BACKGROUND: Sarcoma and its treatment has the potential to limit adult height attainment in skeletally immature patients. However, evidence on the extent of this limitation in sarcoma specifically is mixed, and existing height prediction tools such as the Paley multiplier method have proven unreliable in this setting. As such, orthopaedic surgeons are left with the challenge of counseling patients and their families on expected height deficits without an adequate understanding of the extent of these deficits. Equally important, these surgeons must also understand the amount of skeletal growth

Embryonal tumours most commonly occur in the first few years of life and account for approximately 30% of childhood malignancies. Knowledge of these tumours' genetics has already impacted on their clinical management and further knowledge of their cellular immortalization will hopefully result in novel therapies. The ends of human chromosomes are capped and protected by telomeres; cellular replication, however, causes their loss. A critical length of telomere repeats is required to ensure proper telomere function and avoid the activation of DNA damage pathways that result in senescence and cel

Diseases of the jaw bones in children can be monostotic or polyostotic. Examples are fibrous dysplasia and Langerhans cell granulomatosis. Sarcomas of the jaw bones are rare, but do occur with some preference in children. The dentist should be aware of the clinical and even more so of the rather typical radiologic changes in such diseases.

In immature long bones, radical excision of malignant tumours of the metaphysis may necessitate sacrifice of the adjacent epiphysis. To preserve the adjacent joint while allowing a safe margin of excision, we used physeal distraction before removing the tumour. From July 1984 to August 1992, we operated on 20 patients by this method. After a mean follow-up of 54 months there was no local recurrence in the epiphyseal region. Three patients had developed pulmonary metastases.

Wilms' tumors affecting adults are rare and are thought to have a worse prognosis than similar stage tumors in the pediatric population. To understand these tumors better, the authors reviewed their multi-institutional experience in a series of nine lesions diagnosed as Wilms' tumors in adults. In addition to histologic and immunohistochemical examination, they performed cytogenetic analysis and fluorescence in situ hybridization. On review, four cases were reclassified: two "blastema only" as Ewing's sarcoma/primitive neuroectodermal tumor and the other two as clear cell sarcoma of soft parts

BACKGROUND: Primary osseous sarcomas of the spine provide a challenge due to their anatomic location and aggressiveness. While chemotherapy, radiation, and surgery are frequently employed as treatments for these neoplasms, their efficacy has not been compared between children and adults.
METHODS: Using the Surveillance, Epidemiology, and End Results database (2000-2021), 455 patients were identified with confirmed osteosarcoma, Ewing's sarcoma, or chondrosarcoma. Demographic, tumor, and treatment characteristics were analyzed. Cox proportional hazard models were used to evaluate mortality pre…

INTRODUCTION: The odds of metastatic disease at diagnosis of bone (BS) and soft-tissue sarcomas (STS) of the extremities and pelvis may vary among patients due to several factors. There is limited research comparing the rates of metastatic disease at diagnosis in patients from different demographic and socioeconomic backgrounds.
METHODS: Patients with a primary BS or STS of the extremity or pelvis were identified using International Classification of Diseases codes. Sex, ethnicity, race, insurance type, and preferred language were collected from the medical records. Cross-tabulations and odds

Glypican-3 (GPC3) is a proteoglycan thought to play an important role during development. Germline GPC3 mutations are seen in the rare Simpson-Golabi-Behmel syndrome (SGBS), which predisposes patients to Wilms tumor, hepatoblastoma, and neuroblastoma. While numerous adult tumors have been evaluated by immunohistochemistry for GPC3, no comprehensive assessment has been done in pediatric tumors. We therefore investigated GPC3 expression in 143 pediatric central nervous system (CNS) tumors and 271 non-CNS tumors. Among non-CNS tumors, GPC3 expression was seen in 9/9 (100%) hepatoblastomas, 4/6 (6

