[Acanthamoeba keratitis. Morphologic and isoenzymatic study of a strain isolated from a medically treated case].

We report here the third case of Acanthamoeba keratitis discovered in France in a 30 year-old man wearing soft contact lenses. Amoebas of Acanthamoeba genus could be isolated twice from his corneal ulcer with simple ocular swabs. Owing to medical treatment alone, a successful healing of his eye was obtained. The isolated strain was studied morphologically and biochemically: the cysts observed microscopically after silver stain possessed less than six arms surrounded by a non reticulated ectocyst and belonged to group II defined by Pussard and Pons. But the isoenzymatic study of the ocular stra…

Normal conduction in pathways traversing an asymptomatic multiple sclerosis plaque.

A 31-year-old woman developed right facial myokymia as the initial manifestation of multiple sclerosis (MS). An MRI scan revealed a focal signal abnormality confined to the left dorsolateral pontomedullary region. Brain-stem auditory evoked potentials (BAEPs), somatosensory evoked potentials (SEPs), and blink reflex (BR) failed to show a conduction abnormality through the left brain-stem lesion. Instead, BAEP and BR indicated a conduction defect in the right pons and EMG showed myokymic discharges in right facial muscles. Our findings provide rare documentation of normal conduction through a p…

Occult pontine glioma in a patient with hemifacial spasm.

Hemifacial spasm due to an intracranial mass lesion is rare. We describe a 29-year-old man with hemifacial spasm successfully treated with botulinum A toxin injections for 2 years. The development of acquired diplopia secondary to acquired sixth cranial nerve palsy prompted investigation. Computed tomography done at the time of original diagnosis and on three other occasions (concentrating on the brain stem and cerebellopontine angle) failed to demonstrate an intracranial mass lesion. Magnetic resonance imaging (MRI) showed a large mass lesion in the pons presumed to be a glioma. Patients with…

[X-ray computed tomography in central pontine myelinolysis. 2 cases].

Two cases of central pontine myelinolysis were studied by computerized tomography (CT). One patient had chronic alcoholism, the other porphyria variegata; both initially presented with water-and-electrolyte disorders, notably hyponatraemia. The neurological disorders consisted of acute pseudobulbar syndrome which totally regressed within 15 days to 1 month. CT demonstrated a low density area in the pons, extending to the mesencephalon in one case. Despite clinical cure, this low density persisted for 16 and 20 months respectively, counting from the onset of neurological symptoms. In central po…

Diurnal variation in ornithine decarboxylase activity of different brain regions of the rat.

The activity of ornithine decarboxylase (ODC) was studied in the brain of Sprague-Dawley male rats weighing 200-250 g. In this study, animals were maintained at a temperature of 21 +/- 1 degree C and a 12:12 h light--dark cycle for a minimum of 3 weeks prior to any experimentation. Animals were sacrificed at 06.00 h and 18.00 h and the brain cerebral cortex, hypothalamus, amygdala, hippocampus, midbrain, pons, medulla oblongata and cerebellum were isolated and assayed for ODC activity. The data show significant diurnal variations (P less than 0.05) in hypothalamus, midbrain, medulla oblongata …

Effects of pyrithiamin and oxythiamin on acetylcholine levels and utilization in rat brain.

Regional cerebral acetylcholine (ACh) levels and utilization rate were assessed in vivo in rats rendered thiamin deficient using the thiamin antagonists pyrithiamin or oxythiamin. ACh levels were significantly reduced in all brain regions of pyrithiamin treated rats and in the medulla-pons and striatum of oxythiamin treated rats compared to controls. ACh utilization was significantly reduced in the midbrain, striatum and hippocampus of pyrithiamin treated rats, but was reduced only in the striatum of oxythiamin treated rats compared to controls. Thus, there are some reductions in ACh levels an…

Fisher one-and-a-half syndrome due to a bulb protuberance cavernoma.

The one and a half Fisher syndrome is produced by an unilateral lesion in the dorsal and inferior part of the pons located specifically in the pontine tegmentum. This is an area that contains the medial longitudinal fasciculus (LMF), the paramedian pontine reticular formation (PPRF) and the nucleus of the sixth cranial nerve, responsible for the horizontal movements of the gaze. Clinically it manifests with conjugate gaze palsy with internuclear ophthalmoplegia. The case is reported on a 60 year-old patient, who presented with binocular diplopia, ophthalmoplegia and paralysis of the gaze conju…

Xylometazoline abuse induced ischemic stroke in a young adult.

