The percutaneous repair of the calcaneal tendon (CT) places the sural nerve (SN) at high risk for injury up to 60%. The aim of our study, therefore, was to explore and describe the course of SN in relation to the CT and to provide an anatomical description of the area in which the SN resides in order to assist surgeons in avoiding iatrogenic injury during surgical procedures in the leg. Forty-four lower extremities of 22 adult cadavers were dissected and the course of the sural nerve investigated. The CT was divided into ten horizontal equal fractions. The widths of CT, and horizontal distance

INTRODUCTION: Reports of arterial injuries from both the civilian and military arenas report the brachial artery as the most frequently injured vessel, accounting for approximately 25-33% of all peripheral arterial injuries. The brachial artery is surrounded by important peripheral nerves -the median, ulnar and radial, and also parallels the humerus and associated veins. Due to its close proximity to these structures, associated nerve and osseous injuries are frequent with residual neuropathy from such nerve injuries, often the main sources of permanent disability.

Knowledge of overhead throwing biomechanics is crucial to understand specific injuries encountered in throwing athletes on diagnostic imaging. Most specific injuries of overhead throwing athletes occur at the shoulder and elbow. Throwing athletes are susceptible to rotator cuff tears from tensile overload and external and internal impingement. The labrum is also commonly degenerated or torn secondary to overuse syndrome, internal impingement, and microtrauma. The elbow is typically injured secondary to excessive valgus forces during throwing. The ulnar collateral ligament, ulnar nerve, and com

Apophyseal injuries, which are unique in the adolescent athlete, cause inflammation at the site of a major tendinous insertion onto a growing bony prominence. These injuries typically occur in active adolescents between the ages of eight and 15 years and usually present as periarticular pain associated with growth, skeletal immaturity, repetitive microtrauma and muscle-tendon imbalance. Common apophyseal injuries, and their sites, include Sever's disease (posterior calcaneus), Osgood-Schlatter disease (tibial tuberosity), Sindig-Larsen-Johansson syndrome (inferior patella), medial epicondyliti

Dislocations of the acromioclavicular joint are common injuries in sports and traffic accidents. The most widely used classification of these injuries is the one of Tossy, Tossy III and Tossy II injuries are indications for operative treatment. Among several different surgical approaches a very common procedure is the temporary fixation of the AC-joint by two pins, tendon band wiring and suturing of the ligaments. Recently we have been using a temporary transfixation by only one pin, tension band fixation with PDS and suturing of the ligaments. The advantages of this method are: preservation o

Family physicians play a pivotal role in providing timely care for common fingertip injuries and minimizing long-term disability after injury. Subungual hematoma is diagnosed clinically and treated with observation or nail trephination. Nail bed lacerations are treated by removal of the nail and repair of the nail bed. Distal interphalangeal joint dislocations typically occur dorsally and result from hyperextension or hyperflexion during sports or because of accidental trauma. Management primarily involves closed reduction and splinting; however, surgical intervention might be necessary for co

Fracture-dislocation of the ankle is a common lower limb injury, often associated with syndesmotic damage. While CT and MRI scans are more sensitive than plain radiographs in diagnosing syndesmotic injuries, treatment typically involves either static stabilization using metallic screws or dynamic stabilization with a suture button device. The primary advantage of dynamic stabilization is that it eliminates the need for a second surgery to remove screws. However, chronic syndesmotic injuries can lead to significant morbidity, including pain and impaired function. In patients with inflammatory c

INTRODUCTION: Acute latissimus dorsi tendon injuries are uncommon, having not previously been described in cricketers. The leg spinner's stock ball bowling technique and the fast bowler's back-of-the-hand slow ball, which is used much more widely in T20 cricket, produce a significant eccentric contraction load on the latissimus dorsi muscle.
METHODOLOGY: A retrospective review of a case series of acute latissimus dorsi tendon injuries in 3 elite cricketers (2 fast bowlers and a leg-spin bowler). We compare the outcomes using patient-rated scales and objective strength testing. Two patients un…

An avulsed elbow with severe degloving and crush injury was reconstructed in a 66-year-old man. The biceps brachii was completely transected, and the triceps brachii tendon was partly transected. In addition, the skin around the elbow had become totally necrotic. Four weeks after the injury, a free anterolateral thigh flap with a fascial flap was transplanted to reconstruct both the biceps brachii and triceps brachii tendons simultaneously. Six months after the initial injury, the range of elbow motion had recovered to almost the same level as that before injury.

