Use of Suture as an Alternative to Stainless Steel Wire for Stabilisation of Open Cap Splint in Paediatric Mandibular Fractures.

The presence of ongoing mandibular growth and tooth buds make it challenging for the treatment of mandibular fracture in paediatric. Most common and widely used method is the use of occlusion splint which are secured with the help of circumandibular wiring. The aim of this technical note was to describe a new method for the treatment of paediatric mandibular fracture in which the occlusion splint was used and stabilized with the help of suture so as to prevent minimal trauma to the soft tissue. Management of paediatric mandibular fracture is challenging and is different then that of adult. Usu

[The effects of midazolam on the general circulation, cerebral blood-flow and cerebral oxygen consumption in man (author's transl)].

The effects of midazolam, 0.2 mg/kg b.w., on cerebral blood flow, cerebral oxygen consumption and brain metabolism were studied in 8 patients scheduled for intracranial aneurysm surgery. All patients were anesthetized with fentanyl and N2O; arterial pCO2 and pO2 were maintained within the normal range by controlled ventilation. Midazolam produced minimal effects on the general circulation. Cerebral blood-flow decreased about 11% while cerebral oxygen consumption essentially remained unchanged. Cerebral perfusion pressure slightly decreased due to a reduction in mean aortic pressure. In three p

Effectiveness of Left Atrial Appendage Exclusion Procedures to Reduce the Risk of Stroke: A Systematic Review of the Evidence.

BACKGROUND: Atrial fibrillation is an important cause of cardioembolic stroke. Oral anticoagulants (OAC) reduce stroke risk but increase the risk of serious bleeding. Left atrial appendage (LAA) procedures have been developed to isolate the LAA from circulating blood flow, as an alternative to OAC. We conducted a systematic review of the benefits and harms of surgical and percutaneous LAA exclusion procedures. METHODS AND RESULTS: We searched multiple data sources, including Ovid MEDLINE, Cochrane, and Embase, through January 7, 2015. Of 2567 citations, 20 primary studies met prespecified inc

Who uses legal cannabis and why? Cluster profiles of participants in a Swiss regulated sales pilot trial.

BACKGROUND: As cannabis regulation expands, it is important to understand who enrolls in legal non-medical cannabis programs and how partici … risk awareness suggests that information and perception alone may not reduce harm. Profile-tailored harm reduction and integrated support may be needed within regulated cannabis frameworks.

Risk Management Activities in a Lung Cancer Multidisciplinary Team at a Comprehensive Cancer Center: Results of a Prospective Analysis.

PURPOSE: The objective of the study was to highlight sources of harm that could negatively affect the lung cancer multidisciplinary team (MDT) activities to reduce the level of risk of each factor. METHODS: A modified Delphi approach was used by a board of multi-health care professionals of the lung cancer MDT to identify the main processes, subprocesses, and risk factors of the multidisciplinary pathway of patients with lung cancer. A semiquantitative matrix was built with a five-point scale for probability of harm (likelihood) and severity of harm (consequences) according to the internation

Impact of postnatal corticosteroid use on neurodevelopment at 18 to 22 months' adjusted age: effects of dose, timing, and risk of bronchopulmonary dysplasia in extremely low birth weight infants.

OBJECTIVE: Postnatal steroid use decreases lung inflammation but increases impairment. We hypothesized that increased dose is associated with increased neurodevelopmental impairment, lower postmenstrual age at exposure increases impairment, and risk of bronchopulmonary dysplasia modifies the effect of postnatal corticosteroid. METHODS: Steroid dose and timing of exposure beyond 7 days was assessed among 2358 extremely low birth weight infants nested in a prospective trial, with 1667 (84%) survivors examined at 18 to 22 months' postmenstrual age. Logistic regression tested the relationship bet

Pediatric platelet transfusions in critical illness: a narrative review of recent studies.

