Persistence of abnormal global longitudinal strain in women with peripartum cardiomyopathy.

OBJECTIVES: Data regarding the longitudinal relationship of global longitudinal strain (GLS) and echocardiographic parameters are lacking in peripartum cardiomyopathy (PPCM). We evaluated GLS and its correlation with change (∆) in left ventricular ejection fraction (LVEF). METHODS: We retrospectively identified women age ≥16 years hospitalized at Montefiore Medical Center in Bronx, NY from 1999-2015 with International Statistical Classification of Diseases and Related Health Problems, 9th revision codes for PPCM or an occurrence of unexplained heart failure during or up to 5 months postpartum

Shear Wave Elastography Using High-Frame-Rate Imaging in the Follow-Up of Heart Transplantation Recipients.

OBJECTIVES: The purpose of this study was to investigate whether propagation velocities of naturally occurring shear waves (SWs) at mitral valve closure (MVC) increase with the degree of diffuse myocardial injury (DMI) and with invasively determined LV filling pressures as a reflection of an increase in myocardial stiffness in heart transplantation (HTx) recipients. BACKGROUND: After orthotopic HTx, allografts undergo DMI that contributes to functional impairment, especially to increased passive myocardial stiffness, which is an important pathophysiological determinant of left ventricular (LV

Epidemiology of left ventricular hypertrophy in hypertension: implications for the clinic.

INTRODUCTION: Left ventricular hypertrophy (LVH) is a common complication to hypertension, indicating the presence of hypertensive heart disease, which puts the patient at a very high risk for subsequent clinical cardiovascular events like sudden cardiac death, stroke, myocardial infarction and heart failure. AREAS COVERED: The epidemiology of LVH has changed in recent years as a consequence of the development of new diagnostic tools and demographic changes in hypertensive populations. Expert commentary: In individual hypertensive patients, the presence and type of LVH and associated systolic

Selective pacing sites.

he right ventricular apex (RVA) has always been the most used pacing site, because it is easily accessible and provides a stable lead position with a low dislodgment rate. However, it is well-known that long-term right ventricular apical pacing may have deleterious effects on left ventricular function by inducing a iatrogenic left bundle branch block, which can have strong influences on the left ventricle hemodynamic performances. More specifically, RVA pacing causes abnormal contraction patterns and the consequent dyssynchrony may cause myocardial perfusion defects, histopathological alterati

The segmentation of the left ventricle of the heart from ultrasound data using deep learning architectures and derivative-based search methods.

We present a new supervised learning model designed for the automatic segmentation of the left ventricle (LV) of the heart in ultrasound images. We address the following problems inherent to supervised learning models: 1) the need of a large set of training images; 2) robustness to imaging conditions not present in the training data; and 3) complex search process. The innovations of our approach reside in a formulation that decouples the rigid and nonrigid detections, deep learning methods that model the appearance of the LV, and efficient derivative-based search algorithms. The functionality

Tissue Doppler imaging in the evaluation of left ventricular function in young adults with autosomal dominant polycystic kidney disease.

BACKGROUND: Hypertension and increased left ventricular mass index (LVMI) have been reported in patients with early stages of autosomal dominant polycystic kidney disease (ADPKD). Whether these abnormalities are associated with diastolic dysfunction in this stage remains to be established. The aim of the study is to evaluate diastolic function in young normotensive patients with ADPKD by using tissue Doppler imaging (TDI), the most sensitive method available to date. METHODS: Thirty-two young clinically normotensive patients aged 21 to 30 years were compared with 23 controls with similar ages

Intra-ventricular resynchronization for optimal left ventricular function during pacing in experimental left bundle branch block.

OBJECTIVES: We sought to investigate to what extent intra-ventricular asynchrony (intraVA) and inter-ventricular asynchrony (interVA) determine left ventricular (LV) function in canine hearts with left bundle branch block (LBBB) during ventricular pacing. BACKGROUND: Pacing therapy improves LV pump function in patients with heart failure and abnormal ventricular conduction supposedly due to resynchronization. However, the relationship between LV pump function and measures of asynchrony is not well established. METHODS: In 15 experiments, LV (various sites) and biventricular (BiV) pacing was

Early impairment of right ventricular systolic function in patients with prediabetes and type 2 diabetes mellitus: An analysis of two-dimensional speckle tracking echocardiography.

BACKGROUND: Type 2 diabetes mellitus is a metabolic disease that affects multiple target organs. Current data on right ventricular damage in type 2 diabetes, especially in prediabetes, are limited. Due to the anatomical characteristics of the right ventricle, the assessment of the right ventricle by conventional echocardiography is difficult, whereas the ultrasound two-dimensional speckle tracking echocardiography can provide information on myocardial systolic function by tracking the motion information of myocardial speckles, which can sensitively reflect myocardial mechanical changes. AIMS:

Right and left ventricular function and myocardial scarring in adult patients with sickle cell disease: a comprehensive magnetic resonance assessment of hepatic and myocardial iron overload.

BACKGROUND: Patients with Sickle cell disease (SCD) who receive regular transfusions are at risk for developing cardiac toxicity from iron overload. The aim of this study was to assess right and left cardiac volumes and function, late gadolinium enhancement (LGE) and iron deposits in patients with SCD using CMR, correlating these values with transfusion burden, ferritin and hemoglobin levels. METHODS: Thirty patients with SCD older than 20 years of age were studied in a 1.5 T scanner and compared to age- and sex-matched normal controls. Patients underwent analysis of biventricular volumes and

The effect of left ventricular size on right ventricular hemodynamics in pediatric survivors with hypoplastic left heart syndrome.

BACKGROUND: Outcome status in patients with hypoplastic left heart syndrome (HLHS) is partially dependent on right ventricular (RV) systolic function. In other disease states, ventricular function is impacted by anatomy and physiology of the contralateral ventricle. In HLHS, it is suggested that a relatively larger left ventricular (LV) size may negatively impact RV function because it becomes a "passenger" without providing any systolic or diastolic physiologic benefit. The purpose of this study was to determine whether LV size adversely affects RV systolic function in surviving patients with