BACKGROUND: There has been no research examining the magnetic resonance imaging findings, and in turn the prognosis, for a series of acute quadriceps muscle strains.
HYPOTHESIS: The magnetic resonance imaging findings of acute quadriceps muscle strain injuries are helpful in predicting their prognosis.
STUDY DESIGN: Causal-comparative study.
METHOD: Forty professional players of Australian Rules football were followed over 3 years. Magnetic resonance imaging examinations were performed within 24 to 72 hours of muscle-strain injury. Imaging features of muscle strain injury included the anato

A prospective study of postoperative mobilization of flexor tendon repairs in zone 5 was conducted over a 2-year period between 1994 and 1996 using a controlled active motion (active extension-active flexion) regimen of mobilization. Fifty-two patients, who had a total of 151 flexor digitorum superficialis (FDS) and 103 flexor digitorum profundus (FDP) divisions, were available for review at a mean follow-up of 10 months. Of the 161 fingers with division of one or both flexor tendons, 66% exhibited independent FDS function and 90% achieved good or excellent results of digital range of motion.

Historically, the posterolateral corner (PLC) of the knee has been labelled as the "dark side" of the knee. The PLC is not a single structure but a confluence of multiple structures, including the lateral collateral ligament, popliteus muscle-tendon unit, and popliteofibular ligament. Understanding the individual components and their function is important to successfully identify these injuries. PLC injuries are commonly associated with cruciate injuries, and its early recognition is important to achieve successful outcomes. Injury to the PLC should be treated on a case-by-case basis, and an a

A chronic Achilles tendon rupture is a tendon rupture occurring more than 4-6 weeks after a traumatic injury. Haglund's deformity, caused by bony abnormalities in the ankle (mostly due to osteophyte or bone spur), can cause chronic inflammation and degeneration of the Achilles tendon, eventually leading to rupture. This presents a challenge for clinicians who provide tendon repair procedures. We present a 69-year-old woman who had difficulty moving her left leg and had a deformity on the left leg compared to her right leg after falling nine months before but with pain starting three months bef

Flexor hallucis longus transfer is routinely used as a technique to salvage a chronic or neglected Achilles tendon rupture. The single-incision (or short harvest) technique provides adequate graft suitable for fixation with interference screws. We have used a bore corer instead of a drill to establish the tendon tunnel and at the same time use the harvested autograft for interference fit and avoid the use of screws, providing what we consider to be a more biologic form of fixation.

CASE: A 22-year-old man soldier sustained a Gustilo-Anderson type IIIB hindfoot injury from a landmine blast. Initial limb salvage included polymethylmethacrylate spacer insertion, reverse sural flap, and external fixation. Definitive reconstruction used a composite calcaneus-Achilles tendon allograft and an anterolateral thigh flap with tensor fascia lata. By the 3-year follow-up, he regained full ambulatory function and independence in all daily living activities.
CONCLUSION: Structural allograft reconstruction of the calcaneus and Achilles tendon can be a viable limb salvage option for bla

Ankle sprains are a common, costly, and potentially disabling problem. The proper history and physical examination will determine the need for radiological evaluation and treatment. Complications of ankle trauma like osteochondral fractures, peroneal tendon injuries, fracture of the os trigonum, synovial impingement, tarsal tunnel syndrome, Achilles tendon inflammation or rupture, and nerve injury are reviewed. The treatment of ankle sprains is based on the severity of the injury. Treatment begins with rest, ice, compression, and elevation. Casting and orthotics may be needed to facilitate hea…
