fixed mandibular positioners/appliances (Herbst and Xbow) compared to a control group.
METHODS: A total of 54 patients were included and randomly assigned to 1 of 3 groups (Herbst, Xbow, and control) and followed up for an average of 12.4 months. Two cone-beam computed tomography images corresponding to pretreatment and posttreatment were taken for each patient. The mandibular condyle

BACKGROUND: The orthodontic treatment of patients with a history of severe periodontitis helps to improve periodontal health, functional occlusion, and esthetics. The use of aligners in patients with a history of severe periodontitis, with a single protocol of periodontal treatment, and the report of periodontal parameters throughout the orthodontic treatment has not yet been evidenced. Therefore, this manuscript aimed to describe the treatment with a clear orthodontic aligner system in two patients with reduced periodontium associated with a history of severe periodontitis.

BACKGROUND: Although cervical headgear (CH) is a widely studied method for the correction of Class II malocclusion, there is a lack of evidence regarding the effects of early versus later treatment timing on permanent upper canines and molars.
OBJECTIVES: To evaluate the differences between early versus later cervical headgear treatment to the eruption time and inclination of maxillary permanent canines and second molars, inclination of the first molars, and the overlapping between second and third molars.
