Pathogenesis and management of deformities of the elbow, wrist, and hand in late neonatal brachial plexus palsy.

The incidence of neonatal brachial plexus palsy (NBPP) has remained relatively stable despite awareness of the problem and improved obstetri … he treatment options for these deformities consist of soft tissue releases, corrective osteotomies, tendon transfers, joint fusions, and/or free muscle transfers. Rehabilitation and physical therapy treatment are critical after these procedures. This article reviews the pathogenesis of the common deformities seen in a late presentation of NBPP, the assessment of …

The Addition of a Nerve Transfer to the Treatment of Peroneal Neuropathy Secondary to Intraneural Ganglion: Case Series.

Peroneal intraneural ganglia are rare, and their management is controversial. Presently, the accepted treatment of intraneural ganglia is decompression and ligation of the articular nerve branch. Although this treatment prevents recurrence of the ganglia, the resultant motor deficit of foot drop in the case of intraneural peroneal ganglia is unsatisfying. Foot drop is classically treated with splinting or tendon transfers to the foot. We have recently published a case report of a peroneal intraneural ganglion treated by transferring a motor nerve branch of flexor hallucis longus into a nerve b…

Reverse midpalmar island flap transfer for fingertip reconstruction.

Significant soft tissue defects of the fingertip with exposure of tendon or bone represent a challenging problem when homodigital or heterodigital artery island flaps are not available. In an attempt to resolve this problem, 15 patients with fingertip injuries involving palmar skin defects or amputation were treated with a vascularized flap transfer from the midpalmar area. A 2.5 x 1.5 to 4.5 x 2 cm flap from the radial or distal aspect of the midpalm, pedicled on the terminal branch of the superficial palmar arch or common/proper palmar digital artery and vein, was transferred in a retrograde

Review of Outcomes After Peripheral Nerve Transfers for Motor Nerve Injury in the Upper Extremity.

BACKGROUND: Modern nerve-to-nerve transfers are a significant advancement in peripheral nerve surgery. Nerve transfers involve transferring donor nerves or branches to recipient nerves close to the motor end unit, leading to earlier reinnervation and preservation of the musculotendinous units in proximal nerve injuries. After nerve reinnervation, function may be superior to traditional tendon transfer techniques in terms of strength and independent motion. Nerve transfer surgery has emerged as a promising treatment option for many cases of nerve injury that were previously expected to result i…

Biceps augmentation in dynamic anterior stabilization: A novel on-lay technique for challenging shoulder instabilities.

Management of anterior shoulder instability in the presence of subcritical glenoid bone loss or poor anteroinferior capsulolabral tissue quality remains challenging, as isolated soft-tissue repair (e.g. Bankart repair) is often associated with suboptimal outcomes. Dynamic anterior stabilization (DAS) addresses this limitation by transferring the long head of the biceps (LHB) tendon after tenotomy at its articular origin through a subscapularis split and fixing it to the anterior glenoid rim. Although the DAS has shown favorable short-term results, its effectiveness may be reduced in cases with

This is especially … e Quervain tenosynovitis, open reduction internal fixation of hand fractures, acute tendon repairs, tendon transfers, and reconstructions have been performed under WALANT as outpatient procedures. The future of WALANT surgery in the Philippines is promising. Teaching WALANT to other areas of the country can counterbalance the large disproportion of hand surgeons …

Results of latissimus dorsi and teres major transfer to the rotator cuff in the treatment of Erb's palsy.

A retrospective study of the results of latissimus dorsi and teres major transfer in the treatment of Erb's palsy was conducted in 10 patients. All patients underwent release of the pectoralis major and transfer of the latissimus dorsi and teres major tendons to the rotator cuff at a mean age of 7 years and 2 months. Range of motion, patient satisfaction, and presence of any complications were evaluated postoperatively at a mean of 3 years. Active shoulder abduction improved from a mean of 72 degrees preoperatively to 136 degrees postoperatively. Postoperative shoulder active external rotation

Early Functional Outcomes After Single-Stage Achilles Tendon Reconstruction Using Free Anterolateral Thigh Flap With Tubularized Fascia Lata.

