Massive Lumbar Disk Herniation Following "Therapeutic" Water Boiling of the Lower Extremities: Case Report and Literature Review.

BACKGROUND: Legs burning for treating lumbar radicular pain are still in use nowadays in low socioeconomical environments. They are dangerous as the case we report shows clearly. CLINICAL CASE: A 49-year-old man came to our attention with severe flaccid paraparesis occurred 10 days before, almost immediately after he had immersed his legs in boiling water to treat his severe left lumbocrural pain. This was known to be due to a right L3/4 herniated disk diagnosed by magnetic resonance imaging. At the examination he showed severe motor paresis and absent reflexes of his lower limbs, while crura

Clinical presentation of pseudo-abducens palsy.

PURPOSE: Pseudo-abducens palsy is a neurologic limitation in abduction with an intact abducens nerve. This rare condition can be observed when voluntary eye movements show impairment in lateral gaze, yet the vestibulo-ocular reflex (VOR) demonstrates full abduction. The intact VOR indicates the integrity of the infranuclear abducens nerve, thus suggesting that a pseudo-abducens palsy is likely caused by supranuclear pathology. CASE REPORT: A 63-year-old African American man presented with sudden-onset, binocular, horizontal diplopia. Extraocular motilities revealed a complete restriction of a

Ocular surface disorders in intensive care unit patients.

Patients in intensive care units (ICU) are at increased risk of corneal abrasions and infectious keratitis due to poor eyelid closure, decreased blink reflex, and increased exposure to pathogenic microorganisms. The aim of this retrospective study was to evaluate the ocular surface problems in patients who stayed in ICU more than 7 days and were consulted by an ophthalmologist. There were 26 men and 14 women with a mean age of 40.1 ± 18.15 years (range 17-74 years). Conjunctiva hyperemia, mucopurulent or purulent secretion, corneal staining, and corneal filaments were observed in 56.25%, 36.25

Mitochondrial neurogastrointestinal encephalomyopathy associated with progressive hearing loss.

OBJECTIVE: We report a rare case of mitochondrial neurogastrointestinal encephalomyopathy with hearing loss. CASE REPORT: A 46-year-old woman presented with a three-year history of progressive, bilateral hearing loss and tinnitus. She had been suffering from unexplained abdominal pain and diarrhoea for 20 years. When first seen, her otoscopic findings were normal, and pure tone audiometry showed mild and moderate hearing loss in her right and left ears, respectively. She also had: bilateral ophthalmoparesis, neck and limb muscle weakness, and hypoactive deep tendon reflexes on neurological ex

Facial nerve conduction after sclerotherapy in children with facial lymphatic malformations: report of two cases.

Surgical excision is thought to be the standard treatment of choice for lymphatic malformations. However, when the lesions are limited to the face only, surgical scar and facial nerve injury may impair cosmetics and facial expression. Sclerotherapy, an injection of a sclerosing agent directly through the skin into a lesion, is an alternative method. By evaluating facial nerve conduction, we observed the long-term effect of facial lymphatic malformations after intralesional injection of OK-432 and correlated the findings with anatomic outcomes. One 12-year-old boy with a lesion over the right-s

[A case involving complete removal of C2 dumbbell type neurinoma with preservation of the surrounding supportive tissue].

We report a case in which C2 neurinoma was completely removed with preservation of the surrounding supportive tissue. A 47-year-old female was admitted to our hospital with gait disturbance and spinal tumor identified in other hospital. Neurological examination on admission revealed hyperreflexia in bilateral limbs and pathological reflexes. Cervical plain X-radiography showed erosion of the left C2 lamina. Axial Gd-enhanced T1 weighted magnetic resonance (MR) images showed partial enhancement of a dumbbell shaped tumor at the C1-2 level, revealing compression of the spinal cord to the right a

5HT2 and 5HT3 receptors' contribution to modeling of post-serotonin respiratory pattern in cats.

Cardio-respiratory reflex effects of an exogenous serotonin challenge are suggested to be modulated by activation of the peripheral 5HT2 and 5HT3 receptors. In the present experiments the blocking effects of serotoninergic active drugs: ketanserin and tropanserin (MDL 72222) were studied in six pentobarbitone-chloralose anaesthetized cats. Bolus injection of serotonin (0.05 mg.kg(-1)) into the right femoral vein evoked prompt apnea, hypotension followed by tachypnoeic breathing. Pre-treatment with ketanserin (0.1 mg.kg(-1)), 5HT2 receptor antagonist, shortened the duration of post-serotonin ap

[Acute infantile hemiplegia caused by Coxsackie B virus].

