What are the progesterone-induced changes of the outcome and the serum markers of injury, oxidant activity and inflammation in diffuse axonal injury patients?

To permit appropriate targeted therapy, the present clinical study was aimed to investigate the effects of progesterone on the outcome and the serum markers of injury, oxidant activity and inflammation in diffuse axonal injury (DAI). Forty-eight male DAI patients were divided into two groups (control and progesterone). Progesterone group received progesterone in dose of 1mg/kg per 12h for five days. The outcome was investigated using Extended Glasgow Outcome Scale (GOS-E) and functional independence measure (FIM). The markers of inflammation [interleukin-1β (IL-1β), IL-6, transforming growth f

Prediction of death by neurologic criteria in patients with non-traumatic acute brain injury: a systematic review and meta-analysis.

BACKGROUND: Robust predictors of death by neurologic criteria (DNC) after non-traumatic acute brain injury remain insufficiently defined. We therefore aimed to systematically synthesize the available evidence on prediction of DNC in non-traumatic brain injury. METHODS: We conducted a systematic review and meta-analysis in accordance with PRISMA guidelines. MEDLINE, Embase, Scopus, the Cochrane Library and Web of Science were searched to August 31, 2026. Eligible studies included randomized controlled trials, observational studies, case series and registry data. Studies were included in the qu

Fecal microbiota transplantation alleviates sepsis-associated encephalopathy by reshaping gut microbiota and metabolism.

BACKGROUND: Sepsis-associated encephalopathy (SAE) is a diffuse brain dysfunction secondary to sepsis; however, its pathogenesis remains poorly defined. This study characterizes gut-microbiota-brain axis dysbiosis and aberrant tryptophan metabolism in SAE, providing a multi-dimensional framework to understand the underlying pathways. METHODS: SAE was induced in rats via cecal ligation and puncture, with Sham and fecal microbiota transplantation (FMT) groups as controls. Cognitive and emotional functions were assessed using the open field and novel object recognition tests. Gut microbiota and

Cerebral Hemodynamic Changes During Cardiac Surgery: A Feasibility MR Study in Piglets.

Congenital heart disease often requires early surgical intervention. Cardiopulmonary bypass (CPB) is a standard procedure, but infants undergoing CPB show an increased risk of postoperative white matter injury. These injuries are associated with adverse neurodevelopmental outcomes, yet the underlying mechanisms remain poorly understood. MR imaging offers unique opportunities to study cerebral perfusion and metabolism in vivo but has not previously been feasible during CPB with a heart-lung machine (HLM) in close proximity to the patient and the MR scanner, respectively, because conventional HL

Toxoplasma gondii infection is associated with changes in chronic outcomes in traumatic brain injury.

Traumatic brain injury (TBI) can lead to lasting neurological and emotional effects. Latent Toxoplasma gondii (T. gondii) infection, prevalent worldwide, may exacerbate these outcomes by altering immune and neurochemical pathways. This cross-sectional observational study investigated whether chronic T. gondii infection is associated with structural brain differences and long-term outcomes in survivors of moderate-to-severe TBI ≥ 10 years post-injury. 89 TBI survivors (≥10 years post-injury) were recruited from a tertiary-centre database; 35 (39%) tested positive for latent T. gondii via plasma

Impulse control deficits among patients with nonsuicidal self-injury: a mediation analysis based on structural imaging.

BACKGROUND: Nonsuicidal self-injury (NSSI) is posited to arise from a complex interaction of biopsychosocial factors, with impulsivity playing a critical role. Given that current research on the neural mechanisms underlying this hypothesis remains inconsistent and limited in scope, we sought to explore how NSSI behaviours are associated with impulsivity resulting from structural brain alterations. METHODS: We recruited patients with NSSI behaviours and healthy controls from 11 psychiatric hospitals. We assessed the differences in impulse control between the 2 groups using the Barratt Impulsiv

Frequency, types and causes of intraventricular haemorrhage in lethal blunt head injuries.

Autopsy findings and neuropathological examination of formalin-fixed brains in 676 deaths due to blunt head injury, here with special attention to injuries of the inner (periventricular) cerebral structures and haemorrhages into the ventricles. Intraventricular haemorrhage of any degree was present in 17.6%, considering only distinct and massive haemorrhage in 10% of all cases. Considering the types of trauma, the frequency was lowest in ground level falls and highest in traffic accidents (pedestrians with head contact to the car) - indicating a relation between the severity of impacts and the

Assessing therapeutic response non-invasively in a neonatal rat model of acute inflammatory white matter injury using high-field MRI.

Perinatal infection and inflammatory episodes in preterm infants are associated with diffuse white matter injury (WMI) and adverse neurological outcomes. Inflammation-induced WMI was previously shown to be linked with later hippocampal atrophy as well as learning and memory impairments in preterm infants. Early evaluation of injury load and therapeutic response with non-invasive tools such as multimodal magnetic resonance imaging (MRI) would greatly improve the search of new therapeutic approaches in preterm infants. Our aim was to evaluate the potential of multimodal MRI to detect the respons

Differences in Mortality Between Veterans With Posttraumatic and Nontraumatic Epilepsy.

BACKGROUND AND OBJECTIVES: Higher mortality, relative to the general population, is associated with epilepsy and with particular types of traumatic brain injury (TBI). It is thus presumed that posttraumatic epilepsy (PTE), which indicates epilepsy after TBI, would have higher mortality relative to nontraumatic epilepsy (NTE). However, previous studies have not established a difference in mortality between PTE and NTE. METHODS: We analyzed administrative data in the Veterans Health Administration to identify US military veterans diagnosed with epilepsy from 2005 to 2022 using previously valida

Multimodal MRI to elucidate cross-organ coupling mechanisms of the heart-brain axis.