BACKGROUND: Achilles tendon injury with extensive soft tissue loss presents challenges to lower extremity (LE) reconstruction with functionally favorable results. The use of a free anterolateral thigh (ALT) flap incorporating a tubularized fascia lata (FL) segment offers a reconstructive option for these complex cases. This study evaluates the early outcomes of microsurgical reconstruction for Achilles tendon and soft tissue defects. METHODS: A retrospective review of patients requiring LE free tissue transfer (FTT) between 2012 and 2023 was performed. All patients included had Achilles tendo

Editorial Commentary: Subscapularis Tears With Advanced Fatty Infiltration: When Every Option Is Imperfect.

Full-thickness subscapularis tears with advanced fatty infiltration represent one of the most challenging problems in shoulder surgery, in large part because every available treatment option is imperfect. Reverse total shoulder arthroplasty in older patients and tendon transfer in younger individuals without glenohumeral arthritis are effective but often perceived by patients as extreme, while nonoperative management frequently leaves persistent symptoms. New literature suggests that perhaps a modification of a traditional open approach, in the form of open antegrade muscle advancement, is a v…

Successful Ponseti-treated Clubfeet at Age 2 Years: What Is the Rate of Surgical Intervention After This?

BACKGROUND: In recent decades, nonoperative Ponseti casting has become the standard of care in the treatment of idiopathic clubfoot. However … on procedures performed between 2 and 5 years were limited (a la carte) in scope: tibialis anterior tendon transfer, posterior release, plantar fascia release, and repeat tendo-Achilles lengthening. At age above 5 years, 53 patients (20.1%) with 65 feet (16.9%) underwent surgery. Ten of these 53 patients had already undergone surgery between 2 and 5 years of age. …

Surgical Treatment for Congenital Dislocation of the Patella in a Young Adult: A Case Report.

CASE: We present an 18-year-old man with poor alignment of the lower limbs, marked by congenital dislocation of the left patella, with gait impairment, and an associated 20° flexion contracture. Surgical treatment was performed with a single complex procedure with the release of the lateral retinaculum, capsule, and iliotibial tract, followed by a subtraction trochleoplasty, tibial tubercle transfer, elongation of the quadriceps tendon, and anatomical reconstruction of the medial patellofemoral ligament. At the 5-year follow-up, the patient had a significant improvement in gait pattern, with t

High Median Nerve Injuries.

The median nerve serves a crucial role in extrinsic and intrinsic motor and sensory function to the radial half of the hand. High median nerve injuries, defined as injuries proximal to the anterior interosseous nerve origin, therefore typically result in significant functional loss prompting aggressive surgical management. Even with appropriate recognition and contemporary nerve reconstruction, however, motor and sensory recovery may be inadequate. With isolated persistent high median nerve palsies, a variety of available tendon transfers can improve key motor functions and salvage acceptable …

[Instability of the distal radioulnar joint: Treatment options for ulnar lesions of the triangular fibrocartilage complex].

Injuries of the triangular fibrocartilage complex (TFCC) may be fatal to the distal radioulnar joint (DRUJ). This structure is one of the crucial stabilizers and guarantees unrestricted pronosupination of the forearm. A systematic examination is mandatory to diagnose DRUJ instability reliably. A clinical examination in comparison to the contralateral side is obligatory. Plain radiographs are required to exclude osseous lesions or deformities. Computed tomography of both wrists in neutral, pronation and supination is necessary to verify DRUJ instability in ambiguous situations. Based on a syste

Evaluation and management of brachial plexus birth palsy.

Brachial plexus birth palsy can result in permanent lifelong deficits and unfortunately continues to be relatively common despite advancements in obstetric care. The diagnosis can be made shortly after birth by physical examination, noting a lack of movement in the affected upper extremity. Treatment begins with passive range-of-motion exercises to maintain flexibility and tactile stimulation to provide sensory reeducation. Primary surgery consists of microsurgical nerve surgery, whereas secondary surgery consists of alternative microsurgical procedures, tendon transfers, or osteotomies, all o…

Extracorporeal shockwave therapy for treatment of delayed tendon-bone insertion healing in a rabbit model: a dose-response study.

BACKGROUND: Tendon-bone insertion (TBI) consists of both hard and soft tissues. TBI injury with delayed repair is not uncommon. High-dose extracorporeal shockwave (ESW) is effective for treating nonunion fracture, whereas low-dose ESW is used for tendinopathy therapy. The dosing effect of ESW on delayed TBI healing is lacking. HYPOTHESIS: Low-dose ESW might have a healing enhancement effect comparable to that of high-dose ESW in treating delayed TBI healing. STUDY DESIGN: Controlled laboratory study. METHODS: Partial patellectomy was adopted to create a delayed TBI healing model by shieldin