PURPOSE OF REVIEW: We sought to describe the current use of platelet transfusions, harms associated with platelet transfusion, new methods of platelet processing that attempt to address these harms, and recent platelet transfusion guidance specific to critically ill children. RECENT FINDINGS: Platelet transfusions have been associated with significant morbidity and mortality. New processing techniques, such as pathogen reduction, have been developed to combat infectious risks but in a recent trial of platelet transfusion thresholds in neonates, transfusing platelets more liberally was associa

Dysplasia and anomalies of atlas result in pediatric torticollis: A case report and literature review.

BACKGROUND: Often, the cause of bony torticollis is difficult to determine, especially in cases of multiple craniovertebral junction anomalies. CASE DESCRIPTION: We report a rare case of a dysplastic C1 vertebra (assimilation to the right occiput and C2, a nonseparated left odontoid, and discontinuity in both anterior and posterior arches of the atlas) in a 6-year-old child with progressive torticollis. Notably, the mechanism of torticollis was not a rotatory subluxation of C1-C2, but differential growth between C1-C2. The child underwent a successful C1-C2 Goel and Harms fusion with reductio

Integrated residential treatment for persons with severe and persistent mental illness: lessons in recovery.

This retrospective study examines 24-month outcomes for 38 participants with histories of chronic homelessness and hospitalizations in an urban, residential integrated treatment (IT) program and compares characteristics of those who stayed in the program 24 months with those who left within their first year of residence. Informed by an Assertive Community Treatment approach, characterized by outreach (or what might better be referred to as inreach), low staff to consumer ratio, and meeting of basic needs, the residential program emphasized harm reduction and motivational interventions. The lon…

GLP-1 receptor agonists and serious adverse events: Evidence, uncertainty and clinical implications.

Glucagon-like peptide-1 (GLP-1) receptor agonists provide benefits in weight reduction, glycaemic control in Type 2 diabetes, and cardiovascular and renal outcomes. Their widespread use has given rise to concerns regarding rare but potentially serious adverse events. This narrative review appraises evidence regarding seven serious adverse events linked to GLP-1 receptor agonists: thyroid cancer, non-arteritic anterior ischaemic optic neuropathy (NAION), suicidal behaviours and self-harm, diabetic retinopathy complications, acute pancreatitis, gallbladder and biliary disease and intestinal obst

Results of a workshop intervention trial to reduce HIV and intersectional stigma among sexual and gender minorities and healthcare workers in the Dominican Republic.

Eliminating stigmas that harm sexual and gender minorities (SGM) with HIV is necessary to end the epidemic; however, few validated stigma-reducing interventions for healthcare settings exist, and even fewer have been evaluated in Spanish-speaking contexts. Finding Respect and Ending Stigma around HIV (FRESH) is a healthcare setting stigma-reduction intervention that was adapted and tested to address HIV and intersectional stigmas that harm SGM with HIV in the Dominican Republic. Herein, we detail the intervention adaptation process and present feasibility, acceptability, and pre-post prelimina

DSM-5 Non-Suicidal Self-Injury Disorder in a Clinical Sample of Adolescents with Recurrent Self-Harm Behavior.

INTRODUCTION: Non-suicidal self-injury disorder (NSSID) is a new diagnosis proposed in DSM-5 with a need of further study, especially in adolescent clinical populations where non-suicidal self-injury (NSSI) is particularly prevalent. We aimed to study characteristics of NSSID and estimate an optimal cutoff frequency level of NSSI behavior. METHODS: Data were collected from 103 outpatient adolescents (ages 12-18) with recurrent self-harm behavior. RESULTS: Adolescents with NSSID reported significantly more frequent NSSI behavior and suicide attempts than adolescents without NSSID. Frequency o

Justice Core: An Innovative Response to Incarceration During Pregnancy in North Carolina.

Incarceration during pregnancy is a pressing public health issue, but interventions to minimize harms for this population are not frequently described in the literature. We developed Justice Core, a pretrial diversion intervention for pregnant people with substance use disorders (SUDs), to minimize exposure to incarceration during pregnancy and increase access to SUD treatment. The intervention was feasible and acceptable to referring jail staff. In parallel, we conducted a needs assessment and identified an estimated 1,220 to 1,461 annual incarcerations during pregnancy among 77 responding ja
