An experimental assessment of the risk of compartment syndrome during knee arthroscopy.

The purpose of this study was to objectively evaluate the risk of compartment syndrome as a complication during arthroscopy, particularly with the use of mechanical infusion systems. Little experimental data are available. This study shows that when elevated compartment pressures occur, extravasated fluid dissipates quickly, minimizing the risk of compartment syndrome and subsequent neuromuscular damage. Twelve live pig hind limbs (six swine) were used. Three additional limbs were used as shams. After anesthesia and portal placement for mechanical fluid infusion, two standardized capsulotomies

ISCCM Guidelines for the Use of Non-invasive Ventilation in Acute Respiratory Failure in Adult ICUs.

UNLABELLED: A. ACUTE HYPERCAPNIC RESPIRATORY FAILURE A1. Acute Exacerbation of COPD: Recommendations: NIV should be used in management of acute exacerbation of COPD in patients with acute or acute-on-chronic respiratory acidosis (pH = 7.25-7.35). (1A) NIV should be attempted in patients with acute exacerbation of COPD (pH <7.25 & PaCO2 ≥ 45) before initiating invasive mechanical ventilation (IMV) except in patients requiring immediate intubation. (2A). Lower the pH higher the chance of failure of NIV. (2B) NIV should not to be used routinely in normo- or mildly hyper-capneic patients with acut

[Requisite muscle relaxation using vecuronium for tracheobronchial suction in neurosurgical intensive care patients].

Coordination of respiratory care with protection of the brain is critical in neurosurgical intensive care. Therefore, in addition to hyperventilation, adequate sedation and muscle relaxation are applied to mitigate the difficulties with control of intracranial pressure (ICP) during routine tracheobronchial suctioning (TBS). Although hypnotics have been shown to be effective in mitigating increases in ICP in response to endotracheal suctioning in paralyzed patients, brisk bucking and coughing with further increases in ICP may occur without muscle relaxation. Long-term neuromuscular (nm) paralys

Can slow and (un)steady still win the race? A narrative review of developmental gait characteristics and clinical implications in children born preterm.

UNLABELLED: Independent walking is more than a developmental milestone, as it reflects the integration of neuromuscular maturation, postural control, and sensorimotor coordination. In children born preterm, both the timing and the characteristics of independent walking may differ from those observed in term-born peers. This narrative review synthesizes current evidence on independent walking acquisition and gait features in preterm-born children throughout different stages of life, from toddler to school age, and discusses the clinical implications of quantitative gait assessment, with a focus

Scrotal Botulinum Toxin Exposure and Male Fertility: Mechanistic Insights and Clinical Evidence.

INTRODUCTION: Scrotal botulinum toxin type A (BoNT-A) injections ("scrotox") are increasingly used to relax the dartos and/or cremaster muscles for cosmetic purposes and for functional indications such as chronic scrotal pain/orchialgia, exaggerated cremasteric reflex, and cremasteric synkinesia. Evidence remains limited and dispersed across animal toxicology studies, urologic case reports, and pain-management trials. We conducted a systematic review to evaluate whether botulinum toxin exposure involving the scrotum, dartos, cremaster, spermatic cord/peritesticular region, or predefined regula

Prevalence of Hypomagnesemia in ICU Patients at a Tertiary Care Center: A Prospective Observational Study.

BACKGROUND: Magnesium, the fourth most abundant cation in the human body, plays a crucial role in enzymatic activity, neurochemical transmission, and muscular excitability. Hypomagnesemia in critically ill patients has been associated with neuromuscular dysfunction, cardiovascular complications, metabolic disturbances, and increased morbidity. This study aims to determine the prevalence of hypomagnesemia in critically ill patients and evaluate its impact on disease severity, length of ICU stay, ventilator dependency, and mortality. METHODS: This prospective observational study was conducted i

How Informal Carers Support Video Consulting in Physiotherapy, Heart Failure, and Cancer: Qualitative Study Using Linguistic Ethnography.

BACKGROUND: Informal carers play an important role in the everyday care of patients and the delivery of health care services. They aid patients in transportation to and from appointments, and they provide assistance during the appointments (eg, answering questions on the patient's behalf). Video consultations are often seen as a way of providing patients with easier access to care. However, few studies have considered how this affects the role of informal carers and how they are needed to make video consultations safe and feasible. OBJECTIVE: This study aims to identify how informal carers, u

5th National Audit Project (NAP5) on accidental awareness during general anaesthesia: summary of main findings and risk factors.

We present the main findings of the 5th National Audit Project (NAP5) on accidental awareness during general anaesthesia (AAGA). Incidences were estimated using reports of accidental awareness as the numerator, and a parallel national anaesthetic activity survey to provide denominator data. The incidence of certain/probable and possible accidental awareness cases was ~1:19,600 anaesthetics (95% confidence interval 1:16,700-23,450). However, there was considerable variation across subtypes of techniques or subspecialities. The incidence with neuromuscular block (NMB) was ~1:8200 (1:7030-9700),

A comprehensive normative reference database of muscle morphology in typically developing children aged 3-18 years-a cross-sectional ultrasound study.

During childhood, muscle growth is stimulated by a gradual increase in bone length and body mass, as well as by other factors, such as physical activity, nutrition, metabolic, hormonal, and genetic factors. Muscle characteristics, such as muscle volume, anatomical cross-sectional area, and muscle belly length, need to continuously adapt to meet the daily functional demands. Pediatric neurological and neuromuscular disorders, like cerebral palsy and Duchenne muscular dystrophy, are characterized by impaired muscle growth, which requires treatment and close follow-up. Nowadays ultrasonography is

Knee Strength and Ankle Range of Motion Impacts on Timed Function Tests in Duchenne Muscular Dystrophy: In the Era of Glucocorticoids.

