Adverse Childhood Experiences and Offending as a Function of Acquired Brain Injury Among Men in a High Secure Forensic Psychiatric Hospital.

BACKGROUND: Acquired brain injury (ABI) is a serious problem that disproportionately affects individuals in correctional services, but relatively little is known about ABI risks and correlates in forensic psychiatric services. METHODS: We conducted a retrospective chart review of all admissions to a high secure forensic hospital in Ontario, Canada from January 2009 to December 2012 (n = 637) and collected data on ABI, psychiatric diagnoses, developmental disadvantage, criminal offending, and in-hospital aggression. A k-means cluster analysis was employed to assess risk factors by which men wi

Deconstructing major depressive episodes across unipolar and bipolar depression by severity and duration: a cross-diagnostic cluster analysis on a large, international, observational study.

A cross-diagnostic, post-hoc analysis of the BRIDGE-II-MIX study was performed to investigate how unipolar and bipolar patients suffering from an acute major depressive episode (MDE) cluster according to severity and duration. Duration of index episode, Clinical Global Impression-Bipolar Version-Depression (CGI-BP-D) and Global Assessment of Functioning (GAF) were used as clustering variables. MANOVA and post-hoc ANOVAs examined between-group differences in clustering variables. A stepwise backward regression model explored the relationship with the 56 clinical-demographic variables available.

Resilience and recovery style: a retrospective study on associations among personal resources, symptoms, neurocognition, quality of life and psychosocial functioning in psychotic patients.

Background: Personal resources have been identified as important factors in predicting patient healing or symptoms control in schizophrenia. This observational retrospective study aims to explore the influence of resilience and recovery style on the modalities of clinical presentation of the disease, as well as individual functioning and quality of life. Methods: Participants were patients affected by schizophrenia spectrum disorders assessed at different mental health facilities. The rating scales considered are the following: Resilience Scale 10-items (RS); Recovery Style Questionnaire (RSQ)

The serotonin hallucinogen 5-MeO-DMT alters cortico-thalamic activity in freely moving mice: Regionally-selective involvement of 5-HT1A and 5-HT2A receptors.

5-MeO-DMT is a natural hallucinogen acting as serotonin 5-HT1A/5-HT2A receptor agonist. Its ability to evoke hallucinations could be used to study the neurobiology of psychotic symptoms and to identify new treatment targets. Moreover, recent studies revealed the therapeutic potential of serotonin hallucinogens in treating mood and anxiety disorders. Our previous results in anesthetized animals show that 5-MeO-DMT alters cortical activity via 5-HT1A and 5-HT2A receptors. Here, we examined 5-MeO-DMT effects on oscillatory activity in prefrontal (PFC) and visual (V1) cortices, and in mediodorsal

Negative symptoms of schizophrenia: clinical characteristics, pathophysiological substrates, experimental models and prospects for improved treatment.

Schizophrenia is a complex and multifactorial disorder generally diagnosed in young adults at the time of the first psychotic episode of delusions and hallucinations. These positive symptoms can be controlled in most patients by currently-available antipsychotics. Conversely, they are poorly effective against concomitant neurocognitive dysfunction, deficits in social cognition and negative symptoms (NS), which strongly contribute to poor functional outcome. The precise notion of NS has evolved over the past century, with recent studies - underpinned by novel rating methods - suggesting two maj

In Vivo Availability of Cannabinoid 1 Receptor Levels in Patients With First-Episode Psychosis.

IMPORTANCE: Experimental and epidemiological studies implicate the cannabinoid 1 receptor (CB1R) in the pathophysiology of psychosis. However, whether CB1R levels are altered in the early stages of psychosis and whether they are linked to cognitive function or symptom severity remain unknown. OBJECTIVE: To investigate CB1R availability in first-episode psychosis (FEP) without the confounds of illness chronicity or the use of illicit substances or antipsychotics. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional, case-control study of 2 independent samples included participants receivin

Multimodal microglial and kynurenine pathway alterations across the affective-psychosis spectrum: a systematic review of patterns, heterogeneity, and dimensional implications.