Cardiovascular and cerebrovascular diseases (CCVDs) are leading causes of mortality and disability worldwide, and their development is closely linked to bidirectional interactions between the heart and the brain. The heart-brain axis, a systemic framework that integrates neural, humoral, and inflammatory pathways, provides an important theoretical basis for understanding heart-brain comorbidities. With continuous advances in magnetic resonance imaging(MRI), research in this field has progressively shifted from the traditional single-organ diagnostic model to cross-organ and system-oriented str

Rapid atlas-based predictions of regional brain strain simulations from wearable head kinematics using diffusion MRI-informed machine learning.

Sports-related traumatic brain injury (TBI) remains significantly underdiagnosed, with up to 50% of mild TBI cases in sport going undetected. While finite element (FE) simulations can predict brain deformation from head impacts, their computational cost limits clinical applicability for on-field assessments. This study shows a proof-of-principle in using a personalised machine learning framework capable of rapidly predicting regional brain strain simulations over time from wearable sensor kinematics, requiring as few as 150 head acceleration events (HAE) per athlete for training. Diagnostic value of centrally restricted diffusion in differentiating radiation necrosis from tumor progression in brain metastases: A single-center observational study.

BACKGROUND: Distinguishing radiation necrosis (RN) from true progression (TP) in irradiated brain metastases is challenging. We evaluated the diagnostic performance of the centrally restricted diffusion sign on diffusion-weighted imaging (DWI). METHODS: From August 2014 to August 2024, we screened 321 patients with histologically confirmed brain metastases treated with radiation therapy and follow-up MRI for new or enlarging necrotic lesions ≥1 cm. Two board-certified neuroradiologists independently assessed the centrally restricted diffusion sign-central hyperintensity on b1000 images with c

Aberrant dynamic functional network connectivity in patients with diffuse axonal injury.

Diffuse axonal injury (DAI) results in aberrant functional connectivity and is significantly linked to cognitive impairment. Nevertheless, the network mechanisms influencing neurocognitive function following DAI remain unclear. This study aimed to examine the characteristics of static and dynamic functional network connectivity (FNC) in patients with DAI. Resting-state functional magnetic resonance imaging data were collected from 26 patients with DAI and 27 healthy controls. Resting-state networks were extracted using independent component analysis. We evaluated the connectivity strength thro

Non-invasive in vivo MRI detects long-term microstructural brain alterations related to learning and memory impairments in a model of inflammation-induced white matter injury.

Magnetic resonance imaging (MRI) is currently under investigation as a non-invasive tool to monitor neurodevelopmental trajectories and predict risk of cognitive deficits following white matter injury (WMI) in very preterm infants. In the present study, we evaluated the capacity of multimodal MRI (high-resolution T2-weighted imaging and diffusion tensor imaging)to assess changes following WMI and their relationship to learning and memory performance in Wistar rats as it has been demonstrated for preterm infants. Multimodal MRI performed at P31-P32 shown that animals exposed to neonatal LPS cou

Conditioned Contextual Freezing is A Neurobehavioral Biomarker of Axonal Injury Indicated by Reduced Fractional Anisotropy in A Mouse Model of Blast-Induced Mild Traumatic Brain Injury.

Mild traumatic brain injury (TBI) is an important public health problem generated by closed head injury. This study is focused on the impact of blast-induced mild TBI on auditory trace and delay fear conditioning, models of declarative and non-declarative memory, respectively, and the correlation of conditioned freezing and fractional anisotropy, a measure of axonal state. A supersonic helium pressure wave was generated by a shock tube to blast 8-week-old male mice on Day 1 for 1.4 msec with an incident pressure of 16 psi, corresponding to a reflected pressure of 56.9 psi at the mouse head. Early diffusion-weighted imaging and outcome prediction of comatose survivors after suicidal hanging.

PURPOSE: Early outcome prediction after suicidal hanging is challenging in comatose survivors. We analysed the early patterns of brain diffusion-weighted magnetic resonance imaging (DWI) abnormalities in comatose survivors after suicidal hanging. METHODS: After suicidal hanging, 18 comatose survivors were prospectively evaluated from January 2013 to December 2016. DWI was performed within 3 h after hanging in comatose survivors. We evaluated Utstein style variables and analysed abnormal spatial profile of signal intensity on DWI, brain apparent diffusion coefficient (ADC) values, and qualitat

Post-Traumatic Cerebral Microhemorrhages and their Effects Upon White Matter Connectivity in the Aging Human Brain.

Cerebral microbleeds (CMBs), a common manifestation of mild traumatic brain injury (mTBI), have been sporadically implicated in the neurocognitive deficits of mTBI victims but their clinical significance has not been established adequately. Here we investigate the longitudinal effects of post-mTBI CMBs upon the fractional anisotropy (FA) of white matter (WM) in 21 older mTBI patients across the first ~6 months post-injury. CMBs were segmented automatically from susceptibility-weighted imaging (SWI) by leveraging the intensity gradient properties of SWI to identify CMB-related hypointensities u

Intensive management of severe head injuries. A scheme of intensive management of severe head injuries.

Seventy-six severely head-injured patients, 67% of whom had Glasgow Coma scores of five or less on admission, were managed according to an intensive treatment regime which included controlled hyperventilation (under full muscular paralysis), high-dose steroids, dehydrating agents, diuretics and hypnotics (Althesin and thiopentone). Intracranial pressure (ICP) was measured throughout the period of controlled ventilation. Treatment was directed to keeping the mean ICP below 25-30 mmHg and to the prevention of increases in ICP during chest physiotherapy and other noxious stimulation. Six months a
