Tonsillectomy with Uvulopalatopharyngoplasty in Obstructive Sleep Apnea.

BACKGROUND: Obstructive sleep apnea (OSA) is a very common disorder (prevalence 2-7% in women, 7-14% in men). It impairs the quality of life and increases mortality. Conservative treatment with continuous positive airway pressure is highly effective, but patient compliance is variable. Surgical treatments are controversial, as only a few are supported by evidence from controlled clinical trials. METHODS: Adult patients with OSA, CPAP intolerance, and oropharyngeal obstruction were included in the trial. All underwent polysomnography (PSG) and were randomly allotted to one of two groups. Patie

Surgical treatment of snoring & obstructive sleep apnoea.

Obstructive sleep apnoea (OSA) syndrome is a potentially serious disorder affecting millions of people around the world. Many of these individuals are undiagnosed while those who are diagnosed, often exhibit poor compliance with nightly use of continuous positive airway pressure (CPAP), a very effective nonsurgical treatment. Various surgical procedures have been proposed to manage and, in some cases, treat OSA. In this article we review methods used to assess the sites of obstruction and a number of surgical procedures designed to address OSA. Effective surgical management of OSA depends upon

Surgical procedures and non-surgical devices for the management of non-apnoeic snoring: a systematic review of clinical effects and associated treatment costs.

OBJECTIVES: To review the evidence on the clinical effects and associated treatment costs of surgical procedures and non-surgical devices for the management of non-apnoeic snoring. DATA SOURCES: Major electronic databases were searched for relevant studies published between 1980 and 2007. All treatment costs were estimated based on data from NHS reference costs, device manufacturers and clinical opinion. REVIEW METHODS: Studies were screened, data extracted and quality assessed according to standard methods. Results were broadly grouped according to the intervention and comparator when appli

Treatment of primary snoring using modified radiofrequency-assisted uvulopalatoplasty.

Snoring is a common complaint, especially among the elderly individuals. In the treatment of snoring, many options, surgical or nonsurgical, are available. In this randomized study, we used a modified technique including some components of radiofrequency-assisted uvulopalatoplasty (RAUP) and Uvulopalatopharyngoplasty UPPP (modified-RAUP, MRAUP) and RAUP in a control group. A total of 60 patients (58 male and 2 female), 30 in each group (MRAUP and RAUP groups), with an average age of 38 +/- 9 years were included in the study. In the MRAUP group, in addition to the modified surgery, preoperative

[Effective evaluation of multiple level surgery in obstructive sleep apnea-hypopnea syndrome].

OBJECTIVE: To investigate the therapeutic effects of multiple level surgery in treating obstructive sleep apnea-hypopnea syndrome (OSAHS). METHODS: One hundred ninety two patients with OSAHS diagnosed by polysomnography were treated through uvulopalatopharyngoplasty (UPPP). Thirty patients were combined with sub-mucous resection of the nasal septum. Forty four patients received sub-mucous resection of the nasal septum and partial inferior turbinectomy. Two patients received sub-mucous resection of the nasal septum and partial resection of the tongue base. Two patients received genioglossus ad

Reorganization of sleep architecture after surgery for OSAHS.

CONCLUSION: Genioglossus advancement and hyoid suspension (GAHM) plus uvulopalatopharyngoplasty (UPPP) can decrease the apnea hypopnea index (AHI) and arousal index, especially respiratory-related arousals. Correspondingly, sleep architecture is reorganized, which is expressed postoperatively as an increase in slow wave sleep and a decrease in lighter sleep. OBJECTIVE: To explore the reorganization of sleep patterns after GAHM plus UPPP for severe obstructive sleep apnea hypopnea syndrome (OSAHS). PATIENTS AND METHODS: GAHM plus UPPP was performed on 31 patients with severe OSAHS (AHI > 40).

Same-stage nasal and palatopharyngeal surgery for obstructive sleep apnea: is it safe?

OBJECTIVE: The study goal was to determine the safety of performing same-stage nasal and palatopharyngeal surgery for the treatment of obstructive sleep apnea syndrome (OSAS). STUDY DESIGN AND SETTING: We conducted a retrospective review of 91 consecutive patients who underwent surgery for OSAS at tertiary care facilities. METHODS: Patients were divided into 2 groups: group 1 had same-stage nasal and palatopharyngeal surgery (n = 63), whereas group 2 had palatopharyngeal surgery at a stage separate from the nasal surgery (n = 28). Patient demographics, severity of OSAS, type of surgery, peri

Lack of predictive value of the Epworth Sleepiness Scale in patients after uvulopalatopharyngoplasty.

