[Auditory perception disorders due to bilateral cortical lesions. An electrophysiology study].

A 78-year-old right handed man with a past history of atrial fibrillation developed in November 1994, a slight right hemiparesis with aphasia which cleared over one month. Head CT scan showed a left middle cerebral artery infarct involving the posterior part of the temporal lobe. In September 1995, a second stroke occurred. Head CT scan revealed a recent right middle cerebral artery infarct within the posterior part of temporal cortex. Auditory agnosia was diagnosed. Auditory evoked potentials recording showed bilateral dysfunction of central auditory pathways mainly over the right hemisphere.

Illumination perception in photophobic patients suffering from panic disorder with agoraphobia.

Ten (6.9%) of a sample of 144 patients with panic disorder and agoraphobia developed photophobic behaviour in the course of their anxiety disorder. Their illumination ratings were characterized before and after cognitive behaviour therapy and compared to 10 age- and sex-matched normal control subjects. The illumination of a sheet of printed paper by a continuously adjustable reading lamp was rated by the subjects as 'too dark', 'comfortable' or 'too bright'. The ratings for comfortable luminance were significantly lower in the photophobic patients than in the controls, and renormalized after c

Functional MRI of lateral occipitotemporal cortex during pursuit and motion perception.

We performed functional imaging with a conventional 1.5-T magnetic resonance scanner in 9 normal subjects. We used a gradient-echo technique to examine changes in signal between periods when subjects viewed a stationary black-and-white grating, a moving grating, and when they followed a moving spot. We located image pixels with significant differences between the viewing conditions. In 7 subjects, these occurred in the lateral occipitotemporal cortex, a region previously identified as a putative human homologue of the motion-sensitive middle temporal area (MT, or V5) of monkeys. Signal intensi

Visceral perception in functional gastro-intestinal disorders: disease marker or epiphenomenon?

The pathophysiology of functional gastro-intestinal disorders remains unclear. A relatively new approach to these disorders has been the study of visceral sensory perception. A decreased pain threshold to intraluminal balloon distension has been demonstrated in patients with irritable bowel syndrome, functional dyspepsia, and non-cardiac chest pain. This altered visceral sensitivity does not appear to extend to somatic sensation; patients have generally had normal sensory thresholds to various stimuli applied to the skin. It is uncertain whether altered gut sensation represents a primary event…

Spatiotemporal integration and object perception in infancy: perceiving unity versus form.

3 experiments investigated 5-month-old infants' perception of an object whose center was fully occluded and whose ends were visible only in succession. Infants perceived this object as one connected whole when the ends of the object underwent a common motion behind the occluder, but not when the ends were stationary. Although infants perceived the connectedness of the object, they did not appear to perceive the object's shape. These findings suggest (a) that young infants are capable of integrating information over time to perceive object unity but not object form, (b) that young infants perce…

The nightmare of FDA clearance/approval to market: perception or reality?

Over the last few years the Center for Device Evaluation and Research (CDRH) at the Food and Drug Administration (FDA) has received annually over 16 thousand submissions related to medical devices. Over 10,000 of these are major submissions which include applications to conduct clinical trials and applications to market medical devices for a specified indication for use. Each application is carefully considered. FDA personnel work closely with applicants to ensure that clinical trial design minimizes risk to the patients and maximizes benefit with respect to addressing the safety and effective

Variation in nutrient intake with dental status, age and odour perception.

OBJECTIVES: Since the proportion of elderly people with an insufficient intake of nutrients is high and many of the elderly have poor odour perception or poor dental state, in this study, the relation between age, odour perception, dental state and nutrient intake is explored. DESIGN: Single centre cross sectional study. SETTING: Independently living elderly were tested at their homes in 4 locations in Belgium, ranging in ages 60-90 y. SUBJECTS: 200 elderly participated in a 7 d food record study, resulting in 119 complete records. INTERVENTIONS: Food quantities were converted to nutrient …

Visual perception elicited by electrical stimulation of retina in blind humans.

OBJECTIVE: To evaluate the feasibility of bypassing damaged photoreceptors and electrically stimulating the remaining viable retinal layers to provide limited visual input to patients who are blind because of severe photoreceptor degeneration. METHODS: In the operating room with the patient under local anesthesia, focal electrical stimulation of the retinal surface with brief biphasic pulses was performed using small probes inserted through the sclera. The procedure was performed in five subjects who had little or no light perception. Three subjects had retinitis pigmentosa, one had age-relat…

Imagery without perception--a case study of anosognosia for cortical blindness.

A patient with complete cortical blindness after bilateral posterior cerebral artery infarctions denied her blindness. Her pretended visual experiences could frequently be traced back to synaesthetic translations of acoustic or tactile perceptions into mental visual images. Possibly, the belief to see resulted from a confusion of mental visual images with real percepts. The patient manifested preserved visual imagery also by correct responses to questions concerning the shapes of letters and the shapes and colours of objects. MRI showed an almost complete destruction of primary visual cortex w…

Intensity perception. XIV. Intensity discrimination in listeners with sensorineural hearing loss.