Immune dysregulation is implicated in patient subgroups in major depressive disorder (MDD), bipolar disorder (BD) and schizophrenia (SCZ), but the role of microglia across affective and psychotic illnesses remains unclear. We propose an integrative framework linking systemic immune drivers to microglia-related circuit engagement, cellular phenotype, and kynurenine pathway (KP) branch balance. We systematically reviewed human TSPO-PET, cerebrospinal fluid (CSF) KP metabolite, and postmortem microglial and KP studies. Quantitative synthesis focused on MDD versus SCZ. BD was integrated descriptiv

A psychometric evaluation of the German Revised-Green et al. Paranoid Thoughts Scale (R-GPTS) in clinical and non-clinical groups.

BACKGROUND: Paranoia, characterised by inaccurate fears that others intend to cause harm, can significantly affect social functioning. Research has demonstrated that paranoia exists on a spectrum of severity, with milder forms prevalent in the general population. The Revised-Green et al. Paranoid Thoughts Scale (R-GPTS) is the most commonly used measure of paranoia comprising a scale to assess ideas of reference and a scale to assess ideas of persecution. The aim of the study was to validate a German version of the R-GPTS and assess its psychometric properties in non-clinical and clinical grou

[Niemann-Pick type C disease and psychosis: Two siblings].

INTRODUCTION: Niemann-Pick type C disease (NPC) is a rare, neurovisceral, autosomal recessive disease, with an extremely heterogeneous clinical presentation. The adult form of the disease is usually expressed as a neurological form. Non-specific psychiatric symptoms are often associated with NPC. For some cases, it can also be expressed as an isolated psychiatric disorder form. Since 2009, the launching of a medicine called miglustat has helped to improve the disease evolution. CASE HISTORIES: We report two siblings followed-up in the same department of psychiatry and with an atypical psychot

Antidepressant continuation in pregnancy and postpartum depression risk across racial and ethnic groups.

BACKGROUND: Postpartum depression affects 1 in 7 women in the United States and antidepressant medications during pregnancy are recommended by obstetric and psychiatric professional societies for select patients with a history of depressive disorders. However, prenatal antidepressant medication use remains controversial, and current evidence does not allow conclusions regarding prenatal antidepressant medication effectiveness in preventing postpartum depression. OBJECTIVE: To determine postpartum depression risk in women stopping versus continuing antidepressant medications during pregnancy a

Patients with schizophrenia in Morocco during the COVID-19 pandemic: a longitudinal observational study.

BACKGROUND: During the COVID-19 pandemic, social-distancing and confinement measures were implemented. These may affect the mental health of patients with mental disorders such as schizophrenia. This study examined the clinical course of patients with schizophrenia at a public hospital in Morocco during the COVID-19 pandemic. METHODS: This longitudinal observational study was conducted across three periods in 15 months: 1 April 2020 (start of strict home confinement) to 30 June 2020 (T1), 1 July 2020 to 31 January 2021 (corresponding to the Delta wave) [T2], and 1 February 2021 to 30 June 202

[Neurocognitive and psychiatric management of the 22q11.2 deletion syndrome].

INTRODUCTION: The 22q11.2 deletion syndrome (22q11.2DS) is caused by hemizygous microdeletions on chromosome 22. 22q11.2DS has several presentations including Di George's syndrome, velo-cardio-facial syndrome or Shprintzen's syndrome and it is the most frequent microdeletion syndrome in the general population (prevalence estimated at 1/4000 births, de novo: 90%). The inheritance of the syndrome (10%) is autosomal dominant. Most people with 22q11.2DS are missing a sequence of about 3 million DNA building blocks (base pairs) on one copy of chromosome 22 in each cell. A small percentage of affect

Neutrophil-to-lymphocyte, platelet-to-lymphocyte ratios and systemic immune-inflammation index in patients with post-traumatic stress disorder.

BACKGROUND: Low-grade systemic inflammation has been reported in many psychiatric diseases and is described as a non-severe state of the inflammatory response. Post-traumatic stress disorder (PTSD) is a chronic psychiatric disorder characterized by symptoms of avoidance, re-experiencing and hyperarousal that develop secondary to a serious traumatic event. The trauma itself creates psychological and biological changes in the individual, apart from PTSD. This complex situation has still not been clarified and researchers have tended to research on inflammatory processes. Systemic immune inflamma

Refining the NEED (Needs Examination, Evaluation and Dissemination) Methods for Use in Psychotic Disorders in Belgium: Lessons From the Design Phase.

