[Carotid sinus syndrome. Contribution of arterial pressure monitoring to intracavitary electrophysiological exploration].

A prospective study based on intracavitary electrophysiological exploration coupled with invasive arterial pressure monitoring was conducted in 245 patients aged 70.2 +/- 13 years who had experienced syncopes and/or lipothymias of uncertain origin. The study showed that 63 patients (25.7 p. 100) had carotid sinus syndrome (CSS). The syndrome was of the vagal type (cardiac inhibition) in 27 p. 100 of the cases, of the depressor type (vasodepression) in 22 p. 100 of the cases and mixed in 51 p. 100 of the cases with the two types of pathological response: electrophysiological and haemodynamic. Effect of catheter ablation of the atrioventricular junction on quality of life and exercise tolerance in paroxysmal atrial fibrillation.

The effect of catheter ablation of the atrioventricular junction (AV) and implantation of a rate-adaptive pacemaker on quality of life and exercise capacity was evaluated prospectively in 12 consecutive patients with paroxysmal atrial fibrillation (AF). All patients had been demonstrated to have paroxysmal AF that was refractory to medical therapy over a long period of time (mean 9 +/- 7 years). Patients performed a symptom-limited treadmill exercise test on the day before catheter ablation and 6 weeks after hospital discharge. Quality of life was measured using the physical dimension of the M

Follow-up evaluation of infant paroxysmal atrial tachycardia: transesophageal study.

We report results of follow-up transesophageal electrophysiologic studies in 35 infants seven to 27 months old (mean, 12 months) in whom paroxysmal atrial tachycardia (PAT) using an accessory atrioventricular (AV) connection had been previously evaluated by transesophageal study in the first 2 months (mean, 14 days) of life. No infants were receiving antiarrhythmic drug therapy at the time of follow-up study. To evaluate AV conduction and initiate PAT, a standard transesophageal pacing protocol was used: single extrastimuli in sinus rhythm, incremental pacing to second-degree AV block, and bur

Studies on the stereoisomers of beta-adrenoceptor antagonists in conscious A-V blocked dogs.

Atrial and ventricular chronotropic effects of the individual stereoisomers of propranolol, pindolol, metoprolol and penbutolol were studied in conscious dogs with chronic atrio-ventricular (A-V) block. Ventricular beta-adrenoceptor blocking activity was assessed for all drugs against isoprenaline under the same experimental conditions. At low doses, the stereoisomers of propranolol and penbutolol decreased atrial rate, whereas those of pindolol and metoprolol produced an increase. At higher doses, all drugs increased atrial rate. All drugs decreased ventricular rate dose-dependently except (+

Unusual mechanism of PR interval variation and nonreentrant supraventricular tachycardia as manifestation of simultaneous anterograde fast and slow conduction through dual atrioventricular nodal pathways.

Noninvasive recordings in a 69-year-old woman showed two distinct PR intervals of about 0.21 and 0.58 s, suggestive of dual AV nodal conduction. Various unusual mechanisms of transition from short to long and from long to short conduction intervals and phenomena of concealed conduction were due to the presence of two functionally separated intranodal pathways. Refractoriness of the slow pathway was associated with bradycardia. Episodes of tachycardia exhibited a one-to-two relationship between P-waves and ventricular activations as a consequence of simultaneous anterograde fast and slow conduc

Selection of patients with recurrence of ventricular tachyarrhythmias for electrical studies.

Clinical cardiac electrophysiologic testing has been employed in the evaluation and treatment of patients with tachyarrhythmias. In patients with recurrent supraventricular tachyarrhythmias, electrophysiologic study is indicated when the arrhythmia is life-threatening or significantly disruptive to lifestyle, when empiric therapy has failed to control the arrhythmia, or when supraventricular tachycardia complicates the Wolff-Parkinson-White syndrome. In patients with ventricular tachyarrhythmias, electrophysiologic study should be undertaken when life-threatening sustained ventricular tachyarr

Therapeutic use of adenosine for terminating spontaneous paroxysmal supraventricular tachycardia.

Intravenous adenosine proved effective in eight out of ten episodes of acute spontaneous supraventricular tachycardia. AV node blockade induced by adenosine occurred 20 s after injection, and disappeared very quickly. Side-effects were transient and never serious. Therefore, it is thought that adenosine can be proposed safely for the short-term treatment of supraventricular tachycardia. Because of the different mechanisms of action, the specific side-effects observed after ATP administration are unlikely to be encountered.

