Three cases of contact dermatitis caused by an ethanolamine antihistamine, diphenhydramine hydrochloride (Benadryl), are described. Patients who develop contact dermatitis from it should avoid its oral or parenteral administration because they may develop a localized or generalized flare of dermatitis. Dimenhydrinate (Dramamine), clemastine fumarate (Tavist), carbinoxamine maleate (Clistin-D), and doxylamine succinate (Decapryn) are also ethanolamine antihistamines. Since they may cross-react with diphenhydramine hydrochloride, they should be avoided in sensitive patients.

Glyceryl monothioglycolate has recently been implicated as an allergen. A case of a hairdresser who used this material is presented. He appeared to have an irritant contact dermatitis on his fingertips. Patch testing the patient and extensive patch testing in a group of fifty-four volunteers showed a very high incidence of irritant positive patch tests. No instance of allergy could be demonstrated in the reading of the patch tests or in biopsies of several individuals. Differentiation between allergens and irritants is difficult at times, but is necessary to avoid mislabeling of chemicals.

Fiberglass and rockwool dermatitis is usually due to a mechanical irritant reaction. When several members of a family are affected, scabies is often initially suspected. The irritant dermatitis may be complicated by an urticarial and an eczematous reaction which may mimic an allergic reaction clinically and histologically. Allergic reactions to added epoxy or formaldehyde resins may very rarely occur.

Two cases of contact dermatitis from electroplating solutions were attributed to irritation from cyanide salts. Both demonstrated similar clinical features. In neither could concomitant contact allergy to other constituents of the plating solutions be demonstrated. In one case, irritation could not be attributed to the alkalinity of the plating solution. Contact dermatitis from cyanide plating solutions may be associated with systemic symptoms, since cyanide is rapidly absorbed through the skin. Poor handling and hygiene techniques in the workplace should be corrected immediately.

Transcutaneous electric nerve stimulation (TENS) is an effective treatment modality for chronic pain. Electrical impulses produced by a portable stimulator are transmitted by cable to electrodes which are attached to specific areas of the body by tape or adhesive. A conductive electrolyte jelly is utilized in order to make good electrical contact with the skin. Several dermatologic complications have been associated with the use of this technic, including allergic and irritant contact dermatitis and micropunctate burns caused by the improper use of poorly conductive lubricant jellies. A patien…

Of the 31 patients who developed polyarthritis following jejunoileal bypass for obesity, 24 had cutaneous vasculitis (urticarial, pustular, and nodular), 11 paresthesias, 10 Raynaud's phenomenon, and 1 pericarditis. Blind loop symptoms (14 of 26 patients), cryoglobulinemia (10 of 28), and immune deposits in biopsied skin lesions (5 of 7) support the theory of a relationship between bowel bacteria and immune complexes. Treating the blind loop with antibiotics and sphincteroplasty to prevent bacterial reflux into the blind loop helped 5 of 10 and 6 of 9 patients, respectively. A comparison is ma

Despite the decline in the incidence of berloque dermatitis during the past decade, milder cases resulting from the presence of relatively small quantities of bergapten in perfumed cosmetics continue to occur. The resultant hyperpigmentation may have an atypical presentation leading to error in diagnosis. In addition, the condition can sometimes induce or aggravate melasma and may be responsible for other pigmentary disorders of the face and neck of questionable etiology such as poikiloderma of Civatte, Riehl's melanosis and pigmented peribuccal erythema of Brocq.

A 73-year-old woman developed an allergic contact dermatitis of the feet and legs 24 days following lymphangiography. Dermatitis was accompanied by a generalized urticarial reaction. Intracutaneous skin testing was positive to Evans blue dye, used as a lymphatic marker, but negative to Ethiodol, an iodine-containing contrast medium. Soft tissue x-rays x-rays revealed extravasated Ethiodol in the left leg but not in the right, while dermatitis occurred on both legs. This finding, combined with the positive skin test to Evans blue, stress the importance of testing with both agents when dermatiti…

Exfoliative dermatitis associated with hepatitis has been reported in leprosy patients taking dapsone in daily dosage of 200 mg daily or more previously. This report concerns a 40 year-old-female who developed exfoliative dermatitis and hepatitis six weeks after starting dapsone in daily dosage of 100 mg for treatment of leprosy. She responded dramatically to administration of corticosteroids.

A patient experienced allergic contact dermatitis on two occasions two months apart as a result of wearing the same brand of polyvinyl chloride oxygen cannula. In one instance the cannula was removed and not replaced, as continuing oxygen was unnecessary; on the other occasion the original cannula was replaced by a cannula of another brand. In both cases the dermatitis disappeared after removal of the original cannula. The reaction was probably to a resin remaining in the polyvinyl chloride after the curing process in the manufacture of the plastic from which the cannula was made. Allergic rea…

A 75-year-old woman had lichenoid and bullous lesions in the sun-exposed areas. She had a history of cerebral embolism and received chlorpromazine (CPZ) until consultation at our clinic. Patch tests with CPZ were negative, but photopatch tests were positive at relatively high concentrations of CPZ and ultraviolet light radiation sites of 310, 340 and 370 nm. Rapid improvement of the lesions occurred upon discontinuation of CPZ and the avoidance of exposure to sunlight. From these results, the patient was considered to show lichenoid eruptions due to CPZ phototoxicity rather than photoallergy.
