Non-operative advances: what has happened in the last 50 years in paediatric surgery?

Paediatric surgeons remain paediatric clinicians who have the unique skill set to treat children with surgical problems that may require operative intervention. Many of the advances in paediatric surgical care have occurred outside the operating theatre and have involved significant input from medical, nursing and allied health colleagues. The establishment of neonatal intensive care units, especially those focusing on the care of surgical infants, has greatly enhanced the survival rates and long-term outcomes of those infants with major congenital anomalies requiring surgical repair. Interpreting tacrolimus concentrations during pregnancy and postpartum.

Pregnancy after solid organ transplantation, although considered high risk for maternal, fetal, and neonatal complications, has been quite successful. Tacrolimus pharmacokinetic changes during pregnancy make interpretation of whole blood trough concentrations particularly challenging. There are multiple factors that can increase the fraction of unbound tacrolimus, including but not limited to low albumin concentration and low red blood cell count. The clinical titration of dosage to maintain whole blood tacrolimus trough concentrations in the usual therapeutic range can lead to elevated unboun

Fetal exposure to ethanol has long-term effects on the severity of influenza virus infections.

Alcohol use by pregnant women is a significant public health issue despite well-described risks to the fetus including physical and intellectual growth retardation and malformations. Although clinical studies are limited, they suggest that in utero alcohol exposure also results in significant immune deficiencies in naive neonates. However, little is known about fetal alcohol exposure (FAE) effects on adult infections. Therefore, to determine the long-term effects of FAE on disease susceptibility and the adult immune system, we infected FAE adult mice with influenza virus. In this study, we dem

Are oral contraceptive use and pregnancy complications risk factors for atopic disorders among offspring?

In utero programming of atopic manifestations has been suggested. We investigated the association between oral contraceptive (OC) use before, and complications during pregnancy (CDP) and asthma, along with other atopic manifestations. The study is based on neonates from Kingston and St Andrew, a geographic subcohort from the Jamaican Perinatal Morbidity, Mortality Survey conducted in 1986-1987. Information on OC use and CDP was extracted from maternal interviews and medical records. In a follow up in 1997-1998, via interviews with mothers, trained nurses collected information on asthma/wheezin

Management of group B streptococcal bacteriuria in pregnancy.

OBJECTIVE: To provide information regarding the management of group B streptococcal (GBS) bacteriuria to midwives, nurses, and physicians who are providing obstetrical care. OUTCOMES: The outcomes considered were neonatal GBS disease, preterm birth, pyelonephritis, chorioamnionitis, and recurrence of GBS colonization. EVIDENCE: Medline, PubMed, and the Cochrane database were searched for articles published in English to December 2010 on the topic of GBS bacteriuria in pregnancy. Bacteriuria is defined in this clinical practice guideline as the presence of bacteria in urine, regardless of the

Quantitative evaluation of nursing intervention efficacy in high-risk hypertensive disorders of pregnancy: A retrospective quasi-experimental study.

Hypertensive disorders of pregnancy (HDP) contribute significantly to maternal and neonatal morbidity. Precision medicine emphasizes individualized risk prediction, while nursing interventions (NIs) address behavioral and psychosocial care. This study evaluated the clinical benefit of a structured NI compared to routine prenatal care in reducing complications among women diagnosed with HDP. This retrospective quasi-experimental study was conducted at Peking University Shenzhen Hospital. Participants were primigravida Chinese women (median age 25 years) with a confirmed diagnosis of HDP. Of 326

Ethical aspects of artificial intelligence use in neonatal intensive care units: a scoping review.

UNLABELLED: This study aims to map ethical, legal, social, professional, and implementation issues associated with artificial intelligence (AI) in neonatal intensive care units (NICUs). A JBI-informed scoping review using the population-concept-context framework was reported according to PRISMA-ScR. Peer-reviewed English-language articles published from 1 January 2016 to 31 May 2026 were eligible. Because the original exports and screening log were unavailable, a documented updated rerun was completed on 13 July 2026 using public PubMed-indexed bibliographic searching, supplementary publisher

Cross-cultural beliefs and practices of caregivers regarding sunlight exposure for newborn jaundice: a scoping review.

BACKGROUND: Neonatal jaundice affects most newborns worldwide and remains a major cause of preventable bilirubin-induced neurological injury in low-resource settings. Although most clinical guidelines recommend standardised electrically powered phototherapy and discourage sunlight exposure for newborns with jaundice due to safety concerns, uncontrolled exposure to direct sunlight remains widespread in many communities. We aimed to chart cross-cultural beliefs and practices regarding sunlight exposure for newborn jaundice to inform safer newborn care. METHODS: We conducted a scoping review usi

Maternal perinatal transfer of vitamins and trace elements to piglets.

Nursing piglets are entirely dependent, for their micronutrient provisions, upon in utero, colostrum and milk transfers from the dam. An adequate maternal transfer of micronutrients is all the more important during these periods which, in fact, lasts for approximately half the life cycle (conception to slaughter) of modern pigs. The present study aimed to set up a simple approach to assess the maternal perinatal transfer of vitamins and trace elements in sows. Prenatal transfer (R-u) was estimated as limited, passive or active using the ratio between pre-colostral serum concentrations of a giv

Association between psychosocial barriers, family-centered care, and caregiver presence at the bedside in a pediatric post-acute care hospital: A qualitative study.

PURPOSE: Children with medical complexities are often in need of prolonged admissions to post-acute care hospitals. Caregiver presence at the bedside has been associated with improved outcomes for pediatric patients and caregivers. Ecological barriers have been found to impact caregiver presence at the bedside during pediatric admissions. While caregiver mood, coping, and family-centered care have been associated with caregiver presence in neonatal and pediatric intensive care units, less is known about post-acute admissions. The present qualitative study explored caregivers' psychosocial barr

Managing medicines in decentralization: discrepancies between national policies and local practices in primary healthcare settings in Indonesia.

In Indonesia, primary health centres (PHCs) are mandated to provide essential medicines to ensure equal access to medication for all Indonesians, as stated in the national medicine policy. However, limited information is available regarding the actual practices of health workers within the context of decentralized governance. This paper investigates the discrepancies between national policies and local practices in two Indonesian districts, shedding light on coping mechanisms employed in each phase of medicine management within PHCs. The mixed-method study began by identifying pertinent polici

Birth Weight and Adult-Onset Hearing Loss.

OBJECTIVES: Among low-birth-weight infants, exposure to stress or undernutrition in utero may adversely affect cochlear development. As cochlear reserve declines, the risk of hearing loss may increase with age. While low birth weight is associated with a higher risk of neonatal hearing loss, our objective was to examine whether birth weight was associated with adult-onset, self-reported hearing loss in the Nurses' Health Studies (NHS) I and II (n = 113,130). DESIGN: We used Cox proportional hazards regression to prospectively examine whether birth weight, as well as gestational age at birth,
