Pott's Disease in a 2-Year-Old Child Treated by Decompression and Anterior-Posterior Instrumented Fusion.

Introduction. Paraplegia and kyphotic deformity are two major disease-related problems of spinal tuberculosis, especially in the early age disease. In this study a 2-year-old boy who underwent surgical decompression, correction, and 360° instrumented fusion via simultaneous anterior-posterior technique for Pott's disease was reported. Case Report. A 2-year-and-9-month-old boy presented with severe back pain and paraparesis of one-month duration. Thoracic magnetic resonance imaging demonstrated destruction with a large paraspinal abscess involving T5-T6-T7 levels, compressing the spinal cord. T

The changing face of tuberculosis: trends in tuberculosis-associated skeletal changes.

Tuberculosis remains a major health problem in many areas of the world. Previous research suggested that the frequency of bone lesions has decreased in the modern (but pre-antibiotic) period, and that the predominantly spinal involvement have changed to affect other parts of the skeleton, in particular ribs. The purpose of this study was to investigate whether bone lesions associated with TB became more or less common in the post-antibiotic period, and if the pattern of skeletal involvement has changed. The skeletons of 147 individuals from South Africa who died from TB were assessed. These we

Anterior cervical debridement and strut-grafting for osteomyelitis of the cervical spine.

A retrospective review of the surgical experience in treating 18 patients with osteomyelitis of the cervical spine is reported. The patients ranged in age from 20 to 60 years and all complained of neck pain upon admission. Ten patients had a prior history of intravenous drug abuse, three had previously suffered penetrating injuries of the neck, and one had an extraspinal site of osteomyelitis. Bacteria were isolated in 13 cases and tuberculosis in three. Neurological abnormalities were present in over one-half of the patients, consisting of myelopathy (nine cases) or radiculopathy (four cases)

Concurrent Spinal Dural Arteriovenous Fistula and Varicella-Zoster Virus Meningoencephalitis Unmasked by Corticosteroid-Associated Deterioration: A Case Report on the Diagnostic Value of Serial mNGS.

BACKGROUND: Concurrent spinal dural arteriovenous fistula (SDAVF) and varicella-zoster virus (VZV) meningoencephalitis are exceptionally rare, and overlapping features can delay diagnosis. This case adds to the literature by illustrating how corticosteroid exposure before exclusion of vascular and infectious mimics may be followed by neurological deterioration, and by emphasizing the diagnostic value of serial metagenomic nextgeneration sequencing (mNGS). CASE PRESENTATION: A 48-year-old man developed insidious bilateral lower-limb weakness that progressed to numbness, sphincter dysfunction,

Posterior limited unilateral fenestration approach for treating patients with single-segment thoracic and lumbar tuberculosis.

BACKGROUND: Posterior limited unilateral fenestration approach is extensively used in the treatment of many spinal diseases. But whether it is suitable for spinal tuberculosis (TB) is rarely reported. Hence, the current study evaluated the feasibility and efficacy of the posterior limited unilateral fenestration (PLUF) debridement, bone grafting fusion, and instrumentation to treat single-segment thoracic and lumbar TB. METHODS: Eighty-three patients (45 male and 38 female) aged 17-79 years old with the single-segment thoracic and lumbar TB who underwent PLUF debridement, bone grafting fusion

Use of structural allografts in spinal osteomyelitis: a review of 47 cases.

OBJECT: The use of structural allografts in spinal osteomyelitis remains controversial because of the perceived risk of persistent infection related to a devitalized graft and spinal hardware. The authors have identified 47 patients over the last 3.5 years who underwent a surgical decompression and stabilization procedure in which fresh-frozen allografts were used after aggressive removal of infected and devitalized tissue. The patients subsequently underwent 6 weeks of postoperative antibiotic therapy (12 months for those with tuberculosis [TB]). METHODS: Follow-up data included results of s

[Substantiation of the use of terrilytin in the treatment of spinal cord disorders in tuberculous spondylitis].

Experiments on mice and studies with organotype cultures of the spinal ganglion showed that terrilytin, a proteolytic enzyme had neurite stimulating activity and promoted regeneration of the spinal marrow tissue after affection (squeeze). On the basis of this property and a previously observed favourable effect of the enzyme on microcirculation, the capacity for more rapid rarefaction and elimination of caseous masses and prevention of excessive fibrosis, it was recommended for use in treatment of cerebrospinal disorders in tuberculous spondylitis. A procedure for the treatment was developed.

Imaging of the spine and nerve roots.