INTRODUCTION: Acute infantile hemiplegia (AIH) is a syndrome that appears, typically, in infants without any underlying neurological damage. It can be due to a number of causes, in which viral infections play an important role. We report a case of AIH caused by Coxsackie B virus. CASE REPORT: A 20-month-old female who visited because of fever, vomiting and progressive left hemiparesis. Clinical exploration did not reveal any alteration in the level of consciousness and the only striking feature was the left hemiparesis, with weak deep muscle reflexes. Of the complementary examinations carried

Volume expansion suppresses the tooth-pulp evoked jaw-opening reflex related activity of trigeminal neurons in rats.

The aim of the present study is to clarify whether physiological stimulation of vagal afferents modulates the activity of the trigeminal spinal nucleus oralis (TSNO) neurons related to the tooth-pulp (TP)-evoked jaw-opening reflex (JOR) in pentobarbital-anesthetized rats. The activity of TSNO neurons and the amplitude of digastric electromyogram (dEMG) increased proportionally during 1.0-3.5 times the threshold for JOR. The amplitude of the dEMG of 14 out of 17 rats was suppressed by physiological stimulation of vagal afferents after intravenous infusion of Ficoll. Out of 23, 18 TSNO unit acti

Gestational thyrotoxicosis with acute Wernicke encephalopathy: a case report.

A 35-year-old hyperthyroid woman who developed nausea, vomiting, tachycardia, nystagmus and mental disturbance, was referred to our hospital with a suspected diagnosis of thyroid storm. However, the thyroid gland was only slightly palpable, bruits were not audible, and exophthalmos was not present. Serum levels of thyroid hormone were increased, but TSH receptor antibodies were negative. Echography and color flow doppler ultrasonography revealed a slightly enlarged thyroid gland and a slightly increased blood flow, both of which were much less milder than those expected for severe hyperthyroid

Electrophysiological study of myoclonus in pediatric practice.

Myoclonus can be caused from a variety of central nervous system disorders. With regard to its pathophysiology, much work has been done in adult patients, but reports from pediatric clinics are scarce. We describe the clinical and electrophysiological features of four different types of myoclonus in children using back averaging technique. In a patient with cortical reflex myoclonus, myoclonic jerks were induced in the distal right arm by taps on the ipsilateral fingers. Myoclonus was preceded by the cortical spike which was localized posterior to the central sulcus contralateral to the myoclo

Restraint of the paca (Agouti paca) with ketamine hydrochloride, acetylpromazine maleate, and atropine sulfate.

The paca (Agouti pace) is a neotropical rodent that requires chemical restraint for handling due to its aggressive nature and susceptibility to stress. Eighteen adult pacas (seven males and 11 females) weighing 6.25+/-2.22 (mean +/- SD) kg were given ketamine hydrochloride (25.0 mg/kg), acetylpromazine maleate (0.125 mg/kg), and atropine sulfate (0.05 mg/kg) combined by i.m. injection during field procedures that included sexing, marking, translocations, physical examinations, and minor surgeries. In all individuals, the induction of anesthesia was rapid and uneventful. Righting reflexes were

An analysis of feeding and sensorimotor abilities of rats after decortication.

Feeding and sensorimotor abilities of decorticate rats are described. Within 24 hr after surgery, the rats ate mash, unfamiliar foods, dry chow, and tap and sugar water from a spatula. They sampled nonnutritive substances but rejected noxious substances. Although initially anorectic, given mash, they arrested weight loss as early as 2 to 4 days. With training, they drank water from a spout by Day 10 and gained weight on dry food and water. Throughout survival they had difficulties with chewing food, lapping water, and trimming toenails. They showed sensorimotor neglect to objects touched to th

Adie's syndrome: some new observations.

Adie's syndrome is a disease of unknown etiology. We known where the damage is, and which nerves are involved. We even know something of how the nerves react after the damage is done, but we don't known what causes the primary injury. The first step in working a jigsaw puzzle is to getall of the pieces right side up and take a good look at them. Some of the jigsaw pieces handled in this paper are listed below. Some of them are new observations; many of them are old concepts, partly modified and partly made secure by new facts. 1. Not all "tonic pupils" are due to "Adie's syndrome"; some are du
