Particularities of hand and wrist complex injuries in polytrauma management.

Hand and wrist lesions are relatively common in polytraumatised patients. These subjects sustain a wide range of potential life-threatening conditions and hand and wrist injuries incurred are often not diagnosed or are insufficiently treated. Closed lesions are the most frequently missed diagnosis, but even severe open lesions may be incorrectly treated. Most of these hand and wrist injuries can have a strong negative impact on long-term quality of life, particularly when treatment of these injuries is poor or delayed. Orthopaedic and hand surgeons should be vigilant in their assessment and tr

3D-Printed Metacarpal Prosthesis in the Treatment of Primary Osteosarcoma of the First Metacarpal: A Novel Surgical Technique.

OBJECTIVE: Osteosarcoma at the first metacarpal is extremely rare. Reconstructing the metacarpal after tumor resection is essential, as the thumb accounts for approximately 40%-50% of hand function. Although autografts, arthroplasty, and transposition have been reported as reconstruction options, their use is limited by complications such as secondary injury, nonunion, and displacement. In this study, we present a case of a patient with first metacarpal osteosarcoma who underwent tumor resection followed by reconstruction with a 3D-printed metacarpal prosthesis. We tend to introduce a novel st

Bilateral dorsal perilunate dislocation of wrist.

We present a case of simultaneous dorsal perilunate dislocation of both wrists, without a history of fall on outstretched hands. In contrast, it appeared that the mechanism was reverse. His hands were held in radial deviation with wrists in full palmar flexion. The forearms were in neutral position and elbows in mid-flexion. The wrists were suddenly and forcibly pronated. The radiographs of both wrists showed dorsal perilunate dislocation with avulsion fracture of the tip of ulnar styloid process and avulsion fracture of posterior horn of lunate. Radial translation of the carpal bones was also

[Diabetic osteoarthropathy].

It is well-known that osteoarthropathy is one of common problems in diabetic patients. Limitation of joint mobility and painful disorder associated with inflammation of tendons and sheaths are more frequently observed on hands and shoulders. Most of these complications are not specific for diabetes mellitus, but more often in diabetic populations than in non-diabetic populations. On the other hand, destructive bone change due to diabetic polyneuropathy are commonly observed on lower extremities. To control neuroarthropathy is important for prevention of the following diabetic ulcer and gangren

Deforming chondrosarcoma of the fingers secondary to a long-term enchondromatosis of 28 years.

Enchondromatosis of the hand in the context of the Oilier disease is an infrequent pathologie associated with a high rate of malignancy. In this work, we present a case with multiple enchondromas affecting mainly the fourth and fifth rays of the hand, and with multiple ipsilateral lesions in the foot, sacroiliac joint and iliac bone. This patient is, in addition, an unusual case for a long-term progression (28 years), serious deforming appearance and for the double malignant degeneration to chondrosarcoma of the affected fingers. Clinical features, tumor factors, criterion of diagnosis and pro

Intraosseous nerve transposition for treatment of painful neuromas.

Symptomatic painful neuromas of the sensory nerves are a frequent cause of major impairment of the hand. Numerous techniques and measures have been used for managing this challenging problem. Boldrey first advocated a technique in 1943 in which the nerve end was transposed and implanted into bone and reported the successful use of this method in one patient. In 1981, Hemmy used a similar technique in five patients with no recurrence of pain. To date, no other studies that used this method have been reported. We performed intraosseous transposition of 23 painful neuromas in the hand of 11 patie

Epithelioid sarcoma of the hand and wrist: patterns of extension.

The sites of local and remote extension of primary epithelioid sarcoma of the hand and wrist were studied in six patients. The tumor was found to spread to noncontiguous areas of skin, subcutaneous tissue, fascia, bone, and lymph nodes as well as by direct extension along fascial planes. Neither local nor remote extension of the tumor could be determined accurately or dependably by clinical examination, radiography, xerography, lymphangiography or radioisotope scanning. Wide en bloc excision of the tumor or amputation is recommended as the primary treatment of epithelioid sarcoma of the hand.

High-resolution (18)F-FDG PET/CT for assessing disease activity in rheumatoid and psoriatic arthritis: findings of a prospective pilot study.

OBJECTIVE: Rheumatoid arthritis (RA) and psoriatic arthritis (PsA) commonly affect the small joints of the wrist and hand. We evaluated the performance of a new, high-resolution extremity positron emission tomography (PET)/CT scanner for characterizing and quantifying pathologies associated with the two arthritides in the wrist and hand joints. METHODS: Patients with RA or PsA underwent fluorine-18 fludeoxyglucose ((18)F-FDG) PET/CT wrist and hand imaging, respectively, on the high-resolution scanner. Calibrated CT images and co-registered PET images were reconstructed. PET/CT was derived for

Non-union fractures of the scaphoid and carpal bones in children: surgical treatment.