The Epworth Sleepiness Scale (ESS) has been recognized as a valid measure of sleep propensity. Statistically significant correlations between ESS scores, the respiratory disturbance index (RDI), and the lowest arterial oxygen saturation (LSAT) have been described in patients with surgically untreated obstructive sleep apnea (OSA). We investigated whether the same relationships hold true after uvulopalatopharyngoplasty (UPPP). Forty-two adults with documented OSA treated by UPPP were reevaluated with the ESS questionnaire and 8-hour diagnostic nocturnal polysomnography (nPSG). We found no signi

Snoring, obstructive sleep apnea, and surgery.

Snoring and OSA syndrome are prevalent and important causes of sleep disturbance. Snoring, historically considered to be only a habitual annoyance, has significant physical and social consequences. OSA is now considered to be a major public health concern with significant morbidity and mortality. CPAP is considered the treatment of choice for OSA syndrome, but poor patient acceptance and compliance remain problematic. Surgical procedures have been developed to alter the offending anatomic abnormalities responsible for OSA. Identification of the offending anatomic site with application of the m

Subjective results after uvulo-palato-pharyngoplasty.

The subjective effect of uvulo-palato-pharyngoplasty surgery (U.P.P.P) for snoring was evaluated in 31 patients. Results were obtained following a questionnaire sent to patients after a median follow-up of 10 months, as well as through follow-up visits. Seventy seven point five percent of the patients and their partners were satisfied with the operation. No major complications were noted. Minor complications included pain, transient pharyngonasal reflux and sensitvity problems of the posterior wall of the pharynx.

Reduced nasal airway resistance following uvulopalatoplasty.

Active anterior rhinomanometry was performed on adult healthy snorers before and after uvulopalatopharyngoplasty or laser uvulopalatoplasty. Significant reduction of the nasal airway resistance both before and after pharmacological decongestion of the nasal mucosa was found in a group of 46 patients. Oedema disappearing after surgery may be an explanation for the results.

Uvulitis caused by anaerobic bacteria.

OBJECTIVE: I present two children with bacteremic uvulitis due to anaerobic bacteria. RESULTS: Fusobacterium nucleatum was recovered from the blood, and Haemophilus influenzae type b was recovered from a surface uvular culture of one patient. beta-Lactamase-producing Prevotella intermedia was isolated for the blood of the other patient. Both patients responded to parenteral, followed by oral, antimicrobial therapy. CONCLUSIONS: These findings illustrate the need to send blood cultures for both aerobic and anaerobic bacteria in patients with uvulitis.

Snoring--a simple surgical solution.

In this paper a new palatal surgical technique for the treatment of snoring is described and evaluated. All 22 patients were severe snorers with no significant obstructive sleep apnoea, as confirmed by detailed sleep studies. The aim of the surgical technique was to eliminate the vibration of the soft palate and the posterior pillars which occurs during snoring. This was achieved under general anaesthetic by making a 1.5 cm vertical full thickness incision in the soft palate from the superior pole of the tonsil towards the pterygoid hamulus using monopolar cutting diathermy. Specifically desig

Laser-assisted uvulopalatoplasty for snoring and obstructive sleep apnea: results in 170 patients.

Laser-assisted uvulopalatoplasty (LAUP) is a procedure for the treatment of snoring and, possibly, sleep apnea. Between July 1993 and December 1994, the authors prospectively evaluated 541 consecutive patients referred to their teaching hospital for possible LAUP to treat loud, disruptive snoring. Of these patients, 274 (51%) had one or more LAUP treatments. As of January 1995, LAUP treatment courses were completed in 170 patients. In this paper, the authors report preliminary results on the use of LAUP in these patients. Among the 170 patients, 105 (62%) had a diagnosis of snoring and 65 (38%

[Experience with surgical therapy in snoring and sleep apnea].

Knowledge of the connection between habitual snoring diseases and obstructive sleep apnoea has stimulated research into sleep disturbance in patients at high risk of cardiovascular disease. Any disturbance of nasal ventilation leads to a higher resistance to breathing and may cause cardiovascular complications in the long run. Anatomical and functional conditions that lead to intermittent incomplete blockage or complete occlusion of the upper respiratory tract must be detected and surgically eliminated, if possible. Surgical procedures for treating snoring and obstructive sleep apnoea may be s
