[Idiopathic retroperitoneal fibrosis causing obstructive uropathy: a case report].

Idiopathic retroperitoneal fibrosis leading to obstructive uropathy is a rare disease in our country, so a typical case deserves presentation. The patient is a 56-year-old female to have a clinical manifestation of obstructive uropathy. Retrograde pyelogram permitted 5 Fr. ureteral catheter to pass the stenotic segment. At the mean-time, there was hydronephrosis, medial deviation and gradual thinning and pointing of the middle segment of the ureter (classic triad). Computed tomography revealed that retroperitoneal soft tissue plaque enveloped aorta, inferior vena cava, and bilateral ureters. T

Diagnosis of developmental pathology of the neuraxis by magnetic resonance imaging.

We have used magnetic resonance imaging (MRI) to evaluate 22 patients with various clinical and/or radiographic findings potentially associated with intraspinal pathology whose evaluation would have necessitated myelography or computed tomography scanning in the past. Ten of these 22 children were diagnosed by MRI to have significant intraspinal lesions: diastematomyelia in four, syringomyelia in four, tethered spinal cord in three, intraspinal lipoma in two, and spinal cord tumor in one. MRI is noninvasive, requires no exposure to ionizing radiation, and provides multiplanar images and three-

Recurrent shoulder dislocation after surgical repair: double-contrast CT arthrography. Work in progress.

Nine cases of recurrent postoperative shoulder instability, resulting from failed surgical repair, were studied with double-contrast computed tomography (CT) arthrography. Repeat operations in seven cases showed excellent correlation between CT and surgical anatomic findings. CT arthrography was useful in confirming the direction of instability, particularly of the posterior and multidirectional types. Capsular laxity, subscapularis muscle and tendon abnormalities, and Bankart lesions that were either recurrent or not identified during previous operations were the most common causes of recurre

Importance of comparison of current and prior mammograms in breast cancer screening.

PURPOSE: To retrospectively determine the influence of comparing current mammograms with prior mammograms on breast cancer detection in screening and to investigate a protocol in which prior mammograms are viewed only when necessary. MATERIALS AND METHODS: Institutional review board approval was not required. Participants gave written informed consent. Twelve experienced screening radiologists read 160 soft-copy screening mammograms twice, once with and once without prior mammograms. Eighty mammograms were obtained in women in whom breast cancer was diagnosed later; the other 80 mammograms ha

Synovial Sarcoma Is Not Associated With a Higher Risk of Lymph Node Metastasis Compared With Other Soft Tissue Sarcomas.

BACKGROUND: Reported rates of the incidence of lymph node metastasis in soft tissue sarcoma vary considerably. Many are based on single-institution series and small patient populations. Certain sarcoma subtypes, including synovial sarcoma, have been associated with a higher risk of lymph node involvement. Most single centers have insufficient numbers of patients to assess lymph node metastasis accurately, but larger national databases may allow a more accurate estimation. QUESTIONS/PURPOSES: We queried a large national database and asked the following questions: (1) What proportion of patient

Musculoskeletal ultrasound in traumatic emergencies: what radiologists need to know.

While conventional radiography (CR) and computed tomography (CT) remain diagnostic staples in emergency settings, ultrasound (US) is a critical tool offering a rapid, dynamic, and noninvasive alternative for acute musculoskeletal (MSK) pathologies assessment. Prompt identification of conditions such as unstable fractures, soft tissue or muscular hematomas, joint effusions, and tendon ruptures is essential to prevent or mitigate long-term patient morbidity. This review summarizes the role of US in the emergency department, providing a practical overview of targeted scanning techniques and key s

An Unusual Delayed Recurrence of Temporal Ameloblastoma: A Case Report.

Ameloblastoma is a benign but locally aggressive odontogenic tumor with a recognized tendency for recurrence. Recurrence after more than a decade is exceedingly rare. We report a case of a 46-year-old woman who presented with a firm, non-tender, gradually progressive swelling in the left preauricular region of one-year duration. Fine-needle aspiration cytology (FNAC) revealed a lymphocytic infiltrate. Computed tomography demonstrated a soft-tissue lesion adjacent to a bony remnant resembling a coronoid process. Histopathology confirmed follicular ameloblastoma. Surgical excision of the lesion

Atypical Inflammatory Laryngopharyngeal Web With Concurrent Nasopharyngeal Stenosis in an Older Cat.

An older neutered male Korean Shorthair cat was evaluated for chronic respiratory distress, stertor, dysphagia, and severe weight loss. Radiography and computed tomography revealed nasopharyngeal narrowing and a thin membranous web-like obstructive structure at the laryngopharynx, resulting in near-complete airway obstruction, with concurrent fluid-to-soft tissue attenuating material within the frontal sinus and tympanic bulla. Surgical exploration confirmed a membranous lesion, and histopathology demonstrated chronic ulcerative palatitis without evidence of neoplasia. This case suggests that

Orbital myiasis secondary to neglected medial canthal basal cell carcinoma caused by Chrysomyabezziana.

A case of orbital myiasis caused by Chrysomya bezziana arising from a long-standing, untreated basal cell carcinoma of the medial canthus in a 52-year-old woman is reported. The patient presented with periocular swelling, foul-smelling discharge, bleeding, and visible larvae emerging from the ulcerated lesion. Computed tomography showed orbital soft-tissue involvement with extension into the ipsilateral nasal cavity and ethmoid sinus, and focal bony erosion. Management included prompt mechanical removal of larvae, local wound care, systemic antibiotics, and adjunctive oral and topical ivermect

Radiological Considerations in Diagnosing Angiosarcoma Associated With Lymphedema Following Breast Cancer Surgery.

