A managed care workstation for support of ambulatory care in Veterans Health Administration medical centers.

This paper describes the development of a Managed Care Workstation for implementation in a Department of Veterans Affairs hospital. Each VA hospital information system contains a wealth of information in a comprehensive and well integrated M database, however, a clinician's access to the information is hampered by a lack of usable database tools. The Managed Care Workstation is designed to enhance access to VA databases. The workstation uses M (Mumps) and SQL to present the VA's M hierarchical database as relational tables on the workstation. This work reveals the benefits of using a SQL-M wor

Application of Computer Techniques in Correcting Mild Zygomatic Assymetry With Unilateral Reduction Malarplasty.

Zygomatic assymetry is common in the population, which often requires surgical correction for aesthetic concerns. Previously, surgeons performed the surgery often based on their personal experience and visual evaluation. The purpose of this study was to apply computer techniques in patients with mild zygomatic asymmetry treated with unilateral reduction malarplasty to improve surgical accuracy and reduce preoperative risks. The authors used computer techniques to plan osteotomies, to produce surgical template, and to evaluate the surgical outcome. Postoperative follow-up demonstrated that zygo

On the way home: a BCI-FES hand therapy self-managed by sub-acute SCI participants and their caregivers: a usability study.

BACKGROUND: Regaining hand function is the top priority for people with tetraplegia, however access to specialised therapy outwith clinics is limited. Here we present a system for hand therapy based on brain-computer interface (BCI) which uses a consumer grade electroencephalography (EEG) device combined with functional electrical stimulation (FES), and evaluate its usability among occupational therapists (OTs) and people with spinal cord injury (SCI) and their family members. METHODS: Users: Eight people with sub-acute SCI (6 M, 2F, age 55.4 ± 15.6) and their caregivers (3 M, 5F, age 45.3 ±

Classification of hand and wrist movements via surface electromyogram using the random convolutional kernels transform.

Prosthetic devices are vital for enhancing personal autonomy and the quality of life for amputees. However, the rejection rate for electric upper-limb prostheses remains high at around 30%, often due to issues like functionality, control, reliability, and cost. Thus, developing reliable, robust, and cost-effective human-machine interfaces is crucial for user acceptance. Machine learning algorithms using Surface Electromyography (sEMG) signal classification hold promise for natural prosthetic control. This study aims to enhance hand and wrist movement classification using sEMG signals, treated

Assessment of the Accuracy and Errors of Head-Up Display by an Optical Neuronavigation System in Brain Tumor Surgery.

BACKGROUND: A head-up display (HUD) in which navigational information is projected into the microscope view may enable surgeons to perform operations more efficiently. Projecting depictions of both tumor and important intracranial structures on the HUD may facilitate safe surgery. OBJECTIVE: To investigate accuracy and errors regarding important intracranial structures, errors due to brain shifts, and preservation rates for important intracranial structures. METHODS: A total of 184 surgeries in 172 patients were performed using this operation system. Postoperatively, we determined accuracy a

Intraoperative navigation assisted reconstruction of a maxillo-facial gunshot wound.

INTRODUCTION: This case report describes a suicidal gunshot wound to the head using a handgun (9 x 19 mm). During mandibular penetration, the solid hollow point projectile (Aktion4) was deformed but did not fragment. The mandible, mid-face, orbit, and nasal complex were severely damaged. Here, the authors present, for the first time, a new approach for bilateral mid-facial fractures. MATERIALS AND METHODS: For virtual reconstruction, a naso-orbito-ethmoidal complex of an unaffected skull (same age, sex, and ethnic group) was merged into the computed tomography (CT) data set. Based on this dat

Virtual and augmented reality in a simulated naval engagement: Preliminary comparisons of simulator sickness and human performance.

The aim of this study was to compare simulator sickness symptoms while participants wore either a virtual reality (VR) or augmented reality (AR) headset. A secondary aim involved comparing how physical motion affects symptoms. During a simulation, participants wore VR and AR headsets while standing on a motion platform and firing at hostile ships under three motion conditions: No Physical Motion; Synchronous Motion, in which the physical and displayed motion were coupled; and Asynchronous Motion, in which the physical motion did not match the display. Symptoms increased over time but were not

The Hounsfield value for cortical bone geometry in the proximal humerus--an in vitro study.

INTRODUCTION: Fractures of the proximal humerus represent a major osteoporotic burden. Recent developments in CT imaging have emphasized the importance of cortical bone thickness distribution in the prevention and management of fragility fractures. We aimed to experimentally define the CT density of cortical bone in the proximal humerus for building cortical geometry maps. METHODS: With ethical approval, we used ten fresh-frozen human proximal humeri. These were stripped of all soft tissue and high-resolution CT images were then taken. The humeral heads were then subsequently resected to allo

MR virtual endoscopy for biliary tract and pancreatic duct.

Developments in magnetic resonance (MR) equipment and techniques have been remarkable. Especially, respiratory-triggered three-dimensional MR cholangiopancreatography (3D-MRCP) has been developed to provide images with high spatial resolution of the biliary tract and pancreatic duct. These 3D data can be employed in MR virtual endoscopy (MRVE) with volume rendering to visualize the lumina of the gallbladder, bile duct, and pancreatic duct. To observe the changes in the lumina with threshold settings on a workstation, we made an original phantom with tubes 2, 3, and 6 mm in internal diameter. W

High sensitivity to multisensory conflicts in agoraphobia exhibited by virtual reality.

The primary aim of this study was to evaluate the effect of auditory feedback in a VR system planned for clinical use and to address the different factors that should be taken into account in building a bimodal virtual environment (VE). We conducted an experiment in which we assessed spatial performances in agoraphobic patients and normal subjects comparing two kinds of VEs, visual alone (Vis) and auditory-visual (AVis), during separate sessions. Subjects were equipped with a head-mounted display coupled with an electromagnetic sensor system and immersed in a virtual town. Their task was to lo

Modeling of time-dependent force response of fingertip to dynamic loading.

An extended exposure to repeated loading on fingertip has been associated to many vascular, sensorineural, and musculoskeletal disorders in the fingers, such as carpal tunnel syndrome, hand-arm vibration syndrome, and flexor tenosynovitis. A better understanding of the pathomechanics of these sensorineural and vascular diseases in fingers requires a formulation of a biomechanical model of the fingertips and analyses to predict the mechanical responses of the soft tissues to dynamic loading. In the present study, a model based on finite element techniques has been developed to simulate the mech
