Regeneration of the pulp-dentin complex in mature teeth using autologous demineralized dentin matrix and dental pulp stem cells: two case reports.

BACKGROUND: Regenerating the true pulp-dentin complex in mature teeth with necrotic pulps or apical lesions remains challenging, and consistent clinical evidence of dentin bridge formation is limited. This report describes two adult cases treated with regenerative endodontic therapy (RET) using autologous dental pulp stem cells (DPSCs) and demineralized dentin matrix (DDM). CASE PRESENTATION: In Case 1, a 33-year-old woman developed pulp necrosis with apical periodontitis in a maxillary left central incisor following traumatic subluxation. After canal disinfection, autologous DPSCs isolated f

C-shaped Root Morphology with Four Canals in Mandibular First Molar: A Rare Case Report.

AIM: Aim of this case report is to present an unfamiliar case of mandibular first molar with a C-shaped root having four canals. BACKGROUND: Root canal system (RCS) is generally very complex, and it is always important for every practitioner who perform root canal treatment to have a knowledge of unusual variations in the root anatomy and root canal configuration, such as the C-shaped configuration. The frequency of C-shaped root canals is more common in mandibular second molars but very rare in mandibular first molars. CASE DESCRIPTION: A 28-year-old male patient presented with a history of

Root canal microbiota as an augmented reservoir of antimicrobial resistance genes in type 2 diabetes mellitus patients.

BACKGROUND: Antimicrobial resistance is a global public health problem. Root canal microbiota associated with apical periodontitis represents a well-known reservoir of antimicrobial resistance genes (ARGs). However, the effect of type 2 diabetes mellitus (T2DM) in this reservoir is unknown. This study aimed to establish if root canal microbiota associated with apical periodontitis in T2DM patients is an augmented reservoir by identifying the prevalence of nine common ARGs and comparing it with the prevalence in nondiabetic patients. METHODOLOGY: This cross-sectional study included two groups:

Neutrophil Extracellular Traps Aggravate Apical Periodontitis by Stimulating Osteoclast Formation.

INTRODUCTION: Neutrophil extracellular traps (NETs) have been described as structures composed of DNA and proteins, such as elastase and myeloperoxidase, that are able to kill bacteria extracellularly. The aim of the present study was to evaluate the role of NETs in bone resorption observed in pulp infection-induced apical periodontitis in mice. METHODS: Apical periodontitis was experimentally induced by exposing the dental pulp of the mandibular first molar of mice to the oral microenvironment. The expression of NETs was evaluated by immunofluorescence in mice and biopsies of apical periodon

Effect of the Periapical "Inflammatory Plug" on Dental Pulp Regeneration: A Histologic In Vivo Study.

INTRODUCTION: In the current study, we investigate the effect of the inflammation occupying the apical foramen-a phenomenon we refer to as "inflammatory plug"-on the regenerative potential of a root canal therapy. METHODS: We performed root canal treatment (RCT) in 12 canine root canals while aseptically instrumenting the apex to a 0.5-mm-wide foramen and obturating the canals with the following materials: collagen sponge, platelet-rich fibrin, and blood clot (no material introduced). RESULTS: We were successful in maintaining the integrity of the periapical tissue in 8 of 12 RCTs. Injury to

Effects of 5-lipoxygenase gene disruption on inflammation, osteoclastogenesis and bone resorption in polymicrobial apical periodontitis.

OBJECTIVES: To investigate the regulation of inflammatory and osteoclastogenic signaling by 5-lipoxygenase (5-LO) in apical periodontitis induced by oral contamination of dental root canals in mice. DESIGN: Apical periodontitis was induced in 5-lipoxygenase enzyme knockout (129-Alox5tm1Fun) and 129 wild-type mice (n = 96) by exposure of the dental root canal to the oral cavity. After 7, 14, 21, and 28 days, the animals were euthanized and the tissues removed (n = 12 teeth per period) for histopathological and histometric analyses (hematoxylin and eosin [HE]), evaluation of osteoclastogenic ac

Efficacy of early dental and ENT therapy in preventing nephropathy in pediatric Henoch-Schönlein purpura.

