Acute Scaphoid Waist Fracture in the Athlete.

Scaphoid fractures are the most common carpal fracture and the most challenging. Although appropriately managing acute scaphoid waist fractures is a priority, it also is of primary importance to make a diagnosis acutely. Scaphoid waist fractures can occur with low-energy trauma and lead to mild symptoms. A tendency to minimize symptoms and low level of initial disability lead to delay in diagnosis. Displaced scaphoid fractures require operative intervention uniformly. Although nondisplaced fractures can heal with nonoperative treatment, management of these injuries is affected by patient deman

Rising incidence of scaphoid fracture surgery in Finland.

UNLABELLED: The aim of the present study was to investigate the incidence of scaphoid fracture surgery by obtaining data from the national discharge register in Finland, a country of 5 million inhabitants, for the period from 1997 to 2014. A total of 1380 patients with scaphoid fracture were treated surgically. Half of the patients were aged under 28 years and 84% were men. The surgical treatment of scaphoid fractures was classified into two groups. There were 640 (46%) primary fracture fixations and 740 (54%) treatments of fracture nonunion. The overall incidence of all scaphoid fixations inc

Chronic Scapholunate Ligament Injuries: Treatment with Supplemental Fixation.

Treatment of chronic scapholunate ligament injuries can be challenging. Traditional reconstructive techniques, including varied capsulodeses and tenodeses often yield inconsistent results with loss of reduction and radiographic deterioration. As a result, supplemental hardware fixation has become more popular and may allow more robust stabilization of the scapholunate reconstruction. However, these procedures have complications and few data regarding outcomes are currently available. This article evaluates the role of supplemental fixation in the management of chronic scapholunate instability.

Chronic Scapholunate Ligament Injury: Techniques in Repair and Reconstruction.

Although the true incidence of scapholunate interosseous ligament (SLIL) injury is unknown, a study found that 35% of cadaveric wrists had some degree of scapholunate tear. Of those wrists with SLIL injury, 29% had evidence of arthrosis. Early recognition and treatment of these injuries can delay or prevent the onset of arthritis. This article details treatment options for SLIL injury across the spectrum of pathology with a particular emphasis on chronic scapholunate repair and reconstruction. New techniques and outcomes data also are presented.

A new mechanism of injury for scaphoid fractures: 'test your strength' punch-bag machines.

We investigated the mechanism of injury in a consecutive series of 153 confirmed scaphoid fractures. We found that ten (6%) occurred as a result of using a 'test your strength' punch-bag machine, a device found in public houses and amusement arcades. Clinicians should be aware that scaphoid fractures can occur as a result of punching these machines or other similar objects.

[Dynamic mesh of carpal scaphoid].

Despite the use of cannulated compression screws, it is still difficult to screw non-displaced fractures of the scaphoid percutaneously. That is due in particular to the difficulty in assessing the correct position for the guide pin from the 2D fluoroscopic images. This work is designed to enable 3D visualisation of the scaphoid during surgical operations by using the technique of dynamic meshing and having the image appearing on a computer screen rather than as a mental image. In this context, the MEFP3C software includes applications for converting a generic scaphoid into a virtual scaphoid,

Cost effectiveness of adding magnetic resonance imaging to the usual management of suspected scaphoid fractures.

OBJECTIVE: To determine the cost effectiveness of a magnetic resonance imaging scan (MRI) within 5 days of injury compared with the usual management of occult scaphoid fracture. METHODS: All patients with suspected scaphoid fractures in five hospitals were invited to participate in a randomised controlled trial of usual treatment with or without an MRI scan. Healthcare costs were compared, and a cost effectiveness analysis of the use of MRI in this scenario was performed. RESULTS: Twenty eight of the 37 patients identified were randomised: 17 in the control group, 11 in the MRI group. The gr

Cast adrift: Gortex cast liners allow greater patient activity.

