Techniques and outcomes of hip abductor reconstruction following tumor resection in adults.

The function of the abductor mechanism (AM) of the hip can be disturbed, or even compromised, following tumor resection in the hip area. The … ruction can be done or reconstruction in combination with a tendon allograft or an allograft and/or tendon transfer from the surrounding area. This sacrifice, whether followed by reconstruction or not, in most cases leads to a good functional outcome, except when a complete musculotendinous unit or the superior gluteal nerve is sacrificed. Isolated resection of t …

Quantitative analysis of facial symmetry and animation following intraoral orthodromic temporalis transfer in facial paralysis.

This study aimed to quantitatively analyze the degree and vector of commissure excursion following intraoral orthodromic temporalis transfer. Patients with unilateral facial paralysis who underwent intraoral temporalis transfer were included. Intra-oral coronoidectomy was followed by submucosa tunneling through two vertical intraoral incisions to fixate the temporalis tendon to the perioral location. Oral commissure excursion, upper lip and commissure height differences, and smile angle were measured. Postoperatively, the symmetry of commissure excursion improved in repose (affected side: 114.

Anterior Interosseus to Ulnar Motor Nerve Transfers: A Canadian Perspective.

BACKGROUND: The anterior interosseus nerve (AIN) to ulnar motor nerve transfer has been popularized as an adjunct to surgical decompression … eve the transfer would improve outcomes, 3% cited lack of training, and 3% would preferentially use tendon transfers. Surgeons with hand fellowship training and those less than 30 years in practice were more likely to use nerve transfers in the treatment of CuTS (P < .05). CONCLUSIONS: Most CSPS members would use an AIN-SETS transfer in the treatment of both a h …

A massive rotator cuff tear in association with a posterior dislocation of the long head biceps tendon: a review of the literature on posterior long head biceps tendon dislocation.

We report a complex case detailing a patient with an anterior shoulder dislocation, with an associated, severe, massive rotator cuff tear (MRCT), further complicated by an unusual occurrence of a posterior long head biceps tendon (LHBT) dislocation. A 38-year-old female presented with significant pain and limited active and passive range of motion of the left shoulder following a traumatic anterior shoulder dislocation. MRI revealed full-thickness retracted tears of the entire cuff as well as a posterolateral LHBT dislocation. The patient underwent an initial open repair of the shoulder and lo

Motor nerve transfers to restore extrinsic median nerve function: case report.

Active pronation is important for many activities of daily living. Loss of median nerve function including pronation is a rare sequela of humerus fracture. Tendon transfers to restore pronation are reserved for the obstetrical brachial plexus palsy patient. Transfer of expendable motor nerves is a treatment modality that can be used to restore active pronation. Nerve transfers are advantageous in that they do not require prolonged immobilization postoperatively, avoid operating within the zone of injury, reinnervate muscles in their native location prior to degeneration of the motor end plates…

The use of free tissue transfer in burn reconstruction.

Patients that sustain extensive burns commonly present the reconstructive surgeon with problems caused by exposed tendons, scar contractures, and loss of digits. Frequently there is inadequate local tissue available to solve these reconstructive problems. To solve these complex postburn defects, we have used free tissue transfer in addition to more traditional therapy. From 1987 thru 1994, we have used free tissue transfer as an adjunct to our reconstructive armamentarium in 35 patients who have undergone a total of 45 free tissue transfers for the correction of postburn deformities. There hav

[Reconstruction of the ruptured extensor pollicis longus tendon using a tendon interposition graft].

To restore function of the ruptured extensor pollicis longus (EPL) tendon, two operative principles have been described: reconstruction of the original tendon using a tendon interposition graft or a tendon transposition operation, most commonly transposition of the extensor indicis. From 1992 to 1998, we reconstructed the extensor pollicis longus tendon using an interposition graft in 58 patients. 48 patients were available for follow-up after a mean of 31 months. The mean age of the 25 women and 23 men was 49 years. The time between tendon rupture and reconstruction ranged from one to 40 week

Excision without Reconstruction of a Traumatically Ruptured Extensor Indicis Proprius Tendon in a Diabetic Patient: A Case Report.

