In eyes undergoing vitreoretinal surgery up to now dilatation of a miotic pupil by the pupillary stretching technique could be performed only in aphakic eyes. Now a new needle with a tiny eye at its tip has been developed. With the help of this needle, 4 iris loop sutures can be led around the iris only passing the anterior chamber. This permits pupillary stretching also in phakic eyes. The modified technique was used on 3 phakic and 2 aphakic eyes.

A clear lens extraction with insertion of a posterior chamber intraocular lens (PC-IOL) was performed in one eye of each of 20 rabbits. An intracameral injection of 50,000 organisms of gentamicin-sensitive Staphylococcus aureus was given before completion of the surgical procedure. At the completion of surgery, a subconjunctival injection of either gentamicin (20 mg) or gentamicin vehicle was administered randomly, in a masked fashion, to the operative eye. The appearance of large silicone oil globules in the anterior chamber of phakic eyes leads to serious intraoperative and postoperative problems. Using hyaluronate sodium, the silicone can be safely removed from the anterior chamber, the crystalline lens may be saved, and the posterior segment procedure can be completed.

A Q-switched neodymium: YAG laser was used to perform 67 posterior capsulotomies in 67 patients. A clear visual axis was produced in all the eyes. Alterations in regional ocular blood flow produced by topical epinephrine were studied in phakic and aphakic eyes of Macaca arctoides using the radioactive microsphere technique. Three isotopes were used to make sequential blood flow measurements just prior to topical epinephrine administration, 1 hr later and 3 hr later. The degree rehabilitation obtained by cataract surgery was evaluated by visual acuity and by Visual Functioning Index, which measures the patients' ability of performance of every-day tasks that are dependent on vision. Examinations and Index-scoring were done pre- and post-operatively and at one year follow-up in a non-selected, consecutive series of 114 cataract patients. 82% of the patients obtained a final visual acuity of 0.5 or more. Aniseikonia was measured in unilaterally pseudophakic patients using Aulhorn's phase difference haploscope. Mean values of aniseikonia were 1.5% horizontally and 2.0% vertically without correction, and 2.1% horizontally and 2.3% vertically with correction. With spectacle correction, the greater the dioptric difference between two eyes, the greater the amount of aniseikonia. In either case, patients tolerated these amounts of aniseikonia according to our criteria of aniseikonia tolerance.

A soft contact lens can be designed that will satisfy simultaneously in an optimum manner the optical, mechanical, and physiological requirements of the aphakic eye. The relationship fo water content to both refractive index and oxygen transmissibility allows incorporation of the latter quantity into the equation for back vertex power. Starting with the patient's requirement of back vertex power, base curve, and optic zone diameter a contact lens is designed to be mechanically strong, by having minimum water content, and to supply sufficient oxygen for the cornea's needs.

Seventy-three eyes with limbus to limbus bullous keratopathy following cataract extraction were followed 24 months to determine the long-term visual prognosis following penetrating keratoplasty. All but one eye required anterior vitrectomy. A manual vitrectomy technique was utilized in all cases. Results obtained were equal to or better than other contemporary literature reports utilizing mechanical vitrectomy techniques.