Extremity fractures are a common injury, with nearly 1.5 million cases reported in the United States in 1998. Treatment often involves lengthy periods of immobilization. This report outlines the use of a Gortex cast liner by a subject who was able to engage in swimming and scuba diving during the healing process. We report that a Gortex cast liner may be considered for an active patient who is keen to return to limited activities during fracture healing. Apparently because of a lack of knowledge of their existence, physicians currently are underutilizing this method of casting in active patien

The hysteresis effect in carpal kinematics.

PURPOSE: Carpal bones show hysteresis that is dependent on the direction of wrist motion during a continuous active loading protocol. We describe an accurate methodology for analyzing the hysteresis effect and we apply this model to analyze the effect of sequential ligament sectioning on scapholunate instability. METHODS: In 8 fresh cadaver forearms scaphoid, lunate, and third metacarpal motions were recorded while each wrist was moved in continuous cycles of active motion in flexion-extension and radioulnar deviation. Motions were analyzed for the intact state and after sequential sectioning

Carpal tunnel volume distribution and morphology changes with flexion-extension and radial-ulnar deviation wrist postures.

Non-neutral wrist postures have been reported to cause decreased carpal tunnel volume (CTV) contributing to impingement of the median nerve and development of carpal tunnel syndrome. Recent analysis found CTV did not change with ±20° flexion-extension (FE), however, CTV decreased with ulnar deviation over the range of -5° to 15° radial-ulnar deviation (RUD). These findings suggest CTV may be too coarse of a measure to reflect the effects of slight non-neutral postures, or that volume is conserved and redistributed due to changes in tunnel morphology with posture. The objective of this study wa

The Impact of Scaphoid Malunion on Radioscaphoid Joint Contact: A Computational Analysis.

PURPOSE: The clinical relevance of scaphoid malunion is controversial because the biomechanical sequelae remain poorly understood. In this computational study, the effect of increasing scaphoid malunion on radioscaphoid joint contact was assessed. METHODS: Six computational wrist models of active wrist flexion-extension were used to examine 6 scaphoid malunions of varying severities. The malunions were computationally created using 3-dimensional imaging software. Each scaphoid was shortened at the waist by 2 mm to simulate fracture comminution and the distal pole was angulated volarly from 15

[3D analysis and computer assisted reconstruction for scaphoid non-union].

The odd shape of the scaphoid is a challenge to our spatial sense. Computer assistance is of an unmatched value when reconstructing a non-united scaphoid: From CT data a true 3-D-model can be generated, fully interactive; thus it can be moved, manipulated and of course also printed for hands-on experience. Comparing the virtual 3-D-models of the nonunion with the healthy contralateral scaphoid, the exact amount of the deformity is calculated which allows for the planning of an anatomically precise reconstruction of the scaphoid shape. Finally, computer generated patient specific instruments wi

The Association Between Lunate Morphology and Scaphoid Fractures: A Comparative Radiographic Assessment.

PURPOSE: This study compared lunate morphology on wrist radiographs from patients with scaphoid fractures with those from a control group without scaphoid fractures. METHODS: Patients with scaphoid fractures diagnosed between 2015 and 2023 were reviewed retrospectively. Wrist radiographs were assessed to determine lunate morphology, capitate-triquetrum distance, and scaphoid fracture location. The lunate morphology was assessed according to both the midcarpal (Viegas classification) and radiocarpal (Antuña-Zapico classification) joints. The results were compared with 200 control wrist radiogr

The Medial Femoral Trochlea Osteochondral Flap for Scaphoid Reconstruction: A Systematic Review.

BACKGROUND: The medial femoral trochlea flap has been used to resurface scaphoids with recalcitrant proximal pole fractures or avascular necrosis, providing vascularized osteochondral tissue with similar morphological characteristics. This article aims to review the contemporary literature on its use for scaphoid reconstruction. METHODS: A systematic review of Embase, PubMed, Cochrane Central Register of Controlled Trials, and MEDLINE assessed the use of medial femoral trochlea flaps in scaphoids. RESULTS: Eight studies were included, with 76 patients at a mean age of 26 years. Forty-three p