Thirty-two patients affected with skeletal conditions were examined with MRI using Short TI Inversion Recovery sequence and Spectral Presaturation with Inversion Recovery (SPIR) sequence as well as Spin-Echo (SE) T1-weighted sequence and Fast Spin-Echo (FSE) T2-weighted sequence to compare their value in the assessment of skeletal lesions. SPIR sequence was performed after intravenous injection of Gd-DTPA. The lesions included primary bone tumors (10 cases: 1 osteosarcoma, 1 periosteal sarcoma, 1 Ewing's sarcoma, 1 chondrosarcoma, 2 non-ossifying fibromas, 1 chondroma, 1 chondromyxoid fibroma,

BACKGROUND: Mesothelioma is a rare pleural tumor. Compared to diffuse pleural mesothelioma, localized pleural mesothelioma carries a better prognosis and may be managed with surgical resection. Among histologic subtypes, the sarcomatoid variant is the least common but is associated with the poorest outcome. Diagnosis remains challenging, with immunohistochemistry playing a central role in confirming the disease. While surgical resection is the cornerstone of management, chemotherapy, immunotherapy and radiotherapy are considered in unresectable cases. Overall, mesothelioma continues to carry a

The sacrum can harbor a diverse group of both benign and malignant tumors, including metastases. Primary tumors of the sacrum can arise from bone, cartilage, marrow, notochordal remnants, or surrounding nerves and vessels. Among a variety of primary tumors of the spine, chordoma, germ cell tumors and Ewing's sarcoma are recognized for their propensity to occur in the sacrum. Imaging is essential in diagnosis, pretreatment evaluation, and assessment of response to treatment. Radiography, CT and MRI are the primary modalities in assessing morphology and tumor extent whereas PET/CT is crucial in

Opioid drugs are the most powerful painkillers known. Thus, they are the pivotal treatment for the severe pain often associated with oncological disease. Most of their side effects are known and common. Opioid-induced hyperalgesia (OIH) is one of their unknown and uncommon side effects. This phenomenon is characterized by a paradoxical reaction to opioid administration. In this condition, an increase in the dose and frequency of opioid administration aggravates the pain instead of reducing it. Misdiagnosis of this phenomenon can result in excessive opioids usage which does not relieve the pain

BACKGROUND: Tumors involving the hand skeleton are rare. However, a basic knowledge of hand tumors is necessary for every clinician. This is due to the importance of distinguishing typical benign tumors from life or limb threatening malignant ones.
METHODS: This study is a review of 99 cases of osseous hand tumors presented to the department of orthopedic surgery, Imam Khomeini Hospital in Tehran, Iran, from December 1990 to February 2011.
RESULTS: Ninety-one cases were benign osseous tumors of the hand and eight tumors were malignant which four of them were considered as primary and four co

BACKGROUND: Cytological examination of pleural fluid is one of the most informative laboratory procedures in the diagnosis of pleural effusions. Although tuberculosis is the commonest cause of pleural effusions in developing countries, tumours, including grade ones, can present with effusions.
OBJECTIVE: The aim of the present study was to evaluate the uncommon causes of malignant pleural effusion.
METHODS: A 2-year retrospective analysis of pleural fluid cytological specimens submitted to the Department of Cytopathology, PGIMER, Chandigarh between January 2003 and December 2004 was performe

The management of pathological fractures in children remains controversial. The indications for surgical treatment are unclear and the need for histological diagnosis before or after definitive treatment is not clearly defined. We reviewed retrospectively the records of all patients under the age of 16 years who presented over the past 7 years with a fracture as the first manifestation of bone pathology. There were 23 patients (16 boys and 7 girls) of an average age of 12 years and 2 months (range 4.1-15.8 years). There were nine cases of fracture through a simple bone cyst, five cases of fibr

The outcome of tibial allograft reconstruction after resection of a tumour is inconsistent and has a high rate of failure. There are few reports on the use of tibial allografts in children with open growth plates. We performed 21 allograft reconstructions (16 osteoarticular, five intercalary) in 19 consecutive patients between seven and 17 years of age. Two had Ewing's sarcoma, one an adamantinoma and 16 osteosarcoma, one with multifocal disease. Five patients have died; the other 14 were free from disease at the time of follow-up. Six surviving patients (eight allograft reconstructions) conti
