Modelling and analysis of the HIV/AIDS epidemic with fast and slow asymptomatic infections in China from 2008 to 2021.

The aim of this paper is to investigate the spread of the HIV/AIDS epidemic in China during 2008-2021. A new mathematical model is proposed to study the dynamics of HIV transmission with acute infection, fast asymptomatic infections, and slow asymptomatic infections. The basic reproduction number is obtained by the next-generation matrix method. A quantitative analysis of the model, including the local behavior, global behavior, and permanence, is performed. Numerical simulations are presented to enhance the results of these analyses. The behavior or the model's parameters are estimated from r

Design and Synthesis of Highly Potent HIV-1 Protease Inhibitors Containing Tricyclic Fused Ring Systems as Novel P2 Ligands: Structure-Activity Studies, Biological and X-ray Structural Analysis.

The design, synthesis, and biological evaluation of a new class of HIV-1 protease inhibitors containing stereochemically defined fused tricyclic polyethers as the P2 ligands and a variety of sulfonamide derivatives as the P2' ligands are described. A number of ring sizes and various substituent effects were investigated to enhance the ligand-backbone interactions in the protease active site. Inhibitors 5c and 5d containing this unprecedented fused 6-5-5 ring system as the P2 ligand, an aminobenzothiazole as the P2' ligand, and a difluorophenylmethyl as the P1 ligand exhibited exceptional enzym

Incarcerated Black Women in the Southern USA: A Narrative Review of STI and HIV Risk and Implications for Future Public Health Research, Practice, and Policy.

Incarcerated black women in the southern USA are understudied despite the high prevalence of sexually transmitted infections (STI) and human immunodeficiency virus (HIV). These incarceration and health disparities are rooted in centuries of historically inequitable treatment. Amidst the current dialogue on mass incarceration in the south and its relationship to the health of the black community, individual and environmental risk factors for STI/HIV transmission are seldom paired with discussions of evidence-based solutions. A narrative review of the literature from January 1995 to May 2015 was

Management of Pneumocystis Jirovecii pneumonia in HIV infected patients: current options, challenges and future directions.

The discovery of the Human Immunodeficiency Virus (HIV) was led by the merge of clustered cases of Pneumocystis jirovecii Pneumonia (PCP) in otherwise healthy people in the early 80's.1,2 In the face of sophisticated treatment now available for HIV infection, life expectancy approaches normal limits. It has dramatically changed the natural course of HIV from a nearly fatal infection to a chronic disease.3-5 However, PCP still remains a relatively common presentation of uncontrolled HIV. Despite the knowledge and advances gained in the prevention and management of PCP infection, it continues to

Inflammatory responses in HIV-1 genetic subtypes in relationship with anti-retroviral drug resistance in Minna, Niger State, North Central Nigeria.

BACKGROUND: The high genetic diversity of HIV-1, persistent immune activation and rising antiretroviral drug resistance continue to hinder HIV treatment, especially in Sub-Saharan Africa. This study examined inflammatory responses in relation to HIV-1 genetic subtypes and antiretroviral drug (ARVD) resistance among patients experiencing virological failure in Minna, Niger State, Nigeria. METHODOLOGY: A cross-sectional design involved people living with HIV-1 (PLWH-1) on ART with virological failure (defined as repeated viral load ≥ 1000 copies/mL). Plasma levels of IL-6, IL-10, hs-CRP, and TN

Immune control of HIV viraemia in early paediatric infection is associated with induction of HIV-specific CD8+ T-cell activity following multiple treatment interruptions.

BACKGROUND: Virus-specific CD8+ T-cells play an important part in HIV cure/remission in adults, yet their role in paediatric immune control is limited by tolerogenic early-life immunity. Very-early ART initiation, while effective in restricting viral reservoir size, also prevents antigenic exposure and, thereby, the induction of HIV-specific CD8+ T-cell responses. Analytical treatment interruption (ATI) is an established tool in cure/remission studies for assessing time to viral rebound and viral setpoint off ART yet the impact of ATI on HIV-specific immune responses and plasma viral load (pVL

Sigma-1 receptor regulates HIV-1 and methamphetamine-induced endothelial/pericyte barrier impairment via strain-specific inflammatory responses and mitochondrial dysregulation.

BACKGROUND: Methamphetamine (METH) use disorder (MUD) is a frequent comorbidity among people with HIV-1 (PWH). While both METH and HIV-1 independently disrupt the brain microvascular system and promote neuroinflammation, the molecular mechanisms underlying their synergistic neurotoxicity remain unclear. The sigma-1 receptor (Sigma-1R) signaling pathway has been suggested as a potential target in METH misuse; however, its involvement in the impact of METH and/or HIV-1 infection on brain pericytes remains unknown. METHODS: Transwell co-cultures of human brain microvascular endothelial cells (HB

Rethinking HIV care for youth: Insights from qualitative research with youth in Chad.

Youth ages 15-24 years are significantly impacted by the HIV/AIDS epidemic, representing approximately 37% of new infections globally. This demographic is especially vulnerable in sub-Saharan Africa, where over 80% of HIV-positive youth reside. In Chad, youth face barriers to effective HIV care, including high prevalence rates, particularly among young women, and substantial disparities across regions. Despite overall reductions in new HIV infections, youth remain disproportionately affected, necessitating targeted interventions to improve HIV care outcomes. This study represents a secondary d

High HIV-1 viremia and low anti-Env antibody responses are associated with delayed treatment response to fostemsavir in highly treatment-experienced individuals.