Advances in imaging have greatly improved the ability to display spine anatomy and pathology. This article describes the imaging findings in degenerative, traumatic, infectious, and neoplastic disease. Anatomic information, however, must be tempered by an understanding of asymptomatic disease and interpreted in the context of the clinical syndrome.

Tuberculous lesions of the lumbosacral region. A 15-year follow-up of patients treated by ambulant chemotherapy.

STUDY DESIGN: A 15-year clinical follow-up of tuberculous lesions of the lumbosacral region. OBJECTIVES: To verify the hypothesis that the lumbar lordosis and the specific biomechanics of the lumbosacral region influence and alter the healing pattern and progress of the disease when compared with their effects in other regions of the spine. SUMMARY OF BACKGROUND DATA: An estimated 2 million or more patients have active spinal tuberculosis, and the global incidence of the disease is increasing. The involvement of the lower lumbar region and the lumbosacral junction is relatively rare, with fe

A rare presentation of Pott's disease with bilateral psoas abscess during pregnancy in a resource-limited setting: Case report from a resource-limited setting.

INTRODUCTION AND IMPORTANCE: TB remains a global health threat, especially in low-income countries. Spinal TB diagnosis in pregnancy is challenging, leading to potentially severe complications like psoas TB abscess. CASE PRESENTATION: A 30-year-old multiparous woman from Somalia region, Ethiopia in her second trimester of pregnancy who was diagnosed with isolated Pott's disease complicated by bilateral large Psoas abscesses. The diagnosis was made using lumbosacral X-ray, abdomino-pelvic ultrasound, and ultrasound-guided aspiration confirming tuberculosis. After she presented with a four-year

Calvarial tuberculosis.

Tuberculosis of the skull is very rare. We report on two children: one presented with orbital and scalp swellings and was found to have lytic lesions on skull X-ray, while the other presented with chronic discharging scalp ulcers typical of tuberculosis of the scalp and also showed lytic skull lesions. Both patients had evidence of vertebral and lung tuberculosis.

Comparison of three surgical approaches for thoracolumbar junction (T12-L1) tuberculosis: a multicentre, retrospective study.

BACKGROUND: The surgical approaches to thoracolumbar junction (T12-L1) tuberculosis were controversial. We aimed to compare the safety and efficacy of three different procedures through a multicentre retrospective study. METHODS: The medical records of thoracolumbar junction tuberculosis patients (n = 177) from January 2005 to January 2015 were collected and reviewed. Forty-five patients underwent anterior debridement and instrumented fusion (Group A), 52 underwent anterior combined with posterior debridement and instrumented fusion (Group B) and 80 underwent posterior-only debridement and in

Tuberculosis meningo-encephalitis with positive csf for KB and slowly favourable evolution. Case report.

BACKGROUND: Meningoencephalitis produce by Koch bacillus is a disease including various symptoms, with often atypical onset and evolution, and with an etiological diagnosis being rarely established. It therefore remains a disease with severe prognosis, despite adequate treatment. CASE REPORT: a 47 years old male from Neamt County, with chronic otitis in his recent medical history, comes to the emergency room for fever, severe headache, confusion, equilibrium disorders, influenced health status, and meningeal irritation signs. Given his otitis and antibiotic therapy received in recent medical

Isolated tuberculosis of the lumbar facet joint: A case report.

RATIONALE: Spinal tuberculosis (TB) is the most common in bone and joint TB, of which vertebral TB is more common, while accessory TB is rare. The incidence of isolated adnexal lesions in spinal TB is 2% to 3%. It is difficult to distinguish the imaging changes of spinal adnexal TB from other types of spinal infections and spinal tumors, and the pathological diagnosis of spinal TB is often atypical. Here, we report a case of isolated lumbar facet joint TB. PATIENT CONCERNS: A 64-year-old female patient had an 8-month history of low back pain, decreased pinprick sensation in the left anterior

Continuous spinal anesthesia for TURBT in a morbidly obese patient with EF of 20%: a case report.

BACKGROUND: Patients with severely reduced left ventricular function pose major anesthetic challenges. General anesthesia may cause cardiac depression and pulmonary decompensation, while single-shot spinal anesthesia can trigger sudden hypotension. Continuous spinal anesthesia (CSA), however, allows gradual dosing and improved hemodynamic stability in such high-risk patients. CASE PRESENTATION: We report a 133-kg man with atrial fibrillation, left bundle branch block, global hypokinesia, ejection fraction (EF) of 20%, and prior pulmonary tuberculosis with lung congestion, who underwent transu
