Primary extrahepatic hydatid cyst of the thigh, an unusual presentation of sciatica: A case report of a diagnostic challenge.

INTRODUCTION: Sciatica, a condition characterized by pain along the sciatic nerve distribution, is commonly associated with nerve compression or irritation. However, its etiology can vary, including rare non-spinal causes such as hydatid cysts. We present a case of hydatid cyst in the thigh causing sciatica-like symptoms, highlighting the diagnostic challenges and management approach. CASE PRESENTATION: A 40-year-old patient with a history of pulmonary tuberculosis presented with persistent lumbosciatic pain despite conservative treatment. Physical examination revealed left sciatica without s

HIV-related Neuropathy: Pathophysiology, Treatment and Challenges.

HIV-sensory neuropathy (HIV-SN) is a debilitating complication in HIV patients with or without anti-retroviral treatment (ART). Common symptoms of HIV-SN include pain, decreased sensation, paresthesias, and dysesthesias in a symmetric stocking-glove distribution. While HIV-1 protein such as gp120 is implicated in HIV-SN (e.g. impaired large-diameter fiber), ART itself was recently shown to contribute to HIV-SN in HIV patients and impair thin fiber. Multiple host mechanisms may play roles during the pathogenesis of HIV-SN, including neuron-glia interactions in the spinal dorsal horn (SDH), infl

Extensive paraspinal abscess complicating tuberculous spondylitis in an adolescent with Pott kyphosis.

A 16-year-old girl who suffered from untreated tuberculosis spondylitis since childhood presented with marked Potts kyphotic deformity in adolescence. She had increasing shortness of breath and was admitted for investigation of right pleural effusion. On radiological examination, she was found to have extensive paraspinal abscesses, with the largest collection trapped within the kyphotic angulation in the thoracic region. The latter could only be appreciated with magnetic resonance (MR) imaging and had been overlooked in the initial computed tomography (CT).

Skeletal tuberculosis in children.

The objective of this review is to present the imaging findings of skeletal tuberculosis in children. The incidence of tuberculosis is increasing and skeletal tuberculosis accounts for 10-20% of all extra-pulmonary cases. The most common manifestations of skeletal tuberculosis in children are spondylitis, arthritis and osteomyelitis. Tuberculous spondylitis involves the intervertebral disc only late in the disease. Subligamentous spread of the infection may lead to multiple levels of vertebral body involvement that may either be continuous or skipped. Extension of the disease into the paravert

Intradural extramedullary tuberculoma of the spinal cord: a case report.

Intradural extramedullary (IDEM) tuberculoma of the spinal cord is uncommon entity and moreover, few reports have been documented on concurrent IDEM and intracranial tuberculomas. Authors report a case of IDEM spinal tuberculoma having intracranial lesion simultaneously. A 49-year-old woman suffered from paraparesis and urinary incontinence while being given medical treatment for tuberculous meningitis. Magnetic resonance imaging (MRI) revealed an IDEM mass lesion between the T1 and T2 spinal levels, and multiple intracranial tuberculous granulomas. Surgical resection of the IDEM tuberculoma f

Clinical Features and Treatment Strategies of Q Fever Spinal Infection: A Pooled Analysis of 39 Cases and Narrative Review of the Literature.

BACKGROUND: The incidence of spinal infections is increasing; However, pathogen identification remains challenging. Although Q fever spinal infection is reported infrequently, its accrual incidence is likely underestimated. The causative agent, Coxiella burnetii, cannot be routinely cultured. Consequently, physicians often misdiagnose Q fever spinal infection as spinal tuberculosis, leading to severe patient harm. Thus, improving clinicians' awareness of the clinical characteristics of Q fever spinal infection is urgently needed. METHODS: We present a case of Q fever spinal infection and cond

Clinical Insights and Statistical Analysis of Failed Back Surgery Syndrome: A Multicentric Retrospective Review.

INTRODUCTION: Failed back surgery syndrome (FBSS) is a well-documented yet underinvestigated phenomenon, with prevalence estimates ranging from 10 to 40% among all spine surgeries. FBSS contributes significantly to chronic pain and disability, leading to socioeconomic burdens. Studies have emphasized the multifactorial etiology of FBSS, including inadequate decompression, misdiagnosis, iatrogenic injury, and psychosocial contributors. However, there is a scarcity of large-scale, real-world clinical datasets offering multidimensional analysis of FBSS. MATERIALS AND METHODS: This retrospective

One-stage posterior debridement with transverse process strut as bone graft in the surgical treatment of single-segment thoracic tuberculosis: A retrospective single-center study.

This retrospective study investigated the effect of the novel bone graft transverse process strut (TPS) in single segmental thoracic spinal tuberculosis (TB) with the one-stage posterior approach of debridement, fusion, and internal instrumentation. Thirty patients treated in our department from March 2014 to October 2016 were retrospectively analyzed. Surgical time, blood loss, hospitalization time, drainage volume, and follow-up (FU) duration were recorded. The visual analog scale (VAS), Oswestry Disability Index (ODI), erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), American

Pyogenic osteomyelitis of the spine in the elderly.

The authors report 10 cases of spontaneous pyogenic spinal osteomyelitis encountered within a 3-year period. There were six women and four men, ranging in age from 60 to 84 years. Six cases occurred at the thoracic level, three at the lumbar level, and one in the cervical spine. No patient was diabetic, immunocompromised, or receiving steroid therapy, and none had a history of endocarditis or intravenous drug abuse. No patient had undergone previous spinal surgery. There were no instances of coexisting tuberculosis or malignancy. Contemporaneous cases with known predisposing factors have been

Atlantoaxial tuberculosis: Outcome analysis.

BACKGROUND: Tuberculous (TB) spondylitis occurs in <1% of patients with TB. Atlantoaxial involvement is extremely rare amounting to 0.3%-1% cases of TB spondylitis. The management of this entity has been fluctuating with time. Various classifications and management protocols have also evolved. We present our experience in managing this entity in 8 patients. MATERIALS AND METHODS: Retrospective follow-up study of eight patients with atlantoaxial TB over 2 years. They ranged in age from 14 to 33 years with male preponderance. Four required surgical intervention on initial presentation, 2 were s

Tuberculosis in the world is the mean cause of death from single infectious agent. It is reported increase not only in underdeveloped countries but in developed too. Tuberculosis meningitis is a very serious form of tuberculosis. Outcome in tuberculosis meningitis is associated with the stage of the disease at presentation. When the child at presentation is extremely ill with coma the completely recovery is seen only in about 20% of cases. In our case patient had severe ocular complications: optochiasmatic tuberculomas. It is difficult recognize the two forms of tuberculomas intra- and perichi