Carpal bone fractures in children are rare. This study reports a series of 16 cases: 12 fractures of the scaphoid (11 non-union), two of the trapezium, one (non-union) of the hamatum, and one of the triquetrum. Only three of these fractures (triquetrum, trapezium and hamatum) were seen at an early stage. The non-union cases involving the scaphoid and hamatum were treated by bone graft. The scaphoid fracture seen early and the triquetrum fracture were treated orthopaedically. One trapezium fracture was treated by osteosynthesis with wires, and the other (seen at the defective callus stage) was

The evolving role of wrist arthroscopy.

Wrist arthroscopy is a valuable tool in hand surgery. However, as with every tool, its use must be executed by the right hands with the right training and right instrumentation. In this article, four papers on arthroscopy were examined for their contribution to the literature and to reflect on the evolving role of arthroscopy in hand surgery. The dorsal sensory branch of the ulnar nerve remains vulnerable with the placements of ulnar portals. Building from previous work, a 'nerve density' concept was presented, as it relates to wrist pronation and supination. The advent of arthroscopy has allo

Bilateral trapezium and multiple metacarpal base fractures: An uncommon case report.

Although fractures of the trapezium are extremely rare, the bone plays a crucial role in grip and pinch movements of the hand. These fractures may be associated with a concomitant fracture of the first metacarpal base and dislocation of the carpometacarpal joint. A 19-year-old male presented with bilateral hand and wrist pain following a motorcycle accident. Imaging revealed a comminuted Walker type V trapezium fracture and a second metacarpal base fracture on the right side and a non-displaced Walker type IV trapezium fracture with second, third, and fourth metacarpal base fractures on the le

Low metacarpal index predicts hip fracture: a prospective population study of 3,561 subjects with 15 years of follow-up.

BACKGROUND: Metacarpal index (MCI), measured from hand radiographs as the ratio between combined cortical thickness and bone diameter, has been suggested for assessment of bone mass and risk of osteoporotic fracture. We studied MCI for its ability to predict hip fractures. METHODS: Hand radiographs were taken and MCI determined in 3,561 subjects from a representative population sample of 8,000 Finns who were 30 years of age or over in 1978-80. Record linkage to the National Hospital Discharge Register identified 117 subjects who had been hospitalized for primary treatment of hip fracture by t

[Osteoporosis and intake of carbohydrates].

Adequate energy intake including carbohydrates is essential to maintain bone mass. Emaciation along with deficiency in nutrients, such as calcium, vitamin D, and protein is a significant risk factor for bone loss, and should be avoided. However, there is no clinical evidence that shows the direct effects of carbohydrate on bone mass. On the other hand, excessive intake of carbohydrates results in obesity, which causes other metabolic diseases such as diabetes mellitus (DM). Therefore, dietary regimen must be balanced in general, and complications and conditions of individual patients should be

Intrinsic and Extrinsic Methods for Augmenting Fracture Healing in the Hand and Wrist.

This review examines intrinsic and extrinsic augmentation techniques for uniting hand and upper extremity fractures, including bone morphogenic proteins (BMPs), platelet-rich plasma (PRP), low-intensity pulsed ultrasound (LIPUS), and pulsed electromagnetic fields (PEMF). While BMPs, PRP, LIPUS, and PEMF show potential in accelerating bone healing and reducing nonunion rates, their clinical adoption is limited by high costs and inconsistent results. This paper focuses on understanding the efficacy of these techniques, their drawbacks, and potential next steps for the field.

[Bilateral periostitis and arthritis: about a case of hypertrophic osteoarthropathy].

We report the case of a 67-year-old female patient presenting swelling of the hands and feet and pain in both legs. Clinical examination and bone scintigraphy identify the triad "digital clubbing - arthritis - bilateral periostitis of the long bones", leading to a diagnosis of hypertrophic osteoarthropathy, a syndrome usually associated with pulmonary neoplasia. The thoracic CT-scan, followed by a biopsy, effectively diagnosed a right upper lobe adenocarcinoma. Surgical treatment of the neoplasia allowed the resolution of the clinical complaints and the pathological scintigraphic findings.

Anatomic Locking Plate Fixation for Scaphoid Nonunion.

Nonunion can occur relatively frequently after scaphoid fracture and appears to be associated with severity of injury. There have been a number of techniques described for bone grafting with or without screw fixation to facilitate fracture healing. However, even with operative fixation of scaphoid fractures with bone grafting nonunion or malunion rates of 5 to 10 percent are still reported. This is the first report of an anatomic locking plate for scaphoid fracture repair in a 25-year-old right hand dominant healthy male.

Altered growth and branching patterns in synpolydactyly caused by mutations in HOXD13.

Hox genes regulate patterning during limb development. It is believed that they function in the determination of the timing and extent of local growth rates. Here, it is demonstrated that synpolydactyly, an inherited human abnormality of the hands and feet, is caused by expansions of a polyalanine stretch in the amino-terminal region of HOXD13. The homozygous phenotype includes the transformation of metacarpal and metatarsal bones to short carpal- and tarsal-like bones. The mutations identify the polyalanine stretch outside of the DNA binding domain of HOXD13 as a region necessary for proper p