In a previous study, we demonstrated the benefit of tonsillectomy for early recovery from Henoch-Schönlein purpura (HSP) nephritis (HSPN), suggesting the pathological role of tonsils in HSP (Inoue et al., Clin Nephrol 67:298-305, 2007). In this study, we evaluated the efficacy of extensive eradication of infectious foci directly connected to the tonsils, including those involved in oral as well as ear, nose, and throat (ENT) diseases, in reducing the nephropathy in HSP. For this purpose, we examined the focal points of infection in 40 newly diagnosed HSP patients. After these focal points of i

Dentinogenesis imperfecta: endodontic implications. Case report.

Dentinogenesis imperfecta is a hereditary disorder resulting in defective dentin in both the primary and secondary dentitions. The complications of dentinogenesis imperfecta are difficult to manage and provide a challenge to the dentist. This case report concerns treating an African American patient with dentinogenesis imperfecta who appeared for treatment with endodontic pathosis. It illustrates the need for appropriate and timely restorative treatment to prevent pulpal pathosis. Also demonstrated is the difficulty of endodontically treating dentinogenesis imperfecta teeth because of pulpal o

Electrophoresis of whole-cell soluble proteins of microorganisms isolated from bacteremias in endodontic therapy.

We have previously demonstrated that anaerobic bacteria are the microorganisms most frequently isolated from blood following endodontic therapy of teeth with apical periodontitis. Phenotypic characterisation of the isolates suggested that the bacteria in the blood originated from the root canal. The present experiment using sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) was carried out in an effort to verify these findings, and to further study the microorganisms involved in endodontic bacteremias. Soluble cellular proteins were extracted from 11 reference strains and 26

Immunoglobulins in periapical exudates of infected root canals: correlations with the clinical findings of the involved teeth.

This study quantified the concentrations of IgG and IgA in the periapical exudates obtained from 69 single-rooted teeth using enzyme-linked immunosorbent assays, and examined their correlation with clinical findings of the involved teeth. Changes in the levels of these immunoglobulins during root canal treatment were also investigated. The average levels of IgG (85.61 mg/ml) was significantly higher than that of IgA (0.65 mg/ml) (P < 0.001). The exudates from the canals with large lesions (> or = 1 cm in diameter on radiographs) showed significantly higher IgG and IgA levels than small lesions

Periapical response to dentin filings. A pilot study.

Vital maxillary canines of mature cats were intentionally instrumented through the foramen with No. 50 K-files. Dentin chips were packed into the apical opening in one tooth of each animal. In the contralateral tooth a dentin plug was not formed. Both root canals were then filled with gutta-percha. The animals were sacrificed at 3, 5, and 8 months. The results showed quicker healing characterized by minimal inflammation and cementum deposition in the specimens where dentin fillings were packed.

Comparison of profiles of key periodontal pathogens in periodontium and endodontium.

Despite the established anatomical relationship between the periodontal and pulpal tissues, bacterial migration between endodontium and periodontium is still under discussion. The objective of this study was an investigation of profiles of periodontal pathogens in pulpal and periodontal diseases affecting the same tooth by means of 16S rRNA gene directed polymerase chain reaction (PCR). 31 intact teeth with both pulp and marginal infections were investigated. The diagnosis was based on clinical and radiological examination. Samples were taken from the gingival sulcus or periodontal pocket, res

Dental and Temporomandibular Joint Pathology of the Eastern Pacific Harbour Seal (Phoca vitulina richardii).

Skulls from 214 Eastern Pacific harbour seals (Phoca vitulina richardii) were examined macroscopically according to predefined criteria. The museum specimens were acquired from strandings along the west coast of the USA between 1909 and 2014. Ninety-eight skulls (45.8%) were from male animals, 108 (50.5%) from female animals and eight (3.7%) from animals of unknown sex. Their age varied from neonate to adult, with 101 adult animals (47.2%), 93 juvenile animals (43.5%) and 20 neonatal animals (9.3%). The majority of teeth were available for examination (90.0%); 7.5% of teeth were absent artefac

Use of colorimetry as a diagnostic tool for early detection of peri-implant diseases.

INTRODUCTION: The increasing aesthetic demand in dentistry and the limitations of visual colour assessment have encouraged the development of objective methods for evaluating dental and peri-implant tissues. Although colourimeters were originally designed for tooth shade matching, their application has recently expanded to peri-implant soft-tissue analysis, providing quantitative and reproducible measurements capable of detecting early inflammatory changes. OBJECTIVES: This study aimed to determine whether colour analysis using a colourimeter is a valid diagnostic tool for the preliminary det