BACKGROUND: The NEED (Needs Examination, Evaluation and Dissemination) framework provides a structured, multidimensional approach to assess unmet health-related needs at both patient and societal levels, using predefined criteria, indicators and standardised data collection tools (e.g., questionnaire and interview guide). This paper outlines the methodological adaptations and considerations required when applying the NEED framework to psychotic disorders (PD). PD encompass a spectrum of conditions, including schizophrenia and schizoaffective disorder, and are characterised by overlapping sympt

Systematic Review of the Military Career Impact of Mental Health Evaluation and Treatment.

INTRODUCTION: Military leaders are concerned that active duty members' fear of career impact deters mental health (MH) treatment-seeking. To coalesce research on the actual and perceived consequences of MH treatment on service members' careers, this systematic review of literature on the U.S. Military since 2000 has been investigating the following three research questions: (1) is the manner in which U.S. active duty military members seek MH treatment associated with career-affecting recommendations from providers? (2) Does MH treatment-seeking in U.S. active duty military members impact milit

Integrated treatment of first episode psychosis with online training (e-learning): study protocol for a randomised controlled trial.

BACKGROUND: The integrated treatment of first episode psychosis has been shown to improve functionality and negative symptoms in previous studies. In this paper, we describe a study of integrated treatment (individual psychoeducation complementary to pharmacotherapy) versus treatment as usual, comparing results at baseline with those at 6-month re-assessment (at the end of the study) for these patients, and online training of professionals to provide this complementary treatment, with the following objectives: 1) to compare the efficacy of individual psychoeducation as add-on treatment versus

Detecting schizophrenia, bipolar disorder, psychosis vulnerability and major depressive disorder from 5 minutes of online-collected speech.

Psychosis poses substantial social and healthcare burdens. The analysis of speech is a promising approach for the diagnosis and monitoring of psychosis, capturing symptoms like thought disorder and flattened affect. Recent advancements in Natural Language Processing (NLP) methodologies enable the automated extraction of informative speech features, which has been leveraged for early psychosis detection and assessment of symptomology. However, critical gaps persist, including the absence of standardized sample collection protocols, small sample sizes, and a lack of multi-illness classification,

The postictal systems window hypothesis of electroconvulsive therapy in major depression.

Electroconvulsive therapy (ECT) remains the most effective acute treatment for severe major depressive disorder, particularly in melancholic, psychotic, suicidal, catatonic or retarded, and treatment-resistant presentations. Existing mechanistic accounts are biologically valid but are often presented as parallel lists: monoamine release, excitatory-inhibitory rebalancing, neuroendocrine activation, immune modulation, neurotrophic plasticity, hippocampal change, and network rewiring. We propose the postictal systems window hypothesis. The organizing principle is seizure-induced postictal recoup

Development and evaluation of a risk calculator for fracture-related infection after open reduction and internal fixation: a multi-institutional study.

BACKGROUND: Fracture-related infection (FRI) and subsequent persistent infections significantly affect fracture surgeries. We aimed to develop a precise, personalized risk calculator to assist orthopedic surgeons in evaluating perioperative FRI risk. METHODS: Data from 36,087 patients across two medical centers and four regional hospitals were analyzed. We assessed 29 risk factors, including patient characteristics, comorbidities, fracture location, and surgical variables using multivariable logistic regression. Each factor was weighted based on its regression coefficient. Discrimination and

Eccentric indices based QSPR evaluation of drugs for schizophrenia treatment.

Schizophrenia is a long-term, serious mental health condition that affects how a person thinks, perceives, and behaves. This disorder often results in substantial difficulties in social interactions and work performance. Individuals with schizophrenia might appear disconnected from reality, causing significant distress both for themselves and their Friends. Although symptoms of schizophrenia can vary from person to person, they typically fall into three main categories: cognitive, negative, and psychotic. Creating computational tools to find and develop drugs for schizophrenia has more interes
