In a fatal case of neuroleptic malignant syndrome, a muscle sample taken within 1 h of death showed acute myopathic features with absence of muscle glycogen and neutral lipid. These features suggest that hyperpyrexia in this syndrome may be caused by heat production from uncoupled phosphorylation in muscle and imply that the primary biochemical abnormality responsible for this uncontrolled heat production might be muscular rather than hypothalamic.

Using clinical and computed tomography (CT) criteria, an analysis of 2,000 consecutive stroke unit patients from 1977 to 1984 revealed 30 patients with cerebellar infarction. The case fatality rate was 23%, higher than for any other location of brain infarction studied over the same period. Death was most often due to concomitant brainstem infarction. Obstructive hydrocephalus occurred in 4 patients (13%), and in 2 cases diagnosis, facilitated by urgent CT scanning, allowed early surgical intervention that was life saving. One mammary gland of each of nine primum partum suckling ewes was inoculated with ureaplasma 8 weeks post-partum. Infected glands were swollen, hot, and painful from 16 hours post-infection. A 49-year-old woman underwent 1 year (six injections) of botulinum toxin treatments for essential blepharospasm. When she became dissatisfied with the need for repeat injections, she elected to undergo an orbicularis muscle stripping procedure for control of spasms. Pathologic examination of the orbicularis muscles showed only minimal degenerative changes. Botulinum toxin injection into the orbicularis is known to provide only temporary relief of essential blepharospasm. This may be because the resulting orbicularis atrophy is insufficient to maintain permanent control of these spasms.

It is difficult to recognize tendon pathology in the ankle area when there is no open wound. This manuscript was written to indicate subtle characteristics associated with tendon trauma around the ankle. Failure to recognize these problems will result in improper treatment or lack of treatment and the results may be disastrous to the patient. Diagnostic aids, such as radiographs, ultrasound, or tenography, may be valuable; however, a careful physical examination should make it possible to diagnose correctly and to treat tendon pathology surrounding the ankle.

A case of a full-term female with confirmed trisomy 18 who died a few hours after her delivery is presented. In addition to many severe systemic malformations, some ocular findings are described. They include cataract and hypoplasia of optic nerves in both eyes, and juxtapapillary coloboma, retinal dysplasia and Bergmeister's papilla in the left eye. The last finding is a new one in trisomy 18.

The transoral approach is a surgical technique that can be used effectively in the treatment of anterior craniovertebral junction (CVJ) pathology. In this review, we outline the signs, symptoms, and diagnostic studies that identify the pathology and help to determine subsequent therapy. The transoral approach, preoperative and postoperative management problems, and potential complications of the procedure are reviewed. The nursing care of a patient with CVJ pathology is discussed.
