Obstructive ileus caused by a swallowed foreign body (a "press-through" package) and preexisting adhesions.

A "press-through" package (PTP), because it is large and square-shaped, tends to be caught in the esophagus when swallowed. A rare case of obstructive ileus induced by a PTP that was swallowed by 54-year-old man with a history of diabetes mellitus and acute appendectomy is reported. The PTP was 19 mm x 18 mm and was lodged in the cecum, which was stenosed because of preexisting intestinal adhesions; consequently, the PTP obstructed the ileus. The PTP was surgically removed.

Diminished oesophageal traction forces with swallowing in gastro-oesophageal reflux disease and in functional dysphagia.

Relations between primary oesophageal peristaltic amplitude and traction force were studied in 30 normal volunteers, 12 patients with functional dysphagia, and 48 patients with gastro-oesophageal reflux disease, using a new intraluminal strain gauge device. Forces generated by swallowing in the normal oesophagus were 42 (35-60) g (median and interquartile range), a close positive correlation existing between traction force and contractile amplitude for each subject (r = 0.5 (0.38-0.6). Traction force increased with increasing balloon volume from 62 (50-73) g at 2 ml to 86 (70-105) g at 4 ml (p

Surgical variables affecting postoperative swallowing efficiency in oral cancer patients: a pilot study.

This study examined the correlation between swallow function at 3 months postoperatively and surgical variables including volume resected, flap volume, ratio of flap volume to volume resected, percentage of oral tongue, tongue base, and anterior and lateral floor of mouth resected, and whether or not the mandible was preserved in 30 surgically treated oral cancer patients. Swallows of measured amounts of liquid and paste (pudding) materials were examined videofluoroscopically. Nine measures of swallow function were completed for each swallow. A factor analysis of all swallow variables was done…

Effect of age and bolus variables on the coordination of the glottis and upper esophageal sphincter during swallowing.

We studied 1) the effect of age and bolus variables on a) the coordination of deglutitive vocal cord adduction and upper esophageal sphincte … ct on the duration of deglutitive vocal cord adduction. In both young and elderly volunteers, water swallows, compared with dry swallows, significantly shortened the interval between the onset of deglutitive vocal cord adduction and the onset of UES relaxation. In conclusion, coordination between deglutitive glottal and UES function, as well as the duratio …

[Relationship between cervical osteophytes and globus sensation--a study based on altered swallowing function].

The swallowing function of ten patients showing marked cervical osteophytes were studied by double contrast pharyngogram and manometric examinations. In five cases in which the pharyngeal clearance was B type according to Shuzaki's classification, a significant increase in the maximal swallowing pressure value on the oral side ipsilateral to the osteophytes was often observed. In five cases in which the pharyngeal clearance showed A + A' type (marked impairment), the frequency of significantly decreased maximal swallowing pressure on the oral side ipsilateral to the osteophytes and abnormal wa

[Facial, lingual and pharyngeal electromyography in children: a method to study sucking and swallowing disorders and their pathophysiology].

The authors describe an electromyogaphic protocol developed with the aim of evaluating the sucking-swallowing function and of studying the pathophysiology of its disturbances in the newborn and in infants. This protocol consists of associating several techniques. Some of them are traditional: EMG of the facial muscles and nerves and blink reflex, while others are original: EMG of the tongue and hypoglossal nerve, EMG of the velum palatinum, and a dynamic study using a baby bottle, of the sucking and swallowing reflex for a liquid meal. For each technique, practical procedures are described tog

A case of swallow syncope induced by vagotonic visceral reflex resulting in atrioventricular node suppression.

A 48-year-old man repeatedly experienced syncope associated with paroxysmal atrioventricular block (PAVB) while swallowing. PAVB ("Mobitz type II" AH block) occurred only when "balloon-like" deformity of the lower esophagus developed. Balloon inflation in the lower esophagus induced PAVB. However, inflation in the upper or mid portion, or other vagal maneuvers, caused only sinus slowing. PAVB was not induced after intravenous atropine. The PAVB in this patient was probably caused by a vagotonic reflex triggered by tensoreceptors in the lower esophagus, resulting in selective suppression of the

Oral motor functioning, frequency of swallowing and drooling in normal children and in children with cerebral palsy.

