Late presentation of triceps rupture. A case report and review of the literature.

Rupture of the triceps tendon is a rare injury. The treatment protocol for acute injuries is well described and predictably successful. However, if the diagnosis is missed, the surgeon is presented with a more difficult management problem. We report the case of a 19-year-old woman who presented 8 months following a triceps tendon rupture. The diagnosis and technique for repair and postoperative rehabilitation are described. A review of the literature is also presented. Although our patient regained excellent motion and function, we feel the injury is best managed by avoiding misdiagnosis via a

Case presentation: Concomitant avulsion fracture of the posterior calcaneal tubercle and rupture of the Achilles tendon.

The Achilles tendon acts by transmitting the force created by the contraction of the sural triceps to the calcaneus. This allows the elevation during the gait cycle. A sudden contraction in elderly patients may generate avulsion fracture of the posterior tubercle of the calcaneus due to poor bone quality or rupture of the tendon, with one of these two structures yielding in most cases. We report the case of a 72-year-old woman who suffered a low energy right heel trauma while stepping off the bus. She was diagnosed an avulsion fracture of the posterior tubercle of the calcaneus. When performin

The athlete's foot: the grey zone behind the ankle.

Posterior ankle and heel pain is common among athletes. The most common injury in this region is that of the Achilles tendon. However, besides the Achilles tendon, pain can originate from a retrocalcaneal bursitis, posterior impingement syndrome, os trigonum syndrome, or flexor hallucis longus tenosynovitis. These pathologies often caused by overuse, but can also occur after trauma. With careful examination, and the help of MRI imaging, the distinction can be made among these pathologies and the more common Achilles tendon problems. Like most overuse injuries, the majority of cases of retrocal

Bilateral, spontaneous and simultaneous rupture of the quadriceps tendon in chronic renal failure and secondary hyperparathyroidism. Report of five cases.

Bilateral and simultaneous rupture of the quadriceps tendon of the knee without significant trauma in five patients, two males and three females, with chronic renal failure and secondary hyperparathyroidism is reported. Ruptures of the extensor mechanism are relatively rare injuries. Unilateral rupture is more common. Bilateral, simultaneous and spontaneous rupture is a very rare condition and only eleven cases have been reported in patients with a number of systemic diseases. Two cases have also been described in healthy subjects. The following points contribute to the diagnosis of quadriceps

Operative treatment of typical overuse injuries in sport.

In this report 670 operations, performed in 643 patients were analyzed. The indication for the surgery was a chronic symptomatic overuse injury caused by sports and physical activity. 552 (82.4%) of the operations were performed in men and 118 (17.6%) in women. The mean age of the patients was 24 years. Almost 90 per cent of the patients had presented following competitive sports and ten per cent recreational sports. More than 91 per cent of the operations were performed for lower extremity problems. The knee, foot and ankle regions needed surgical treatment most often in this series. Tendon a

Factors influencing flexor tendon adhesions.

Formation of adhesions is precipitated by injury to the tendon sheath, suturing of the tendon, and immobilization. Of the factors associated with formation of adhesions, immobilization has been most extensively investigated clinically and experimentally. Early protected passive mobilization to improve flexor tendon healing is currently the most effective means of inhibiting formation of adhesions.

Spontaneous flexor tendon rupture in systemic lupus erythematosus: A case report.

Spontaneous flexor tendon rupture is an unusual complication of systemic lupus erythematosus (SLE) and has not previously been reported. While tendon ruptures in association with SLE have been focused on the previous studies, upper extremity tendon ruptures are infrequently reported in the literature. Here, we present an uncommon case of spontaneous flexor tendon rupture of the ring and little fingers in a patient with SLE and discuss the mechanism of injury and its surgical treatment.

Lateral dislocation and incarceration of the posterior tibial tendon through the distal tibiofibular syndesmosis.

A case of acute, traumatic, lateral dislocation of the posterior tibial tendon is presented as a unique pattern of posterior tibial tendon dislocation. Several attempts at both closed and open reduction were required to relocate the tendon and reduce the fracture-dislocation. This case is being presented to demonstrate the use of cross-sectional imaging to both recognize the initial bony injury and to identify tendon anatomy and pathology that may not be seen in the operating room with limited visualization.

Long Term Follow-up of Composite Flaps for Single-stage Reconstruction of Concomitant Tendon and Soft Tissue Defects.

