MR imaging of bladder neoplasms: correlation with pathologic staging.

This retrospective study describes the accuracy of magnetic resonance (MR) imaging in determination of local disease extent in 34 examinations performed for evaluation of primary or recurrent bladder carcinoma. The 34 examinations were carried out in 29 patients (17 men, 12 women, aged 34-89 years). All studies were followed within 30 days by surgical and pathologic evaluation. Images were interpreted by two radiologists unaware of clinical or pathologic findings. MR imaging staging conformed to the TNM system of the International Union Against Cancer (IUAC). Pathologic evaluation showed bladd

Opportunities to Avoid Invasive Cancer by Diagnosis and Interception of Preneoplastic Lesions and Cancer Risk Conditions.

BACKGROUND: Preneoplastic lesions (PNL) represent a state intermediate between normal cells and invasive cancer. Individuals diagnosed with PNL are at increased risk of developing invasive cancer and represent a unique risk group. We estimated the proportion of invasive cancer that could be intercepted at multiple anatomic sites in individuals with PNL. METHODS: Using a literature survey of the prevalence, latency, and probability of progression of PNL to invasive cancer, we undertook a sensitivity analysis across proportions of invasive cancer from 5% to 95% that could be intercepted in a co

Clinicopathological and prognostic significance of the microcystic elongated and fragmented pattern in endometrial cancer: a systematic review and meta-analysis.

OBJECTIVE: The presence of the microcystic elongated and fragmented (MELF) pattern, distinguished by its microcystic, elongated and fragmented attributes, constitutes a common manifestation of myometrial invasion (MI) within endometrial carcinoma. However, the prognostic significance of this pattern has not been definitively established. Consequently, this research aimed to clarify the prognostic implications of the MELF pattern for individuals diagnosed with endometrial carcinoma. DESIGN: Systematic review and meta-analysis of observational clinical studies. DATA SOURCES: An extensive revie

Imaging Assessment of Prostate Cancer Extra-prostatic Extension: From Histology to Controversies.

Prostate cancer (PCa) is the most common non-skin malignancy among men and the fourth leading cause of cancer-related deaths globally. Accurate staging of PCa, particularly the assessment of extra-prostatic extension (EPE), is critical for prognosis and treatment planning. EPE, typically evaluated using magnetic resonance imaging (MRI), is associated with higher risks of positive surgical margins, biochemical recurrence, metastasis, and reduced overall survival. Despite the widespread use of MRI, there is no consensus on diagnosing EPE via imaging. There are 2 main scores assessing EPE by MRI:

[Muscle-invasive and metastatic urothelial carcinoma from a pathological point of view].

Muscle-invasive and metastatic urothelial carcinoma is a heterogeneous disease with a broad morphological and molecular spectrum. The amendment of the WHO classification has resulted in some changes in the nomenclature and classification of muscle-invasive and metastatic urothelial carcinomas. Due to the increasing individualisation of therapeutic options, the correct diagnosis of morphological variants of urothelial carcinoma, which are associated with specific molecular alterations, is becoming more and more important. The morphological variants also correlate with molecular subtypes of urot

Periostin+ Stromal Cells Guide Lymphovascular Invasion by Cancer Cells.

UNLABELLED: Cancer cell dissemination to sentinel lymph nodes is associated with poor patient outcomes, particularly in breast cancer. The process by which cancer cells egress from the primary tumor upon interfacing with the lymphatic vasculature is complex and driven by dynamic interactions between cancer cells and stromal cells, including cancer-associated fibroblasts (CAF). The matricellular protein periostin can distinguish CAF subtypes in breast cancer and is associated with increased desmoplasia and disease recurrence in patients. However, as periostin is secreted, periostin-expressing C

Characteristics of local extension based on tumor distribution in nasopharyngeal carcinoma and proposed clinical target volume delineation.

