Neuropsychologic assessment and cognitive rehabilitation in a patient with locked-in syndrome and left neglect.

We describe a patient affected by severe incomplete locked-in syndrome (LIS) and left neglect caused by a combination of vascular lesions. Our patient's neglect prevented the use of augmentative communication devices based on a computerized eye-tracker system. For this reason, we adapted a visual scanning training for neglect rehabilitation. At the end of the rehabilitative training, the patient had regained full exploration of the monitor and could use the eye-tracker system for communicative purposes. This case report shows that specific rehabilitative approaches can be devised in severely d

Behavioral interventions for tic disorders.

A variety of treatment approaches have been used to manage tic symptoms in Tourette syndrome and other tic disorders. Pharmacological interventions remain the most common approach, but in the past 3 decades, various nonpharmacological treatment options have emerged including: (1) massed practice, (2) relaxation training, (3) self-monitoring, (4) function-based/contingency management procedures, (5) habit reversal training, (6) exposure and response prevention, and (7) cognitive behavior therapy. Each of these procedures is described along with the evidence reflecting its efficacy and usefulnes

Improving the Recognition and Assessment of ICU Delirium.

BACKGROUND: Delirium is acute neurological failure that alters cognition and results in a state of confusion and lack of self-awareness. It commonly affects critically ill patients within intensive care units (ICUs), and the wide array of fluctuating symptoms frequently leads to underdiagnosis. Delirium results in cognitive impairment and increased rates of fatality, and accurate assessments are essential for improving patient outcomes. Barriers to delirium assessment stem from a lack of knowledge and assessment training among ICU nurses. METHOD: Using the Cumulative Index to Nursing and Alli

Combining exercise and hypoxia for brain health: from elite athlete altitude training camps to clinical applications.

Regular physical exercise promotes overall and brain health and can improve cognition and mood. Similar effects on brain health have been observed after well-controlled intermittent exposures to low ambient oxygen (hypoxia), termed hypoxia conditioning, in several pilot studies. This raises the question of how exercising in hypoxia affects the brain. Among hypoxic exercise training forms are altitude training camps, which have been used systematically by athletes to improve physical performance for many decades, or exposure to simulated altitude/inspiratory hypoxia in laboratory settings, comb

Group cognitive therapy for social anxiety disorder among adolescents treated at routine adolescent psychiatric services: preliminary effects and implementation model.

PURPOSE: Social Anxiety Disorder (SAD) typically onsets in adolescence. Few studies have targeted delivery of evidence-based interventions for SAD in mental health services for adolescents. In specific, no prior studies have examined group cognitive therapy (CT) for SAD in adolescent services delivered by method-trained staff. We implemented group CT in psychiatric outpatient units to assess its feasibility and effectiveness in routine services. MATERIALS AND METHODS: In the context of a region-wide implementation program launched by Helsinki University Hospital (HUH), eleven psychiatric nurs

BACKGROUND: Early detection of cognitive decline during the preclinical stage of Alzheimer's disease and related dementias (AD/ADRD) is crucial for timely intervention and treatment. Clinical notes in the electronic health record contain valuable information that can aid in the early identification of cognitive decline. In this study, we utilize advanced large clinical language models, fine-tuned on clinical notes, to improve the early detection of cognitive decline. METHODS: We collected clinical notes from 2,166 patients spanning the 4 years preceding their initial mild cognitive impairment

The effects of task-relevant suggestions on motivated cognitive control.

To deal with the only constant in life, change, we use a set of top-down processes called cognitive control. Cognitive control enables us to develop new stimulus-response associations appropriate for the task at hand instead of being rigidly bound to our existing repertoire of responses. Given the central role cognitive control has in our lives, it is not a surprise that different methods have been tested for improving it; however, few have shown generalizable, long-term effects. One approach, which has shown great promise in enhancing performance in different tasks requiring cognitive control

[Treatment of social phobia in childhood and adolescence].

OBJECTIVES: The following article investigates the extent to which the current status of cognitive behavioral research on the treatment of socially phobic children and adolescents is reflected by the currently used guidelines for psychotherapy. METHODS: The current literature on research in psychotherapy was systematically reviewed. RESULTS: The results show the significance of single-setting treatment, of cognitive interventions, and of parental participation in the therapy. The results also show the limitations encountered if the treatment consists only of training social competence skills

A virtual reality-based self-guided training on identification of negative automatic thoughts in healthy adults: a mixed-methods feasibility study.

BACKGROUND: Cognitive restructuring (CR) is an evidence-based psychological technique for depression. However, face-to-face CR is not easily accessible. Digital CR interventions often overlook the difficulties individuals experiencing depression encounter in identifying their negative automatic thoughts (NAT), leading to suboptimal outcomes. Virtual Reality (VR) has potential advantages in assisting the identification of NAT in CR intervention. OBJECTIVE: The aim of this preliminary feasibility study is to assess the efficacy, acceptability and safety of a VR-based self-guided training on the

Skill-learning by observation-training with patients after traumatic brain injury.

UNLABELLED: Traumatic brain injury (TBI) is a major cause of death and disability in Western society, and often results in functional and neuropsychological abnormalities. Memory impairment is one of the most significant cognitive implications after TBI. In the current study we investigated procedural memory acquisition by observational training in TBI patients. It was previously found that while practicing a new motor skill, patients engage in all three phases of skill learning-fast acquisition, between-session consolidation, and long-term retention, though their pattern of learning is atypic

Perturbation-based gait training to improve daily life gait stability in older adults at risk of falling: protocol for the REACT randomized controlled trial.

BACKGROUND: The European population is rapidly ageing. There is an urgent need for innovative solutions to reduce fall risk in older adults. Perturbation-based gait training is a promising new method to improve reactive balance responses. Whereas positive effects on task-specific dynamic balance recovery during gait have been shown in clinical or laboratory settings, translation of these effects to daily life gait function and fall risk is limited. We aim to evaluate the effect of a 4-week perturbation-based treadmill training on daily-life dynamic gait stability, assessed with inertial sensor

Description and Effectiveness of Environmental Approaches to Cognitive Rehabilitation for Traumatic Brain Injury: A Systematic Review of Randomised Controlled Trials.

Interventions addressing cognitive impairments after traumatic brain injury (TBI) often rely on survivors' relatively preserved capacity to learn, initiate, and consistently use rehabilitation strategies, along with the ability to apply strategies in various contexts. The cognitive functions required for such active engagement in rehabilitation are commonly impaired after TBI, consequently limiting the benefits of many existing cognitive rehabilitation approaches. Environmental approaches, which involve manipulating the environment to support cognition and function, may benefit TBI survivors,

A prediction nomogram for mild cognitive impairment in type 2 diabetes mellitus based on the Chinese visceral adiposity index.

Visceral adiposity has been proposed to be closely linked to cognitive impairment. This cross-sectional study aimed to evaluate the predictive value of Chinese Visceral Adiposity Index (CVAI) for mild cognitive impairment (MCI) in patients with type 2 diabetes mellitus (T2DM) and to develop a quantitative risk assessment model. A total of 337 hospitalized patients with T2DM were included and randomly assigned to a training cohort (70%, n = 236) and a validation cohort (30%, n = 101). Demographic, clinical, and neuropsychological data were collected. CVAI levels were compared between patients w