UNLABELLED: Tendon rupture in the setting of significant soft tissue loss poses a challenging reconstructive situation, which requires (1) recreating a stable gait cycle, (2) reducing shear forces and re-rupture risk, and (3) providing adequate soft tissue coverage. In this study, we outline our experience with composite flaps in single-step reconstruction of various lower extremity tendinous injuries with soft tissue loss. METHODS: A retrospective review of all patients requiring free tissue transfer at our tertiary wound care center between 2011 and 2020 was performed. Patients undergoing s

Selective 360° percutaneous extensor carpi radialis brevis tendon release for tennis elbow: an experimental study.

OBJECTIVES: The purpose of this study was to define in volunteers and cadavers the positions of structures at risk and the extensor carpi radialis brevis (ECRB) origin limits for sonographically guided percutaneous tendon release in tennis elbow. METHODS: First, in volunteers, we used Doppler sonography to determine the position (danger zone) of the structures at risk (neurovascular bundle and radial collateral ligament) from the most lateral point of the epicondyle (point of entry). Second, in cadavers, we studied the footprint of the ECRB's origin for finally performing sonographically guid

Persistent isokinetic knee flexion strength deficits at the time of return to sport are not associated with a second ACL injury.

PURPOSE: To investigate the rate of a second anterior cruciate ligament (ACL) injury based on different levels of knee flexion strength limb symmetry index (LSI) at the time of return to sport (RTS) after ACL reconstruction with hamstring tendon autograft. METHODS: Data was extracted from a rehabilitation registry for patients aged 15-40 years, who participated in knee-strenuous sports pre-injury (Tegner ≥ 6) and underwent ACL reconstruction with hamstring tendon autograft. Isokinetic knee flexion strength was analysed and reported as LSI. Patients were categorised into three groups (≥90%, 80

[Rupture of the pectoralis major tendon and other extra-articular tendons of the shoulder : Recognition and treatment].

Ruptures of the pectoralis major (PM) tendon are rare but have increased in recent years, especially during fitness exercising, such as bench pressing. The pathomechanism is an eccentric load under pretension of the PM (falling onto the outstretched arm, injuries during ground combat, boxing and during downward movement when bench pressing). The rupture sequence starts from superior to inferior at the insertion site with initial rupture of the most inferior muscle parts, followed by the sternal part and the clavicular part. Most classifications are based on rupture location, extent and time of

Ultrasound of ankle and foot: overuse and sports injuries.

Sports and overuse injuries of the ankle and foot are commonly encountered in clinical practice. Ultrasound (US) has been established as an excellent diagnostic modality for foot and ankle injuries, providing a rapid noninvasive, economical, and readily available tool that is well tolerated by the patient with acute or chronic pain. The opportunity for dynamic examination is another advantage of US in evaluating ankle and foot pathology, where maneuvers such as muscle contraction and stressing of the joint may be particularly helpful. In many cases, US can be used as a first-line and only imag

Complications in brief: Quadriceps and patellar tendon tears.

Effective treatment of knee extensor mechanism disruptions requires prompt diagnosis and thoughtful decision-making with surgical and nonsurgical approaches. When surgery is chosen, excellent surgical technique can result in excellent outcomes. Complications and failures arise from missed or delayed diagnoses and from technical problems in the operating room. In particular, inappropriate surgical timing (especially late surgery), misplaced patellar drill holes, and failure to address concomitant injuries can result in complications seen when repairing a patellar or quadriceps tendon tear. Avoiding unfavorable results in microsurgical reconstruction in upper-extremity trauma.

The treatment of hand and upper-extremity injuries is complex. The treatment goals are to restore maximal hand function and, if possible, preinjury form. Several complications are frequently encountered while treating hand and upper-extremity injuries. Complications such as edema, direct structural destruction, and nerve palsies are the result of the inflicting injury. Skin graft contraction, fascial adhesions causing restricted tendon motion, and diminution of muscle function with time during staged reconstructions are treatment-related complications. The nature of these complications as well

MR imaging of midfoot injury.

MR imaging is the imaging modality of choice for the evaluation of acute and chronic injuries of the midfoot. The complex anatomy of this region demands the multiplanar capabilities of this modality, and because injuries of this region most commonly involve the tendon, ligament, and muscular supports of the foot and ankle, MR imaging offers superior soft tissue resolution. MR imaging has the added advantage of accurate osseous injury characterization in most instances; however, CT also may be required, because the two modalities are complementary for demonstrating pathology of bone.