INTRODUCTION: Isolated traumatic rupture of the extensor indicis proprius (EIP) is uncommon and may be masked by preserved index finger extension through the intact extensor digitorum communis (EDC). While primary repair or tendon transfer is standard when reconstruction is feasible, the functional redundancy of the dual-extensor index finger raises the question of whether excision alone can suffice in selected cases. We report what we believe to be the first documented case of intentional EIP stump excision without reconstruction as definitive surgical treatment following delayed-presentation…

The clinical outcomes of total vs. half patellar tendon transposition combined with robot-assisted medial patellofemoral ligament reconstruction and extensive lateral release for the treatment of habitual patellar dislocation in adolescents.

PURPOSE: This study reviews a series of adolescent patients with habitual patellar dislocation (HPD) who underwent combined surgical procedu … -up. CONCLUSIONS: The combined procedures, which include extensive lateral release, total patellar tendon transfer, and robot-assisted MPFL reconstruction, can achieve good patellar tracking and stability in the treatment of HPD in adolescents, and may provide better clinical outcomes than the half patellar tendon transposition group.

Thenar insertion of abductor pollicis longus accessory tendons and thumb carpometacarpal osteoarthritis.

Although the etiology of osteoarthritis of the thumb carpometacarpal (CMC) joint remains unclear, some theories have focused on variations in the local anatomy of the abductor pollicis longus tendon insertion. This cadaver study of 68 specimens analyzed the relationship between a thenar insertion of an accessory abductor pollicis longus tendon and the presence and severity of thumb CMC osteoarthritis. The joint cartilage surfaces were visually graded for degenerative changes. Thirty-five of 68 specimens (51%) had a thenar insertion, most frequently inserting on either the abductor pollicis bre

Reconstruction of extensor mechanism after trauma and infection by transposition of the Achilles tendon: report of technique and four cases.

High-energy injuries to the knee often involve extensive soft tissue loss as well as fractures. In certain situations, the patellar tendon may be lost due to initial injury or post-injury complications such as infection. Use of the gastrocnemius flap is a popular method of covering the defect as well as controlling infection and promoting healing by increasing local vascularity. We present a method of reconstructing the patellar tendon by extending the use of the gastrocnemius flaps using its Achilles tendon and calcaneal insertion. The distal insertion of the Achilles tendon is fixed to the t

Heel cord advancement in children with spastic equinus deformity.

Heel cord advancement has been advocated for treatment of spastic equinus deformity. Transferring the gastrosoleus anteriorly weakens it by changing the lever arm rather than the resting length. A retrospective review of 90 children with 122 limbs undergoing heel cord advancement revealed 11% excellent, 53% good, and 35% poor results (average follow-up 9.7 years). The results were statistically better in diplegics and community walkers. The better results in previous studies may be due to procedure modifications and shorter follow-up. Since we obtain comparable results with simpler heel cord l

[Functional electromyography analysis of radial replacement operation].

The aim of this electromyographic study was to evaluate the functional result of tendon transfers for radial palsy. Eighteen patients were compared to 16 healthy volunteers. The intramuscular emg of four muscles was recorded in parallel during extension-flexion movements of the wrist and fingers. The data were stored on FM tape and evaluated off-line. Contrary to expectations, transferred flexor muscles revealed only part of their overall emg activity during extension. Therefore, an "extensor quota" (EQ) was calculated for each muscle. It reflects its emg activity during extension as part of i…

Plantar Flexion-Induced Entrapment of the Dorsalis Pedis Artery in a Teenaged Cross-Country Runner.

BACKGROUND: Symptomatic peripheral artery disease of the lower extremity rarely affects young adults and, when present, typically has a nonatherosclerotic etiology. Anatomical variants have manifested as symptomatic foot ischemia in four cases in the literature. We describe the case of a 17-year-old girl presenting with foot pain upon plantar flexion due to dynamic dorsalis pedis (DP) artery entrapment by fibrous bands and the extensor hallucis brevis (EHB) tendon. METHODS: The patient was a 17-year-old girl who presented with right foot pain upon plantar flexion, which resolved upon returnin

Evaluation of early dynamic splinting versus static splinting for patients with transposition of the extensor carpi radialis longus to the extensor pollicis longus.

We present a group of 20 patients who underwent transposition of the extensor carpi radialis longus to the extensor pollicis longus between April 2003 and September 2007. The group was divided into ten patients with an early dynamic splint and ten patients with a static plaster cast following the standard rehabilitation protocols. We compared these two rehabilitation protocols. The post-rehabilitation total range of motion was 87.5 degrees in the dynamic group and 58.75 degrees in the patients treated by the static protocol. The total time of rehabilitation was 10.2 weeks in the dynamic and 14