Elective prolongation of atrioventricular conduction by multiple discrete cryolesions: a new technique for the treatment of paroxysmal supraventricular tachycardia.

The present study describes a surgical method for the permanent prolongation of atrioventricular (AV) conduction. In 22 dogs, a series of nine discrete cryolesions were placed along the perimeter of the triangle of Koch while the His bundle electrogram was being monitored continuously. Ten dogs underwent a sham operation to control for the effects of cardiopulmonary bypass and atriotomy on AV conduction. The conduction time through the AV nodal region was significantly (p less than 0.01) prolonged acutely (3 hours postoperatively) and chronically (14 weeks postoperatively) in the animals subje

Sudden death in mitral valve prolapse: associated accessory atrioventricular pathways.

Sudden death in patients with mitral valve prolapse is a rare complication with a possible arrhythmic mechanism. We made a detailed postmortem examination of the conduction system in three patients with MVP who died suddenly. One patient who died in cardiac failure had a normal conduction system. The two other patients with no cardiac symptoms prior to death had both accessory atrioventricular pathways. These findings suggest a higher incidence than hitherto known of accessory bypass tracts in patients with MVP who die suddenly, and support the presumption of an arrhythmic cause of death.

Anatomic effects of cryoablation of the atrioventricular conduction system.

Because of the value of cryoablation of the atrioventricular (AV) conduction system in treatment of refractory cardiac rhythm disorders, the anatomic effects of cryoablation on the cardiac conduction system must be defined. In this report we summarize studies done on four patients who had intractable recurrent supraventricular tachyarrhythmias or or refractory atrial flutter-fibrillation. They were treated by cryoablation of the AV conduction system and died 8-360 days postoperatively. Serial sections of the AV conduction system were studied. Cryoablation produced lesions that completely destr

Dual atrioventricular nodal pathways: a benign finding in arrhythmia-free children with heart disease.

The incidence and significance of dual atrioventricular (A-V) nodal pathways are described in 78 children with associated congenital or acquired heart disease. None of these patients had clinical or electrocardiographic evidence of arrhythmia. Dual A-V nodal pathways were observed in 35 percent of the preoperative group and in 33 percent of the postoperative group. Despite this substrate for A-V nodal reentry, supraventricular tachycardia was neither induced during electrophysiologic evaluation nor did it develop clinically over a follow-up period of 1 month to 15 years. It is concluded that d

Effect of the autonomic blockade on the automaticity of the A-V junctional pacemaker in awake dogs.

The effect of the autonomic blockade on the automaticity of the A-V junctional pacemaker was evaluated in 15 awake dogs with experimentally induced A-V junctional rhythm. The duration of asystole after overdrive (D.A.O.) in these dogs was prolonged significantly in accordance with increase in the drive rate, and the mean +/- SD of the D.A.O. reached 4.7 +/- 1.1 seconds (N = 15) after overdrive at 2.5 times the spontaneous heart rate. After administration of atropine (0.4 mg/kg; i.v.) to eight dogs, the mean +/- SD of the D.A.O. at the same rate decreased from 4.5 +/- 0.9 to 3.4 +/- 1.2 seconds

Retrograde block during dual pathway atrioventricular nodal reentrant paroxysmal tachycardia.

There are limited reported data regarding the occurrence of retrograde block during dual pathway atrioventricular (A-V) nodal reentrant paroxysmal tachycardia. This study describes two patients with this phenomenon. The first patient had 2:1 and type 1 retrograde ventriculoatrial block during the common variety of A-V nodal reentrance (slow pathway for anterograde and fast pathway for retrograde conduction). Fractionated atrial electrograms suggested that the site of block was within the atria. The second patient had type 1 retrograde block (between the A-V node and the low septal right atrium

Multiple ventricular premature beats in five adolescents.

Multiple, short runs of repetitive ventricular premature beats are present in 5 adolescents. These premature beats are continually present, uniform, long and variably coupled and occasionally fuse with sinus beats. This pattern is constant over several years and is associated with generally healthy individuals and otherwise normal hearts. Four of the subjects are completely asymptomatic without syncope or palpitations. The 5th case was asymptomatic for 2 yr until she died suddenly during sleep. Autopsy revealed a pathology of the specialized conduction system in the region of the AV junction.
