The Ototoxic Potential of Cobalt From Metal-on-Metal Hip Implants: Objective Auditory and Vestibular Outcome.

OBJECTIVES: During the past decade, the initial popularity of metal-on-metal (MoM) hip implants has shown a progressive decline due to increasingly reported implant failure and revision surgeries. Local as well as systemic toxic side effects have been associated with excessive metal ion release from implants, in which cobalt (Co) plays an important role. The rare condition of systemic cobaltism seems to manifest as a clinical syndrome with cardiac, endocrine, and neurological symptoms, including hearing loss, tinnitus, and imbalance. In most cases described in the literature, revision surgery

Effects of fast head turns on head, trunk and pelvis motions during standing and walking in patients with unilateral vestibular deficit.

Patients with unilateral vestibular deficit (UVD) report difficulties with maintaining balance while executing fast head turns. Our aim was to determine whether head, trunk, and pelvis angular displacements were symmetrical in patients with UVD as they executed voluntary yaw rotation of the head towards or away from the side of the vestibular lesion, during standing and walking. Eight patients who underwent surgical resection of an acoustic neuroma stood with feet together or walked at comfortable pace across a 10-meter walkway. They turned the head as quickly and as fast as possible in the di

Otolith-ocular responses in Meniere's patients before and after endolymphatic shunt operation.

Caloric testing in prone/supine position and constant velocity off-vertical axis rotation (OVAR) in yaw axis (rotate-then-tilt paradigm) can evaluate labyrinth function and vestibular-ocular reflex (VOR) behaviour before and after endolymphatic shunt surgery (ESS). Preoperative and postoperative otolith dysfunction can be documented by constant velocity OVAR, before the VOR is modulated by the vestibular compensation. Vestibulo-ocular responses in prone/supine position and linear VOR (lVOR) OVAR responses were observed in 10 patients before and after ESS. Ipsilateral caloric reaction in prone/

Evaluation of the brain-stem function by the auditory brain-stem response and the caloric vestibular reaction in comatose patient.

The auditory brain-stem response (ABR) and the caloric vestibular reaction (CVR) were investigated in 100 patients in deep coma to evaluate the brain-stem function of these patients precisely and to predict their prognosis accurately. In the first ABR examination, 54 patients showed normal and 46 abnormal ABRs. Among the latter, five showed partial wave disappearance and 21 exhibited no ABRs. Twenty-six of the 30 patients who recovered (87%) showed normal ABRs and 21 of the 57 who died (37%) also exhibited normal latency at that time. On the other hand, all the patients who showed wave disappe

[The diagnostic value of vertical nystagmus in the simultaneous bilateral calibrated caloric vestibular test (author's transl)].

Vertical nystagmus provoked by simultaneous bilateral caloric stimulation has been known since 1907 (Bàràny). However, if a controlled and calibrated injection os water at 44 degrees C or 30 degrees C for 30 seconds at a flow rate of 250 ml/minute is given in normal subjects, this stimulus is insufficient to provoke a response, whilst in subjects with a central vestibular disorder there appear upward vertical movements with hot stimulation and downward movements with cold stimulation. The degree of this response is proportional to the degree of horizontal nystagmic responses to classical unila

[Management of vestibular schwannomas].

Vestibular schwannomas are benign neoplasms originating from the Schwann cells of the vestibular vestibular nerve of the vestibulocochlear nerve, and rarely from the pars cochlearis. These are tumors that are in contact with the nerve but do not bind the fibers. Benign neoplasms of the Schwann cells of the auditory and equilibrium nerves can also occur primarily in the inner ear and are referred to as intralabyrinthine schwannomas (ILS). Vestibular schwannomas represent 6-7 % of all intracranial and 90 % of cerebellopontine angle tumors. Bilateral occurrence occurs in < 5 % of cases, and then

Skew deviation following vestibular nerve surgery.

Eighteen consecutive patients undergoing vestibular nerve surgery underwent pre- and postoperative examination of ocular motility. Five patients developed a skew deviation following surgery, with the lower eye on the operated side and an incomitant pattern of deviation in all cases. Three patients experienced diplopia lasting from 1 day to 6 months. The magnitude of skew deviation was increased by head tilt away from the operated side in only 1 case. The development of skew deviation was not determined by preoperative ocular alignment or binocular function, or any particular type or pattern of

Clinical examination of labyrinthine-defective patients out of the vertigo attack: sensitivity and specificity of three low-cost methods.

Many reports have appeared in the medical literature concerning the clinical examination at the bedside of patients with vertigo and, even if few controversial opinions exist, the observation of one or more kinds of nystagmus is generally regarded as suggesting an organic aetiology. So far, the presence of nystagmus has been generally considered to be crucially important for clinicians who are daily asked to differentiate between an "organic" cause of vertigo (for example, a labyrinthine dysfunction) and a "non-organic" cause of vertigo, such as a panic disorder. Albeit, it should not be forgo

Superior semicircular canal dehiscence and subsequent closure induces reversible impaired decision-making.

BACKGROUND: Vestibular loss and dysfunction has been associated with cognitive deficits, decreased spatial navigation, spatial memory, visuospatial ability, attention, executive function, and processing speed among others. Superior semicircular canal dehiscence (SSCD) is a vestibular-cochlear disorder in humans in which a pathological third mobile window of the otic capsule creates changes to the flow of sound pressure energy through the perilymph/endolymph. The primary symptoms include sound-induced dizziness/vertigo, inner ear conductive hearing loss, autophony, headaches, and visual problem

Rebalancing the Vestibular System by Unidirectional Rotations in Patients With Chronic Vestibular Dysfunction.

Introduction: Vestibular dysfunction is a common disorder that results in debilitating symptoms. Even after full compensation, the vestibulo-ocular reflex (VOR) could be further improved by using rehabilitation exercises and visual-vestibular adaptation. We hypothesized that in patients with asymmetric vestibular function, the system could be rebalanced by unidirectional rotations toward the weaker side (i.e., a pure vestibular stimulation). Methods: Sixteen subjects (5 female and 11 male, 43.2 ± 17.0 years old) with chronic vestibular dysfunction that was non-responsive to other types of medi

Proposal of a standardized testing protocol for BPPV: Using 3D simulations for insights into movement of otoliths during positional tests.

BACKGROUND: Benign Paroxysmal Positional Vertigo (BPPV) is a common vestibular disorder caused by displaced otolith debris in the inner ear. This study explored the order effect related to the sequence of performing different diagnostic positional maneuvers. METHODS: 3D simulations of the Supine Roll Test (SRT), Dix-Hallpike maneuver (DHM), and Deep head hanging tests, performed in the standardized prescribed way, were used to study various types of horizontal, posterior, and anterior canal BPPV. The simulations allowed visualization of the movement of the otolith debris and the resulting nys
