The value of elbow arthroscopy in diagnosing and treatment of radial head fractures.

BACKGROUND: Surgical treatment of radial head fractures is increasingly performed arthroscopically. These fractures often feature concomitant injuries to the elbow joint, which may be under-diagnosed in the radiological examinations. Little is known about the diagnostic value of arthroscopy, the treatment options that arise from arthroscopically assisted fracture fixation and clinical results. We hypothesized that arthroscopy can detect additional concomitant injuries and simultaneously expands the therapeutic options. Therefore aim of this study was to compare arthroscopic and radiologic find

Comprehensive Elbow Joint Morphology Changes in Surgically Treated Osteochondritis Dissecans of the Capitellum: Clinical Implications.

BACKGROUND: Capitellar osteochondritis dissecans (OCD) frequently results in radial head enlargement. However, due to the elbow joint's complex anatomy, deformities likely extend beyond just the radial head. HYPOTHESIS/PURPOSE: It was hypothesized that in patients with OCD treated with surgery, imbalances in bone morphology during growth would affect the entire joint, leading to postoperative clinical symptoms. The purpose of this study was to test this hypothesis through a 3-dimensional evaluation of morphological changes. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS:

Dermal Allograft and Biceps Superior Capsule Reconstruction for Massive Irreparable Rotator Cuff Tears.

Since the use of autologous fascia lata for superior capsule reconstruction of massive irreparable rotator cuff tears (MIRCTs), the technique has evolved into various modifications, including dermal allografts, long head of the biceps tendon (LHBT), and combinations of both, which will be discussed in this article. After making sure the remnant cuff cannot be restored to the anatomical footprint of the supraspinatus, a double-loaded or triple-loaded, suture-based anchor is inserted 5-8 mm posterior to the bicipital groove to secure the long head of the biceps (LHBT) initially. A bone trough is

Computed Tomography Does Not Improve Intra- and Interobserver Agreement of Hertel Radiographic Prognostic Criteria.

Background and Objectives: Proximal humerus fractures are the second most frequent site of avascular necrosis (AVN), occurring in up to 16% … ed only in some moments of the second round of evaluation. Conclusions: The prognostic criteria for humeral head ischemia evaluated in this study showed weak intra- and interobserver agreement in both the radiographic and tomographic evaluation. CT did not help surgeons in the primary interpretation of Hertel prognostic criteria used in this study when compare …

Intelligent anatomic design of porous radial head prosthesis and microscopic-macro biomechanical finite element analysis in the long-term after replacement surgery.

BACKGROUND: The microporous structure of porous titanium alloy may affect osteoblast differentiation and reduce effective elastic modulus (EEM) of the prostheses. Therefore, the biomechanics-based anatomic design of porous radial head prosthesis (PRHP) may help promote bone healing and reduce postoperative complications. METHODS: A microscopic-macro virtual testing platform (VTP) was built to design cells with excellent mechanical properties, further, to construct the PRHP. An intelligent anatomic platform of healthy human elbow-forearms was developed to construct finite element (FE) models o

How bone quality may influence intraoperative and early postoperative problems after angular stable open reduction-internal fixation of proximal humeral fractures.

BACKGROUND: With the introduction of the deltoid tuberosity index (DTI), a simple radiographic tool has become available to measure bone mineral density of the proximal humerus. The aim of this study was to assess the influence of local bone mineral density on the early failure rate after angular stable open reduction-internal fixation of proximal humeral fractures (PHFs). METHODS: We retrospectively followed up all patients treated with angular stable implants for PHFs from 2007 to 2014. The fractures were classified according to Neer, and the DTI, metaphyseal head extension (MHE), medial hi

Management of complex elbow instability.

Complex elbow instability is a challenging injury even for expert elbow surgeons. The preoperative radiographs should be carefully evaluated to recognize all lesions that may occur in complex elbow instabilities. Recognizing all the possible lesions is critical to achieve an optimal outcome. The most common types of injuries are as follows: (1) radial head fractures associated with lateral and medial collateral ligaments lesions (with or without elbow dislocation); (2) Coronoid fractures and lateral collateral ligament lesion (with or without elbow dislocation); (3) Terrible Triad; (4) Transol

Rotator Cuff Arthropathy: A Comprehensive Review.

Rotator cuff arthropathy is a spectrum of disease states secondary to full-thickness cuff tears classified by rotator cuff insufficiency and degenerative disease within the shoulder joint. Diagnosis can be made through standard physical exam and radiographic films demonstrating varying levels of weakness, along with acetabularization, femoralization, and superior migration of the humeral head. Severity of disease is classified through both the Hamada and Seebauer grading systems, which are used clinically to determine the appropriate treatment algorithm. Treatment exists along the spectrum fro…

Biological aspects of crosses between populations of Triatoma mexicana (Hemiptera: Reduviidae: Triatominae).

We examined the degree of reproductive isolation in four populations of Triatoma mexicana Herrich-Schaeffer from Mexico along with fertility and the segregation of morphological characteristics in two generations of offspring from crosses between these populations. The percentage of couples with (fertile) offspring was high among all sets of crosses between cohorts from Peñamiller, Meztitlán, and Orizabita. It was low in crosses involving a cohort from Tierra Blanca, mainly in crosses with Meztitlán. Among sets of crosses involving Tierra Blanca specimens, whole first-generation (F1) individua

Editorial Commentary: Increased Graft Thickness With Superior Capsular Reconstruction Results in Improved Acromiohumeral Distance, but Increased Graft Tear Rate and Lateral Acromial Erosion.

For the treatment of irreparable posterosuperior rotator cuff tears of the supraspinatus and infraspinatus, superior capsular reconstruction … has been a renewed interest in increasing the thickness of an SCR graft to further prevent superior humeral head translation and diminish subacromial contact pressures and secondary radiographic, or more importantly, clinical failure. Some authors have sought to augment dermal allografts for increased stiffness, whereas other authors have explored the option o …