OBJECTIVES: To summarize the characteristics of local extension of eccentric and central nasopharyngeal carcinoma (NPC) by magnetic resonance imaging (MRI) and to improve clinical target volume (CTV) delineation. METHODS: MRI of 870 newly diagnosed NPC patients were reviewed. According to tumor distribution features, the NPCs were divided into eccentric and central lesions. RESULTS: All local invasions presented as continuous invasion from gross lesions and structures adjacent to the nasopharynx were more likely to be invaded. There were 240 (27.6%) and 630 (72.4%) cases with central and ecc

Measuring depth of invasion of submucosa - invasive adenocarcinoma in oesophageal endoscopic specimens: how good are we?

AIMS: Emerging data support that submucosa-invasive (pT1b) esophageal adenocarcinomas are cured via endoscopic resection, provided that invasion measures ≤500 μm, they lack other histological features predictive of nodal metastasis and have negative margins. Hence, pathologists' measurement of the depth of submucosal invasion in endoscopic resections may dictate further management (i.e. endoscopic follow-up versus oesophagectomy). In this study, we assessed the interobserver agreement in measuring the depth of submucosal invasion in oesophageal endoscopic resections. METHODS AND RESULTS: Six

Advanced Penile Cancer Presenting With Renal Failure.

"MK," a man aged 67 years, presented with fatigue and nausea to his primary care physician. Routine blood tests showed a stage III acute kidney injury and the patient was admitted urgently into the hospital for investigation. On initial assessment by the nursing staff, with urine dip and bladder scan, he was found to have a large fungating mass on his penis. Upon further questioning, MK reported having had trouble with urination for a number of years and that he had been concealing the mass for at least 1 year due to embarrassment. He required a suprapubic catheter as the mass had completely o

Coexistence of benign phyllodes tumor and invasive ductal carcinoma in distinct breasts: case report.

This report describes a rare case of coexistence of benign phyllodes tumor, which measured 9 cm in the right breast, and invasive ductal carcinoma of 6 cm in the left breast, synchronous and independent, in a 66-year-old patient. The patient underwent a bilateral mastectomy due to the size of both lesions. Such situations are rare and usually refer to the occurrence of ductal or lobular carcinoma in situ when associated with malignant phyllodes tumors, and more often in ipsilateral breast or intra-lesional.

Lymphoplasmacyte-rich meningioma with atypical invasive nature.

A 12-year-old boy presented with a lymphoplasmacyte-rich (LPR) meningioma in the posterior fossa. The tumor was subtotally removed. Histological examination showed the tumor had invaded the normal brain tissue despite its benign grade in the World Health Organization classification. The Ki-67 staining index using MIB-1 monoclonal antibody was relatively high. (18)F-2-fluoro-2-deoxy-D-glucose positron emission tomography revealed high uptake in the tumor. These findings indicate the atypical nature of LPR meningioma.

Lipoma of the deep lobe of the parotid gland: report of 3 cases.

Since lipomas are common soft-tissue neoplasms in other regions of the body, they are found only rarely in the parotid gland and are, therefore, often not considered in the initial differential diagnosis of parotid gland tumours. Even rarer are lipomas arising in the deep lobe of the parotid gland with only 6 cases presented in the literature. Lipomas originating in the parapharyngeal space are also extremely rare. When there is any suspicion, computed tomography and especially magnetic resonance imaging are of high diagnostic value. Fine-needle aspiration cytology seems not to contribute much

Massive immature mediastinal teratoma extending into the left pleural cavity.

We report a case of a massive mediastinal teratoma in an 18-year-old woman who presented with a short history of exertional dyspnoea. The tumor arose from the left lobe of the thymus and extended into the left pleural cavity, completely compressing the left lung and extensively shifting the mediastinum to the right. Measuring 23 cm x 17 cm x 9 cm and weighing 2005 g it is one of the largest anterior mediastinal teratomas reported. It was successfully treated by surgical resection, with a final pathological diagnosis of a grades 1-2 immature teratoma.
