Bilateral pectoral myocutaneous advancement flaps and anatomic sternal wound reconstruction in cyanotic infants with mediastinitis.

OBJECTIVE: The purpose of this study was to assess the results and applicability of a modified chest closure technique employing bilateral pectoral myocutaneous advancement flaps after sternal re-approximation for postoperative mediastinitis in cyanotic infants. METHODS: The study population is of a single surgeon's pediatric cardiac experience (n = 253) over a 2-year period. With retrospective hospital chart review six cases with deep sternal wound complications were identified (five mediastinitis and one hypoxemic wound necrosis). Sternal wound reconstruction was done with the above techniq

Does good trunk extensor performance protect against back-related work disability?

In this cohort study, dynamic trunk extensor performance was studied as a predictor of permanent work disability due to back disorders. As part of the comprehensive Mini-Finland Health Survey in 1978-80, the back muscle performance of 535 persons (267 men, 268 women) was measured using standardized repetitive arch-up and sit-up tests. At baseline, the participants were between 30 and 64 years of age. Retirements were followed for 12 years on average. During the follow-up, 56 subjects developed permanent work disability; 15 of these cases were back-related. Good dynamic trunk extensor performan

Effects of Reduced-Level Coated Inorganic Compound Trace Minerals on Breeder Hen Performance, Eggshell Quality, Antioxidant Status, and Progeny Development Compared with an Inorganic Premix.

This study evaluated the effects of reduced inclusion levels of coated inorganic compound trace elements as a reduced substitution for uncoated inorganic trace elements in layer breeder diets. A total of 360 Hy-Line Brown layer breeder hens were randomly assigned to three groups, with six replicate pens of 20 hens per group: control group A (inorganic trace elements), group B (500 mg/kg coated inorganic compound trace elements), and group C (1000 mg/kg coated inorganic compound trace elements). Compared with the control, the coated trace element groups significantly increased the laying rate,

Mediastinal tracheostomy: techniques, outcomes and quality of life-a systematic review.

BACKGROUND: Mediastinal tracheostomy (MT) is a rare, technically demanding salvage procedure in which the distal trachea is redirected into the upper mediastinum. It is indicated when extensive resection of the proximal trachea precludes conventional tracheal reconstruction or cervical tracheostomy. It is performed for a variety of underlying diseases. The aim of this systematic review was to summarize the available evidence on operative techniques, perioperative outcomes, disease-specific outcomes, and quality of life. METHODS: A systematic review was conducted. PubMed was searched for case

Arthroscopic Biceps Tenodesis With Interference Screw Fixation: A Technique Video.

BACKGROUND: Management of long head of the biceps tendon (LHBT) pathology is accomplished through a biceps tenotomy or tenodesis. While both modalities provide beneficial outcomes, a biceps tenodesis may confer improved cosmesis, functional outcomes, and decreased muscle cramping postoperatively. Many procedural considerations are undertaken prior to a tenodesis, such as the surgical approach and fixation device. While similar clinical outcomes are achieved between an open subpectoral and arthroscopic suprapectoral biceps tenodesis (ASPBT) with interference screw (IS) fixation, the latter tech

Vertical mammaplasty.

Current criticisms regarding vertical mammaplasty include problems with poor immediate postoperative appearance, nipple-areola complex malposition, and excessive lower pole length. These problems can be avoided by proper patient selection, by utilizing correct concepts of skin design, and by observing correct glandular resection and closure concepts. Vertical mammaplasty also can result in other problems, such as hypertrophic circumareolar scars and lower pole deformities, including notching, boxy shape, infra-areolar depression, and flatness. These problems are also largely avoidable by using

Retrospective Study on Optimizing Breast Augmentation Outcomes in Transgender Patients: A Comprehensive Analysis of Tumescent Local Anesthesia Technique.

BACKGROUND: Tumescent local anesthesia (TLA) involves the infusion of a saline solution containing lidocaine and epinephrine into tissues to achieve localized anesthesia and vasoconstriction. While the use of TLA in sub-glandular augmentation mammoplasty has been documented, we present a modified TLA approach for primary sub-muscular breast augmentation in transgender patients based on our experience over the past years. METHODS: Between the years 2014 and 2021, we performed primary sub-muscular breast augmentation on 20 transgender patients under TLA and conscious sedation. The tumescent sol

Video Analysis of Pectoralis Major Injuries in Professional Australian Football Players.

BACKGROUND: There is little evidence regarding the mechanisms of pectoralis major (PM) injury and player outcomes in Australian Football League (AFL) players. PURPOSES/HYPOTHESIS: The study aims were to investigate (1) the mechanisms of PM muscle injury in elite AFL players via video analysis and (2) the player profile, method of management, and clinical outcomes of the PM injuries sustained. We hypothesized that the majority of PM tears would occur in outer-range PM positions (hyperextension of the glenohumeral joint). STUDY DESIGN: Case series; Level of evidence, 4. METHODS: We analyzed v

Transvenous cardioverter defibrillator lead malfunction due to terminal connector damage in pectoral implants.

Lead failure places patients with implantable cardioverter defibrillators (ICD) at risk for sudden cardiac death or results in delivery of inappropriate shocks. This study describes a mechanism of lead malfunction occurring at the junction of the terminal ring with the conductor coil of the rate sensing terminal connector in one specific model of a transvenous ICD lead. We detected the problem in a population of 179 patients with a mean age of 61 +/- 10 years and a mean lead implant duration of 16 +/- 11 months. All patients underwent pectoral ICD implantation using a submuscular approach. The

Minimally Invasive Subscapularis Release for Internal Rotation Contracture of the Shoulder in Residual Brachial Plexus Birth Injury.

BACKGROUND: Shoulder internal rotation contracture is one of the most common problems observed in patients with residual brachial plexus birth injury1,2. Minimally invasive subscapularis release is a simple extra-articular procedure that involves the release of the subscapularis origin from the undersurface of the scapula. This procedure addresses the contracture and has been shown to result in remodeling of the glenohumeral joint when concomitant conjoined tendon transfer is performed3. DESCRIPTION: The procedure is performed with the patient in the lateral decubitus position. The procedure

Surgical treatment of Candida albicans spondylodiscitis.

IntroductionSpinal infection poses a demanding diagnostic and treatment problem for which a multidisciplinary approach with spine surgeons, radiologists, and infectious disease specialists is required. Infections are usually caused by bacterial microorganisms, although fungal infections can also occur. Most patients with spinal infections diagnosed in the early stages can be successfully managed conservatively with antibiotics, bed rest, and spinal braces. In cases of gross or pending instability, progressive neurological deficits, failure of conservative treatment, spinal abscess formation, s

Erector Spinae versus Surgically Placed Pain Catheters for Thoracic Outlet Decompression.

BACKGROUND: Perioperative care after surgery for thoracic outlet syndrome (TOS) involves multimodal pain control. Pain catheters with bupivacaine infusion are a modality to minimize perioperative narcotic use. Our study aims to compare surgically placed pain catheters (SP) with erector spinae pain catheters (ESP) placed by the anesthesia pain service. METHODS: Retrospective review of a prospectively maintained surgical TOS database identified patients undergoing transaxillary first rib resection (FRR) who had either SP or ESP placed for pain control. Patients were matched for age and gender.