Intensity discrimination of pulsed tones (also called level discrimination) was measured as a function of level in 13 listeners with sensorineural hearing impairment of primarily cochlear origin, one listener with a vestibular schwannoma, and six listeners with normal hearing. Measurements were also made in normal ears presented with masking noise spectrally shaped to produce audiograms similar to those of the cochlearly impaired listeners. For unilateral impairments, tests were made at the same frequency in the normal and impaired ears. For bilateral-sloping impairments, tests were made at di

Clinicopathological correlations of visual depth perception in patients with cerebrovascular disease.

Visual depth perception in 100 patients with cerebrovascular disease was evaluated using the Titumus stereotest. Twelve patients showed depth perception impairment. CT scans revealed that the lesion was located on the right hemisphere in 6 patients and on the left hemisphere in 3 patients, and that the remaining 3 patients had multiple infarctions. 123I SPECT was performed in 7 patients with moderate to severe depth perception impairment, of whom 6 patients showed a reduced blood flow in the posterior half of both the right and left cerebral hemispheres. Moderate to severe impairment of depth …

Depth perception and cortical physiology in normal and innate microstrabismic cats.

Evidence is presented that innate microstrabismus and abnormal cortical visual receptive-field properties can occur also in cats without any apparent involvement of the Siamese or albino genetic abnormalities in their visual system. A possible cause for microstrabismus in these cats may be sought in an abnormally large horizontal distance between blind spot and area centralis indicated by a temporal displacement of the most central receptive fields on both retinae. Depth perception was found to be impaired in cats with innate microstrabismus. Behavioral measurements using a Y-maze revealed in …

Risk perception, family history, and use of breast cancer screening tests.

A family history of breast cancer is an established risk factor for the disease, but there are few published studies on the use of breast cancer screening tests among family-history-positive women. Among 1015 respondents to telephone surveys of random samples of women 50 to 75 years old residing on Long Island (New York), the frequency of ever having had a mammogram or having a mammogram during the past year was significantly higher in respondents with a self-reported family history of breast cancer than in other women 50 to 64 (but not 65 to 75) years old. The association between family histo

Orientation perception, motion sickness and vertigo: beyond the sensory conflict approach.

Current theoretical issues central to the understanding of pathological disorientation (vertigo) are addressed through a critical review of research into perceptual disorientation in healthy subjects (motion sickness). Investigations inspired by the 'sensory conflict' model of orientation perception typically paid insufficient attention to higher-order meaningful properties of the environment, purposive activity, and individual differences in sensorimotor experience and skill. These factors are incorporated into an alternative 'active perception' approach, which characterizes perception of ori…

Perception of figure orientation by nonambulatory children with profound mental retardation.

An habituation-dishabituation procedure was used to study perception of changes in figure orientation by nonambulatory children with profound mental retardation. On each of 3 days, 16 participants were given familiarization trials with either a vertical or an horizontal pattern, followed by test trials with the familiarized stimulus and one of three novel stimuli--a 90 degree or 45 degree rotation of the pattern, or a rearrangement of the pattern elements into a square. Visual fixation times decreased over familiarization trials. Fixation times on test trials were longer for novel than for fam…

Social perception of AIDS in the general public: a French study.

The paper presents the results of the first national survey about social perception of AIDS in a representative sample of the French general public (which was carried out in June 1987). A large majority of the French public (73.1% of respondents) support mandatory screening for HIV, and a significant part (21.9% of respondents) even favours isolation of AIDS patients. Results show the relations between false beliefs on transmission of AIDS by casual contact and willingness to agree with measures that carry a great danger of stigmatization for AIDS patients and HIV carriers; but strong correlat…

Quantification of reaction time and time perception during Space Shuttle operations.

A microprocessor-based test battery containing simple reaction time, choice reaction time, and time perception tasks was flown aboard a 1985 Space Shuttle flight. Data were obtained from four crewmembers. Individual subject means indicate a correlation between change in reaction time during the flight and the presence of space motion sickness symptoms. The time perception task results indicate that the shortest duration task time (2 s) is progressively overestimated as the mission proceeds and is statistically significant (p less than 0.01) when comparing preflight and postflight baselines. Th…

Looking but not seeing: attention, perception, and eye movements in simultanagnosia.

We studied three subjects who reported the apparent "disappearance" of stationary objects from direct view. They had simultanagnosia caused by CT-verified bilateral superior occipital lobe lesions. They had no abnormalities of visual acuity or fields to explain their defect. EOG with computer analysis showed intact motility and scanning. Most important, the subjects reported intermittent disappearance of a light target during EOG-verified fixation--ie, they were looking but not seeing. Results indicate that attention mechanisms that permit sustained awareness of visual targets depend on the su

The correlation between the field defects for ocular vergence and stereomotion perception leads the authors to suggest that the same defect in binocular interaction is responsible for both the eye movement and sensory abnormalities.
