Skeletal muscle ergoreflex overactivity is not related to exercise ventilatory inefficiency in non-hypoxaemic patients with COPD.

Increased ventilatory response to the metabolic demand ("ventilatory inefficiency") is commonly found during dynamic exercise in patients with chronic obstructive pulmonary disease (COPD). However, the role of enhanced muscle ergoreflex activity on this phenomenon is yet unknown. Ten non-hypoxaemic patients with varying degrees of disease severity (median and range of post-bronchodilator FEV(1) = 37.5 (27 to 70%) predicted) and 7 age- and gender-matched controls were studied. Subjects were submitted to wrist flexion tests to the limit of tolerance (Tlim) with and without post-exercise regional

Physical allergies and exercise. Clinical implications for those engaged in sports activities.

There are several allergic responses that may occur in susceptible individuals as a result of exposure to physical stimuli. Most of these conditions are mediated by vasoactive substances and usually result in symptoms of urticaria and/or angioedema. There are 2 such conditions that may occur as a direct result from exercise. The first of these is cholinergic urticaria. Patients with cholinergic urticaria experience punctate (2 to 4mm) hives which occur reproducibly with exercise or with passive warming, such as might occur in a steam bath or hot pool. Life-threatening hypotension or angioedema

Distinct Exercise Response Patterns in Patients With Heart Failure With Preserved Ejection Fraction.

BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) is a heterogeneous disease characterized by exercise intolerance. Defining pathophysiologically distinct subgroups allows more personalized therapy, but efforts mainly relied on resting examinations. This study aimed to define HFpEF phenotypes based on exercise limitations using combined cardiopulmonary exercise testing with stress echocardiography. METHODS: A total of 913 patients with HFpEF were recruited from 4 third-line centers and divided into derivation (n=623) and validation cohorts (n=290). Unsupervised graph-based cl

Which Exercise Prescriptions Improve Physical Fitness in Patients with Breast Cancer Before, During, and Following Treatment? A Systematic Review and Meta-analysis of Randomized Controlled Trials.

BACKGROUND: Exercise is now recognized as an essential component of the cancer care continuum. However, the optimal exercise prescriptions to improve physical fitness in breast cancer remain under debate. OBJECTIVES: In a systematic review and meta-analysis of randomized controlled trials (RCTs), we investigated how exercise affects muscular strength, fat-free mass, functional exercise capacity (FEC) and cardiorespiratory fitness during pretreatment, active treatment, and post-treatment for breast cancer. We also investigated how exercise mode, frequency, intensity, supervision, and duration

Prognostic impact of chronotropic incompetence in transthyretin cardiac amyloidosis: a multicentre study.

INTRODUCTION: Chronotropic incompetence (CI) is a frequent but underappreciated feature of cardiac amyloidosis and may contribute to exercise intolerance. However, its prognostic significance remains incompletely defined. We investigated the prevalence, functional correlates, and prognostic value of chronotropic incompetence in patients with transthyretin amyloid cardiomyopathy. METHODS: In this multicentre retrospective study, 212 stable outpatients with transthyretin amyloid cardiomyopathy, in sinus rhythm and naïve to disease-specific therapy, underwent maximal cardiopulmonary exercise tes

Reference standards and diagnosis-specific trends in cardiorespiratory fitness in paediatric patients with repaired CHD.

BACKGROUND: Exercise capacity (VO2peak) predicts mortality in adult patients with CHD. There is a lack of paediatric exercise capacity data based on specific CHD lesions, limiting the ability to contextualise interpretation based on expected performance during testing. The primary aim of this study was to establish VO2peak percentiles for paediatric patients with repaired CHD undergoing treadmill-based cardiopulmonary exercise testing (CPET). METHODS: Retrospective analysis of CPET data from 2004 to 2022. CPETs were analysed for patients with CHD aged 6-18 years. Patients with repaired CHD we

Right Heart Adaptation to Exercise in Pulmonary Hypertension: An Invasive Hemodynamic Study.

BACKGROUND: Right heart failure (RHF) is associated with a dismal prognosis in patients with pulmonary hypertension (PH). Exercise right heart catheterization may unmask right heart maladaptation as a sign of RHF. We sought to (1) define the normal limits of right atrial pressure (RAP) increase during exercise; (2) describe the right heart adaptation to exercise in PH owing to heart failure with preserved ejection fraction (PH-HFpEF) and in pulmonary arterial hypertension (PAH); and (3) identify the factors associated with right heart maladaptation during exercise. METHODS AND RESULTS: We ana

Cardiac rehabilitation program improves exercise capacity in heart transplantation recipients regardless of marginal donor factors.

Cardiac rehabilitation (CR) is recommended to improve exercise capacity after heart transplantation (HTx); however, the effects of marginal donor factors are unclear. Forty-one recipients participated in a 3-month CR program early after HTx (mean age 39 ± 14 years; 88% male). Patients were divided into marginal (≥ 2 marginal donor factors; n = 24) and control groups (< 2 marginal donor factors; n = 17). We examined donor and recipient factors related to change in peak oxygen uptake (peak VO2) during the CR program using multiple linear regression analysis. Baseline characteristics were similar

Effect of Passive Stretching of Respiratory Muscles on Chest Expansion and 6-Minute Walk Distance in COPD Patients.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a major cause of morbidity and mortality worldwide. Hyperinflation of the lungs leads to a remodeling of the inspiratory muscles that causes postural deformities and more labored breathing. Postural changes include elevated, protracted, or abducted scapulae with medially rotated humerus, and kyphosis that leads to further tightening of respiratory muscles. As the severity of the disease progresses, use of the upper limbs for functional tasks becomes difficult due to muscle stiffness. There are various studies that suggest different re