INTRODUCTION: substance abuse is an important cause of ischemic stroke in the young. This includes over-the-counter dietary supplements and cough and cold remedies, which were reported to be an independent risk factor for hemorrhagic stroke. CASE REPORT: this article describes a young male patient with acute ischemic infarctions in the posterior inferior cerebellar and posterior cerebral artery territories bilaterally, the right cerebral peduncle, the left pontine tegmentum, and lateral pons following abuse of xylometazoline-containing nasal decongestant for 10 years. CONCLUSION: this is the…

Cerebellopontine angle tumors: role of magnetic resonance imaging.

The cerebellopontine (CP) angle is bound anterolaterally by the posterior aspect of the petrous temporal bone and posteromedially by the cerebellum and pons. It contains important vascular structures and cranial nerves and is subject to a certain gamut of lesions, notably tumors with interesting radiological manifestations. Radiological investigation of these lesions has seen significant improvement in recent decades. Magnetic resonance is the imaging modality of choice for lesions of the CP angle and internal auditory canal. Lesions of the CP angles usually are divided into those native to th…

[Clinical and pathologic-anatomic findings in acute basilar thromboses. A study of 30 autopsy verified cases].

In a retrospective study of 30 cases with fatal basilar artery thrombosis we compared clinical and autopsy findings for clinicopathological correlation. There are only few clinical symptoms which point to a definite localisation and extent of thrombosis or cerebral infarction. In most cases loss of consciousness and hemi- or tetraparesis are due to pontine or mesencephalic infarction. Palsy of the facial nerve suggests infarction of the pons. The incidence of cranial nerve involvement is too low to allow localisation of infarction. There was no case showing infarction of the diencephalon or th…

Pathological report of four patients presenting with cranial dystonias.

Secondary dystonias and experimental models of dystonia suggest that mechanisms responsible for primary dystonias may lie in the basal ganglia or brainstem. A histological study has been done in three patients with cranial dystonia (blepharospasm with oromandibular dystonia in two, blepharospasm alone in one), and one patient with craniocervical dystonia (oromandibular dystonia with retrocollis). In the patient with blepharospasm alone, an angioma, 0.5 mm in diameter, was found in the dorsal pons at the site of the central tegmental tract, confirming that some patients presenting with primary …

Effect of substance P on neonatally axotomized noradrenaline neurons in rat brain.

Transection (axotomy) of the dorsal tegmental noradrenaline bundle in the neonatal stage leads to a permanent degeneration of noradrenaline … in the neocortex, whereas dose-dependent and significant increases were found in the cerebellum and pons-medulla. The results indicate that substance P may have a growth-stimulatory effect on damaged locus coeruleus noradrenaline neurons in the CNS during ontogeny.

[Obliterating parietal hemodissection or vertebro-basilar dissecting aneurysm].

The authors report the pathological and clinical findings of a dissecting aneursym of the basilar artery in a 32 year old man up till then in good health, who died two months later. The pathological study showed bilateral lesions of osteo-malacia in the neighborhood of the pons due to hemo-dissection affecting the right vertebral artery just before it enters the dura-mater, then its intra-cranial portion, the basilar trunk and the first part of the right posterior cerebral artery. The originality of this case resides in the finding of segmental stenosing panarteritis of the same right vertebra…

Gliomas (astrocytomas) of the brain-stem with spinal intra- and extradural metastases: report of three cases.

Astrocytomas of the pons and medulla oblongata ('brain-stem gliomas') while often invasive locally, do not as a rule seed and metastasize along the spinal meninges. Three cases are here reported (two adults, one child) in whom astrocytoma of the brain-stem metastasized along the spinal cord. The dura mater itself and the spinal epidural space were invaded in two cases. The child and one adult had a pontine astrocytoma, the other adult's tumour originated in the medulla oblongata. In the two cases that came to necropsy the tumour of the brain-stem was much better differentiated than the meninge…
