Analysis of Graft Length Change Patterns in Medial Patellofemoral Ligament Reconstruction via a Fluoroscopic Guidance Method.

BACKGROUND: A fluoroscopic guidance method for medial patellofemoral ligament (MPFL) reconstruction has been widely used to determine the anatomic femoral attachment site. PURPOSE: To examine the graft length change patterns in MPFL reconstruction with a fluoroscopic guidance method. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: Forty-four knees of 42 patients who underwent MPFL reconstruction for the treatment of recurrent patellar dislocation were examined prospectively. During surgery, suture anchors were inserted into the proximal one-third and center of the patell

Proximal tibiofibular dislocation accompanied by bucket-handle meniscal tear: Case report.

Proximal tibiofibular dislocation (PTFD) is a rare injury. In this article, we report a case of a PTFD, complicated with an intraarticular injury, a bucket-handle tear of the lateral meniscus. A 37-year-old male patient presented to the emergency department with pain and swelling on the lateral side of his right knee due to lateral trauma to the fibula during external rotation of the leg while his foot was on the ground. Closed reduction was performed under sedation. The lateral meniscus was repaired, and the fibular head was fixed with a cannulated screw. The patient had no pain in the tibiof

Further insight into the phenotype associated with a mutation in the ORC6 gene, causing Meier-Gorlin syndrome 3.

Mutations in genes encoding the origin recognition complex subunits cause Meier-Gorlin syndrome. The disease manifests a triad of short stature, small ears, and small and/or absent patellae with variable expressivity. We report on the identification of a homozygous deleterious mutation in the ORC6 gene in previously described fetuses at the severe end of the Meier-Gorlin spectrum. The phenotype included severe intrauterine growth retardation, dislocation of knees, gracile bones, clubfeet, and small mandible and chest. To date, the clinical presentation of ORC6-associated Meier-Gorlin syndrome

Medial patellofemoral ligament reconstruction with a looped semitendinosus tendon, using knotless anchor fixation on the patella and hybrid fixation on the femur.

Medial patellofemoral ligament (MPFL) reconstruction is a reliable surgical method for stabilizing a dislocating patella, with multiple techniques previously described. Although outcomes are generally favorable, the procedure is technically demanding and relies on precise identification of native MPFL insertion sites, secure fixation of the graft to these sites, and appropriate graft tension. We describe a technique for MPFL reconstruction with a looped semitendinosus tendon. The 2 free limbs of the graft are secured into blind-end patellar sockets with knotless anchors, and the looped end is

Patellectomy: an overview with reconstructive procedures.

Patellectomy is a relatively old procedure and its effect on knee function has been a matter of controversy. Although indications for a patellectomy have been narrowed, patellectomy still is indicated as the last choice of treatment in certain situations including comminuted fractures, advanced chondromalacia or osteoarthritis, dislocations, infections, and tumoral conditions. Excision of the patella results in a decrease in the moment arm of the extensor mechanism, alteration of the forces acting on the tibiofemoral joint and instant center of motion, limitation of range of motion, anterior i

In-line skating injuries. Epidemiology and recommendations for prevention.

In 1993 there was an estimated 12.6 million in-line skaters in the US. In-line skating is popular because of its affordability and its exercise and recreational value. The main risk factors for injury include speed, obstacles and hard surfaces. Using the National Electronic Injury Surveillance System in US hospitals, 31,000 skaters were reported injured over a 12 month period. Fractures, dislocations, sprains, strains and avulsion made up 67% of all injuries. It is recommended that skaters wear protection equipment including, helmet, wrist guards, knee-pads and elbow-pads. Although head injuri

[Hip diseases in children].

In the orthopedic practice, the hip joint is of special importance at all ages. For the general practitioner and the pediatrician are those disorders of the hip of high importance, for which early diagnosis and initiation of treatment is critical for prognosis. The following disorders should be looked for in priority, if symptoms indicate a possible affection of hip: congenital dysplasia and dislocation of the hip, coxitis, slipped capital femoral epiphysis, and tumors. One should always be aware that especially in children disease of the hip need not necessarily present with symptoms directly

[Lesions of the acetabular rim. Clinical manifestations, therapeutic strategies and perspectives].

Partial rupture of the acetabular rim have usually been described as accompanying traumatic or dysplasic dislocations of the hip. The five cases reported here are in favor of a specific pathology of this fibrocartilage as it is in the knee or the shoulder. Besides, the functional symptoms are not without reminding that of these two joints with sudden onset of cracks, jerks, blockings and even a real joint instability. The pain is never isolated, but may accompany other disorders. The cases were selected in order to evaluate the clinical manifestations and the therapeutic consequences of such a

Lateral release and proximal realignment for patellar subluxation and dislocation. A long-term follow-up.

Fifty-two patients (sixty knees) who had a diagnosis of either patellar subluxation or dislocation had an operation consisting of a lateral release and proximal realignment of the patella. The duration of follow-up ranged from two to nine years. At the latest follow-up, forty-two (80.8 per cent) of the patients had a good or an excellent clinical result; only one patient had redislocation of the patella. The results were not affected by the grade of chondromalacia that was present at the time of operation; however, the differences in the results as correlated with the age and sex of the patien

Perinatal fractures in arthrogryposis multiplex congenita.

Thirty-six patients with arthrogryposis have been followed at The James Lawrence Kernan Hospital of Baltimore since 1951. In this group, nine patients sustained 16 perinatal fractures. Femoral shaft fractures and epiphyseal separations were found more commonly in patients with fixed knee extension or dislocated hips and were related to relative stiffness and obstructed delivery. Femoral fractures were more common than humeral or tibial fractures. Arthrogrypotic infants displaying irritability and poor feeding are suspect of skeletal trauma. Thickening of limbs or joints may be the only clinica

Anatomic Multiligament Knee Reconstruction With Biceps Femoris Tendon and Medial Meniscal Repair.

BACKGROUND: Multiligament knee injuries are defined as tears of ≥2 of the following ligament structures: anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), posteromedial corner, or posterolateral corner (PLC). These injuries occur in various settings and may be associated with knee dislocations. INDICATIONS: The literature has reported that anatomic technique and a single-;stage approach for treating multiligament knee injuries are optimal because they may lead to improved outcomes. An anatomic double-;bundle PCL reconstruction is suggested to improve knee kinematics compare…

[Patellar position and lateral approach for total knee arthroplasty in degenerative knees with lateral femoropatellar arthrosis].

PURPOSE OF THE STUDY: Patellar malposition is a well-recognized patellar complication after total knee arthroplasty. Such residual malposition is particularly frequent when the knee presents lateral femoropatellar arthrosis. We compared the radiological position of the patella after total knee arthroplasty in degenerative knees with lateral femoropatellar arthrosis performed via a medial or lateral approach with elevation of the anterior tibial tuberosity. MATERIAL AND METHODS: Twenty-six total knee arthroplasties were reviewed retrospectively. Thirteen prostheses had been inserted via a medi

IMPAD1 mutations in two Catel-Manzke like patients.

Catel-Manzke syndrome is characterized by hyperphalangism with bilateral deviation of the index fingers and micrognathia with or without cleft palate. Some atypical patients present with additional malformations. No molecular basis is yet available. Most patients have an unremarkable family history but autosomal recessive inheritance has been recently suggested in a consanguineous family with recurrence in sibs. Catel-Manzke syndrome has overlapping features with Desbuquois dysplasia type 1 due to CANT1 (calcium-activated nucleotidase 1) mutations and also with "chondrodysplasia with joint dis
