[A pediatric case of HIV who diagnosed by virtue of disseminated cryptococcus infection].

Cryptococcus neoformans is an important opportunistic pathogen that causes serious mortality and morbidity in AIDS patients. Although its incidence has decreased with proper antiretroviral treatment (ART), it is still a major concern in areas with low socioeconomic HIV endemic countries with poor sources of therapy. In our country, pediatric HIV infection and so, HIV-related opportunistic infections are very rare. In order to pay attention to this unusual collaboration; herein, we presented a pediatric case who was diagnosed with HIV and disseminated cryptococcus infection concomitantly. A 6.5

In vitro characterization of the anti-bacterial activity of SQ109 against Helicobacter pylori.

The most evident challenge to treatment of Helicobacter pylori, a bacterium responsible for gastritis, peptic ulcers and gastric cancer, is the increasing rate of resistance to all currently used therapeutic antibiotics. Thus, the development of novel therapies is urgently required. N-geranyl-N'-(2-adamantyl) ethane-1, 2-diamine (SQ109) is an ethylene diamine-based antitubercular drug that is currently in clinical trials for the treatment of tuberculosis (TB). Previous pharmacokinetic studies of SQ109 revealed that persistently high concentrations of SQ109 remain in the stomach 4 hours post or

Tuberculosis pharmacotherapy: strategies to optimize patient care.

INTRODUCTION: The treatment of tuberculosis (TB) is a mature discipline, with more than 60 years of clinical experience accrued across the globe. The requisite Multi-drug treatment of drug-susceptible TB, however, lasts 6 months and has never been optimized according to current standards. Multi-drug resistant TB and TB in individuals coinfected with HIV present additional treatment challenges. OBJECTIVE: This article reviews the role that existing drugs and new compounds could have in shortening or improving treatment for TB. The key to treatment shortening seems to be sterilizing activity, o

Immune Responsiveness Induced by Administration of Heat-Inactivated M. bovis and BCG Prior to Experimental Infection in African Buffalo.

African buffalo (Syncerus caffer) are the primary wildlife reservoir for bovine tuberculosis (BTB) in Africa. Infected buffalo populations spread BTB to numerous species in conservation areas as well as to cattle at park boundaries. Quarantine requirements for BTB complicate wild animal metapopulation management. Considerable interest has arisen in the vaccination of cattle and indigenous wildlife species against BTB as an additional control strategy. This pilot trial aimed to evaluate cell-mediated and humoral immune responses in African buffalo immunised against BTB. Twenty-one sub-adult Afr

Tofacitinib for Calcinosis Cutis Associated With Juvenile Dermatomyositis: An Open-Label Single-Arm Study.

OBJECTIVE: Calcinosis cutis (CC) is dystrophic calcification affecting 20% to 40% of patients with juvenile dermatomyositis (JDM). Management of CC is often challenging owing to its being refractory to usual therapeutic options. Type I interferon (IFN)-mediated immune dysregulation may be involved in the pathogenic role in CC, suggesting that altering JAK/STAT signaling through JAK inhibitors might offer a therapeutic benefit. METHODS: In this open-label, single-arm study, the primary objective was to study the effect of tofacitinib (MSN Pharmaceuticals) on the burden of CC in children (2-18

Pathology of peliosis.

Peliosis is a pathological entity characterized by the gross appearance of multiple cyst-like, blood-filled cavities within parenchymatous organs. Peliosis has been related to several underlying debilitating illnesses such as tuberculosis, hematological malignancies, the acquired immunodeficiency syndrome (AIDS), and post-transplant immunodeficiency, as well as intravenous drug abuse, chronic alcoholism, and in conjunction with the intake of oral contraceptives or steroids. The classical pathoanatomical concept is based upon the opinion that peliosis exclusively develops in organs belonging to

Pulmonary cryptococcosis with cavitation in an immunocompetent child: a case report and literature review.

BACKGROUND: In recent years, the incidence of pulmonary cryptococcosis (PC) has risen among patients without human immunodeficiency virus (HIV) infection, including individuals with preserved immune function. The clinical manifestations of PC are largely non-specific, frequently presenting with fever, cough, chest tightness, or chest pain. In some cases, PC remains asymptomatic, which increases the likelihood of misdiagnosis or delayed recognition. The present study reports the diagnostic evaluation and therapeutic course of a child with PC and normal immune function, accompanied by a literatu

Socio-ecological vulnerability subgroups predict adherence and treatment outcomes in multidrug-resistant tuberculosis and HIV: a latent class analysis.

BACKGROUND: Despite the availability of effective all-oral regimens, treatment outcomes for rifampicin-resistant or multidrug-resistant tuberculosis (RR/MDR-TB) remain suboptimal, particularly in high HIV-burden settings. Factors across the socio-ecological spectrum strongly influence medication adherence and outcomes, yet approaches to identifying high-risk groups remain limited. We aim to identify clinically meaningful subgroups of individuals with RR/MDR-TB and HIV based on baseline socio-ecological vulnerabilities and examine whether subgroup membership is predictive of medication adherenc

High-throughput sequencing of bronchoalveolar lavage fluid confirms pulmonary paragonimiasis: A case report.

RATIONALE: Paragonimiasis, a rare parasitic disease, often presents diagnostic challenges due to its insidious onset, multiorgan involvement, and nonspecific clinical manifestations, frequently leading to misdiagnosis. While numerous reports describe diagnostic errors in paragonimiasis management, cases involving repeated misdiagnoses across multiple tertiary hospitals over 2 years (particularly with comprehensive epidemiological evidence of freshwater crab consumption) remain exceptionally uncommon. We herein present such a noteworthy case. PATIENT CONCERNS: A 45-year-old male was hospitaliz

Anesthetic Challenges during Endobronchial Brachytherapy: A Case Report.

Lung cancer is the leading cause of cancer death worldwide. It may present as airway obstruction in a patient with endobronchial masses. Endobronchial brachytherapy (EBBT) has been shown to provide palliative therapy. It is the insertion of a radioactive material near the mass to reduce tumor size, thereby improving airway obstruction. This is the first case of EBBT done in our institution during the COVID-19 pandemic. A 53-year-old male, 60 kg, ASA Physical Status 2 for hypertension, smoker, malignancy, and previous pulmonary tuberculosis patient, presented with a cough and dyspnea. An endobr

Topical isoniazid as a novel treatment for melasma: A randomized, double-blind, vehicle-controlled clinical trial.

INTRODUCTION: Melasma is a chronic hyperpigmentation disorder, and its treatment poses a challenge to dermatologists due to its chronicity and resistance to conventional therapies. Oral isoniazid is used for the treatment of tuberculosis. One of us had previously showed that topical isoniazid exerts a strong depigmenting action in animal models. In this clinical trial, we assessed the therapeutic effect of topical isoniazid on melasma. METHODS: Twenty female patients suffering from epidermal melasma were enrolled and divided equally into two groups. The treatment group received topical isonia