Angiosarcoma associated with lymphedema is a rare soft tissue malignancy that occurs owing to chronic lymphedema, primarily following breast cancer surgery. We present a case of an 83-year-old female who developed angiosarcoma 14 years after undergoing breast cancer surgery. Diagnosis was confirmed through excisional biopsy, histopathological evaluation, and imaging. Computed tomography and magnetic resonance imaging revealed diffuse dermal thickening with an enhanced subcutaneous nodular lesion on the right arm. This unusual case emphasizes the importance of vigilant monitoring in patients wi

Dyspnea with Hemidiaphragm Elevation in a Patient with Giant Cell Arteritis.

We herein report the first case of dyspnea with hemidiaphragm elevation in a 68-year-old woman with active giant cell arteritis (GCA), including successful treatment. Contrast-enhanced computed tomography showed a reduced density of the left ophthalmic artery and the left superficial temporal artery with increased soft tissue compared to the other side, indicating that the GCA had flared up and suggesting that the hemidiaphragm elevation might be caused by vasculitis-associated ischemia of the right phrenic nerve. Hemidiaphragm paralysis due to vasculitis-associated ischemia in patients with G

Intratracheal recurrence of thymoma 7 years after complete resection.

A 66-year-old woman with a massive anterior mediastinal tumour was diagnosed with type B2 thymoma. After 2 courses of chemotherapy, extended thymectomy and partial resection of the lung and large vessels were performed, and the tumour was completely resected. The patient received no postoperative therapy. Seven years after the surgery, follow-up chest computed tomography showed a papillary soft tissue tumour in the trachea almost obstructing the airway. Intratracheal tumour resection was performed under rigid bronchoscopy, and the pathological diagnosis of intratracheal recurrence of thymoma w

Imaging Findings of Breast Metastasis from Malignant Peripheral Nerve Sheath Tumor of Mediastinum: A Case Report.

Malignant peripheral nerve sheath tumor (MPNST), a rare soft-tissue sarcoma, is most commonly located in the trunk, extremities, and head and neck, but rare in the breast. We report a metastatic breast MPNST in a 27-year-old woman with neurofibromatosis type 1 (NF-1). Chest computed tomography revealed a well-defined, oval, mildly enhancing nodule in the right breast. US revealed a circumscribed, oval, heterogeneous echoic mass with vascularity and intermediate elasticity in the right upper outer breast. The breast mass was excised and diagnosed as MPNST on histopathology evaluation. Although

Acute airway obstruction by a mucus plug in a patient with a 12-year history of inserting a double cannula tracheostomy tube: A case report.

Acute airway obstruction long after tracheostomy has rarely been reported. An 81-year-old Japanese woman with a tracheostomy tube for 12 years developed a 2-day history of coughing-up sputum with difficulty, foreign body sensation, and mild dyspnea. Dyspnea worsened immediately after computed tomography, showing soft tissue opacity between the tip of the tracheostomy tube and the bronchi. A movable mass in the trachea, identified as mucus by pathological examination, was removed using bronchoscopy. Acute airway obstruction by a mucus plug potentially occurred with a long history of insertion o

Unusual Manifestation of Immunoglobulin G4-Related Disease Involving the Retroperitoneum: A Case Report.

Immunoglobulin G4 (IgG4)-related disease is a systemic disease characterized by dense lymphoplasmacytic infiltrates with abundant IgG4-positive plasma cells and fibroblast proliferation. The retroperitoneal involvement of IgG4-related disease usually appears as a soft-tissue mass covering the abdominal aorta or entrapping the ureters, resulting in hydronephrosis. Here, we present a case of IgG4-related disease with retroperitoneal involvement in a 75-year-old woman with an unusual manifestation. A preoperative computed tomography (CT) scan revealed an irregular infiltrative retroperitoneal mas

Olfactory ganglioneuroblastoma in a dog: case report and literature review.

A 7-y-old, intact male Alaskan Malamute was presented with a 3-mo history of stertor and epistaxis. Computed tomography of the skull revealed generalized loss of gas throughout both nasal passages with replacement by a soft tissue mass that traversed the cribriform plate. Histopathology revealed neoplastic neuroblast cells arranged in anastomosing cords, as well as separately located aggregates of ganglion cells. Both neoplastic cell populations demonstrated immunoreactivity to MAP-2, TuJ-1, and synaptophysin. Neuroblastic cells additionally exhibited punctate immunoreactivity to MCK and CK8/1

Orthodontic Management of a Palatal Fistula in a Patient With Pierre Robin Sequence Using 3D Intraoral Scanning and Computer-Aided Design.

Pierre Robin sequence (PRS) is a rare congenital abnormality that may be classified as a first branchial arch syndrome. Patients with PRS who present with cleft palate have a deformity to the soft or hard palate of the maxilla that is repaired within the first year of birth. Despite the repair, patients may experience oronasal complications. Surgical repair is usually necessary if palatal fistula presents, and prosthetic obturators are used to provide temporary relief prior to surgery. This case report outlines the steps in the successful fabrication of a custom-fitted temporary vacuum-formed