Fostemsavir (FTR) is an approved first-in-class small molecule Env antagonist for treating multi-drug resistant (MDR) HIV-1 infection. In the BRIGHTE study, viral suppression rates in heavily treatment-experienced people with HIV (PWH) increased from week 48 through week 96. Factors that contribute to this late response are not well understood. Given FTR's ability to stabilize a native HIV-1 envelope trimer conformational state, we examined anti-HIV humoral immune responses in the BRIGHTE study to explore how evolving antibody responses in the presence of drug correlate with delayed viral supp

"Men Take Care of Each Other": Evaluation of a Community-Based Model for Pre-exposure Prophylaxis Services Among Male Bar Patrons in Rural South Africa.

Low engagement with HIV services persists among young men with harmful alcohol use in South Africa. We previously piloted a rural community-based HIV service delivery model to engage this key population. In the initial study, male nurses visited alcohol-serving venues to provide HIV testing and pre-exposure prophylaxis (PrEP) services. From November 1 to December 30, 2021, we conducted interviews with 17 of 34 male pilot participants to evaluate program barriers, facilitators, and suggestions. All interviewees were satisfied with HIV testing and PrEP services. Participants overcame testing avo

Transgender Youth Experiences and Perspectives Related to HIV Preventive Services.

BACKGROUND: In the United States, transgender youth are at especially high risk for HIV infection. Literature regarding HIV prevention strategies for this vulnerable, often-hidden population is scant. Before effective, population-based HIV prevention strategies may be adequately developed, it is necessary to first enhance the contextual understanding of transgender youth HIV risk and experiences with HIV preventive services. METHODS: Two 3-day, online, asynchronous focus groups were conducted with transgender youth from across the United States to better understand participant HIV risk and ex

Multisite Study of Women Living With HIV's Perceived Barriers to, and Interest in, Long-Acting Injectable Antiretroviral Therapy.

BACKGROUND: Adherence to antiretroviral therapy (ART) is imperative for viral suppression and reducing HIV transmission, but many people living with HIV report difficultly sustaining long-term adherence. Long-acting injectable (LAI) ART has the potential to transform HIV treatment and prevention. However, little LAI ART-related behavioral research has occurred among women, particularly outside of clinical trials. SETTING: Six Women's Interagency HIV Study sites: New York, Chicago, Washington DC, Atlanta, Chapel Hill, and San Francisco. METHODS: We conducted 59 in-depth interviews with women

Strategies to eradicate HIV from infected patients: elimination of latent provirus reservoirs.

35 years since identification of HIV as the causative agent of AIDS, and 35 million deaths associated with this disease, significant effort is now directed towards the development of potential cures. Current anti-retroviral (ART) therapies for HIV/AIDS can suppress virus replication to undetectable levels, and infected individuals can live symptom free so long as treatment is maintained. However, removal of therapy allows rapid re-emergence of virus from a highly stable reservoir of latently infected cells that exist as a barrier to elimination of the infection with current ART. Prospects of a

Tuberculous meningitis is associated with higher cerebrospinal HIV-1 viral loads compared to other HIV-1-associated meningitides.

To gain a better understanding of the immunopathogenesis of tuberculous meningitis (TBM) and identify potential diagnostic biomarkers that may discriminate TBM from other HIV-1-associated meningitides, we assessed HIV-1 viral load levels, drug resistance patterns in antiretroviral therapy (ART)-experienced patients with persistent viremia and soluble immunological analytes in peripheral blood and cerebrospinal fluid (CSF) of HIV-1 infected patients with TBM versus other meningitides. One hundred and three matched blood and CSF samples collected from HIV-1 infected patients with TBM or other me

Th1/Th2 cytokine expression in saliva of HIV-positive and HIV-negative individuals: a pilot study in HIV-positive individuals with oropharyngeal candidiasis.

Current data suggest that T-helper (Th)2-type cytokine responses are often associated with progression to AIDS in HIV-positive individuals. Similarly, Th2-type cytokines are associated with susceptibility to mucosal candidiasis, of which oropharyngeal candidiasis (OPC) is one of the most common opportunistic infections in HIV-positive individuals. Although little information is available on host defense mechanisms at the level of the oral mucosa, recent studies suggest that local cell-mediated immunity (CMI) is equally or more important than that in the periphery for host defense against mucos

HIV and Low Vision: A Narrative Review.

Human Immunodeficiency Virus (HIV) continues to be a significant global public health concern, affecting over 38 million individuals globally. Although the introduction of Highly Active Antiretroviral Therapy (HAART) has led to a significant decline in HIV-related mortality and opportunistic infections, the ocular manifestations of HIV remain a major contributor to morbidity, particularly in low-resource settings. Among the spectrum of visual complications, low vision is an under-recognized yet profoundly disabling consequence that adversely impacts quality of life, independence, and mental he

HIV-1 Nef activates proviral DNA transcription by recruiting Src kinase to phosphorylate host protein Nef-associated factor 1 to compromise its viral restrictive function.

UNLABELLED: HIV-1 accessory protein Nef is a multifunctional pathogenic factor that mediates immune evasion, enhances virion infectivity, antagonizes host restrictive factors, and promotes viral dissemination. However, the modulation of Nef on proviral DNA transcription of latently infected viruses is not well understood. In this study, we found that Nef activated HIV-1 proviral DNA transcription by recruiting Src Family Kinases (SFKs) member Src to stimulate the downstream PI3K/AKT/mTOCR1/CDK9 cellular pathway, and that Naf1 (Nef-associated factor 1), a host protein that is known to suppress