Attempts to eliminate drooling in children with cerebral palsy have ranged from speech therapy to radical surgery. Drooling primarily results from an overflow of saliva from the mouth due to dysfunctional voluntary oral motor activity, improper swallowing, or oral sphincter deficits and rarely from hypersalivation. A study was undertaken to determine typical orofacial electromyographic patterns and swallowing frequencies of normal children and children with cerebral palsy with oral involvement who do, and who do not drool, as well as to determine the correlation between swallowing frequency an

Dupilumab is efficacious for eosinophilic esophagitis irrespective of prior swallowed budesonide or fluticasone, or prior treatments used alongside swallowed topical corticosteroids: results from the phase 3, randomized, placebo-controlled, LIBERTY EoE TREET trial.

BACKGROUND: Standard treatments for eosinophilic esophagitis (EoE) may present adherence, tolerance, and efficacy challenges. Dupilumab 300 mg weekly is approved for the treatment of EoE in patients ≥ 1 year old, weighing ≥ 15 kg. This analysis aimed to evaluate dupilumab efficacy in patients from the LIBERTY EoE TREET trial (NCT03633617), with prior history of different EoE interventions. RESEARCH DESIGN AND METHODS: This analysis included patients from Parts B/B - C of LIBERTY EoE TREET. Dupilumab efficacy was analyzed according to prior swallowed budesonide or fluticasone use and in those

Swallowed dental prosthesis causing late oesophageal obstruction and tracheo-oesophageal fistula in a young patient.

A 25-year-old man presented to the emergency department after a rugby accident having swallowed a dental plate. Initial assessment at the time of ingestion, including a chest radiograph, was unremarkable and the plate was presumed to have been expectorated. Five years later he presented with progressive dysphagia, recurrent food bolus obstruction, and haemoptysis. Computed tomography of the thorax performed 5 years later revealed circumferential oesophageal thickening and a large inflammatory mass around a retained radio-opaque foreign body at the level of the carina, with an associated trache

Characterizing Submental Neuromuscular Activity of Swallowing Rehabilitation: An Electromyographic Evaluation of Rehabilitative Maneuvers in Healthy Adults.

PURPOSE: The effortful swallow (ES), the Mendelsohn maneuver (MM), and isometric tongue presses (TPs) are widely used swallowing maneuvers/exercises to improve elements of swallowing, such as muscle strength and biomechanics. However, the underlying neuromuscular mechanisms of these exercises remain unclear, potentially limiting our ability to specify treatment targets and improve treatment efficacy. This study aimed to compare submental neuromuscular activation patterns during the ES, MM, TPs, and typical swallows in healthy young and older adults. METHOD: As part of a larger randomized cros…

Role of the gigantocellular reticular nucleus in suppression of the swallowing reflex during stimulation of the pedunculopontine tegmental nucleus.

OBJECTIVES: A previous study reported that stimulation of the pedunculopontine tegmental nucleus (PTg) and the gigantocellular reticular nucleus (Gi) suppressed and modulated the swallowing reflex, respectively. The swallowing central pattern generator comprises the nucleus of the solitary tract (Sol) and lateral reticular formation (LRF). The PTg projects to the Gi, and the Gi projects to the Sol and LRF. Thus, suggesting that PTg-induced suppression of the swallowing reflex is mediated via the Gi. We investigated whether electric lesions of the Gi, or microinjection of methyllycaconitine (ML

Cassava starch enhances the printability and swallowability of rapeseed protein gels for 3D-printed dysphagia foods.

Plant-based proteins have attracted increasing attention in the development of easy-to-swallow foods and three-dimensional printing (3DP) materials due to their excellent nutritional properties. However, the disordered arrangement of protein molecular chains in rapeseed protein (RP) results in insufficient structural stability and poor self-supporting capacity during 3DP, leading to collapse behaviour. To address this limitation, cassava starch (CS) was incorporated into RP gels at different CS: RP ratios to improve printability and swallowing properties. FT-IR and XRD analyses demonstrated th