BACKGROUND: Duchenne Muscular Dystrophy (DMD) is a neuromuscular disorder that presents in childhood and is characterized by slowly progressive proximal weakness and lower extremity contractures that limit ambulatory ability [1, 2]. Contractures develop in the ankles, knees, and hips due to muscle imbalances, fibrotic changes, loss of strength, and static positioning [2, 5]. Currently, standards of care guidelines emphasize the importance of maintaining good musculoskeletal alignment through stretching, bracing, and glucocorticoid (GC) therapy to preserve strength and function. METHODS: This

Is body composition associated with an increased risk of developing anterior knee pain in adolescent female athletes?

OBJECTIVE: To determine the relationship between relative body composition and body mass to height, anterior knee pain, or patellofemoral pain (PFP) in adolescent female athletes. BACKGROUND: Patellofemoral pain is common in female athletes and has an undefined etiology. The purpose of this study was to examine whether there was an association among higher body mass index (BMI), BMI z-scores, and relative body fat percentage in the development of PFP in an adolescent female athlete population. We hypothesized that female athletes who developed PFP over the course of a competitive basketball s

Single-centre, double-blinded, randomised placebo-controlled trial to determine the effect of a 12-week home-based programme of footplate neuromuscular electrical stimulation on walking capacity in people with peripheral artery disease: a protocol for the Foot-PAD trial.

INTRODUCTION: Patients with peripheral artery disease (PAD) can experience intermittent claudication, which limits walking capacity and the ability to undertake daily activities. While exercise therapy is an established way to improve walking capacity in people with PAD, it is not feasible in all patients. Neuromuscular electrical stimulation (NMES) provides a way to passively induce repeated muscle contractions and has been widely used as a therapy for chronic conditions that limit functional capacity. Preliminary trials in patients with PAD demonstrate that stimulation of the leg muscles usi

Towards Developing Novel Prostate Cancer Recurrence Suppressors: Acute Toxicity of Pseurotin A, an Orally Active PCSK9 Axis-Targeting Small-Molecule in Swiss Albino Mice.

The proprotein convertase subtilisin kexin type 9 (PCSK9) emerged as a molecular target of great interest for the management of cardiovascular disorders due to its ability to reduce low density lipoprotein (LDL) cholesterol by binding and targeting at LDLR for lysosomal degradation in cells. Preliminary studies revealed that pseurotin A (PsA), a spiro-heterocyclic γ-lactam alkaloid from several marine and terrestrial Aspergillus and Penicillium species, has the ability to dually suppress the PCSK9 expression and protein-protein interaction (PPI) with LDLR, resulting in an anti-hypercholesterol

The antimyotonic effect of lamotrigine in non-dystrophic myotonias: a double-blind randomized study.

Mexiletine is the only drug with proven effect for treatment of non-dystrophic myotonia, but mexiletine is expensive, has limited availability and several side effects. There is therefore a need to identify other pharmacological compounds that can alleviate myotonia in non-dystrophic myotonias. Like mexiletine, lamotrigine is a sodium channel blocker, but unlike mexiletine, lamotrigine is available, inexpensive, and well tolerated. We investigated the potential of using lamotrigine for treatment of myotonia in patients with non-dystrophic myotonias. In this, randomized double-blind, placebo-co

Skeletal advancement for the treatment of obstructive sleep apnea in children.

OBJECTIVE: Maxillomandibular advancement is curative for some adult patients with obstructive sleep apnea (OSA). Little is known, however, about the efficacy of this treatment in children. The purpose of this retrospective analysis is to assess the clinical outcomes of children with medically refractory OSA who were treated with a variety of procedures to advance the maxillofacial skeleton. METHODS: The records of eight children with OSA (five boys and three girls; mean age, 8.6 years; range, 2 to 17 years) were reviewed. Six children had identifiable syndromes associated with micrognathia, o

Interventions for treating people with symptoms of bladder pain syndrome: a network meta-analysis.

BACKGROUND: Bladder pain syndrome (BPS), which includes the condition of interstitial cystitis, is a poorly understood clinical condition for which patients present with varying symptoms. Management of BPS is challenging for both patients and practitioners. At present, there is no universally accepted diagnosis and diverse causes have been proposed. This is reflected in wide-ranging treatment options, used alone or in combination, with limited evidence. A network meta-analysis (NMA) simultaneously comparing multiple treatments may help to determine the best treatment options for patients with

[Guidelines for the diagnosis and treatment of obstructive sleep apnea in adults (2025)].

Obstructive sleep apnea (OSA) is a highly prevalent sleep-related breathing disorder with a considerable healthcare burden. Previously, the 2011 guideline for adult obstructive sleep apnea was largely formulated on the basis of expert opinion and consensus, which played an important role in guiding clinical practice. With the growing body of evidence from high-quality clinical studies, however, guideline development models that rely primarily on expert consensus are no longer adequate to support contemporary clinical decision-making. To further standardize the screening, diagnosis, treatment,

Six Steps to Squats: Design, Feasibility, and Baseline Results of a Pilot Functional Rehabilitation Protocol After Orthopaedic Trauma.

Background and objective Functional, multi-joint assessments provide more insight into orthopaedic recovery than isolated range-of-motion or time-based metrics. The squat requires coordinated hip, knee, and ankle motion and reflects mobility, strength, and neuromuscular control. Squat-based assessments have shown value in anterior cruciate ligament injury, osteoarthritis, and arthroplasty populations, but no standardized squat-focused protocol exists for orthopaedic trauma rehabilitation. This article describes the Six Steps to Squats protocol, a structured, progressive squat-based rehabilitat
